1 Overview: The UCSD Proposal Signature Page was designed to be used for General Campus, Health Sciences, and Scripps Institution of Oceanography (SIO) proposals being submitted to funding agencies that do not have their own specific Signature Page. This form provides essential basic information about a proposed project such as: UCSD # Principal Investigator and (if applicable) Co-Principal Investigator Agency Name, Contact Name, and Street Address Project Title Project Begin and End Dates Total Costs Requested Addresses for Mailing or Courier for subsequent award documents that need to be sent In addition, signatures are required by the Principal Investigator and, if applicable, the Co-Principal Investigator, as well as the OCGA or SIO Contract and Grant office official authorized to sign on behalf of The Regents. Please Note: Should there be more than one Co-Principal Investigator, please use the UCSD Proposal Signature Page 2, which was designed and created solely for that purpose. Principal Investigator / Co-Principal Investigator: UCSD #: List the UCSD # for this proposal. PI Last Name: List the Last Name of the Principal Investigator. PI First Name: List the First Name of the Principal Investigator. PI Title: List the official Payroll Title of the Principal Investigator. If you are not certain what the title is, please either contact your Department/ORU Business Office or the University s Payroll Department. PI Department/ORU: List the Department/ORU the proposal is being submitted from. This is the Department/ORU that will administer the project. PI Mail Code: List the Department/ORU mail code for the Principal Investigator. Office of Contract and Grant Administration 1

2 Principal Investigator / Co-Principal Investigator - Continued: PI Phone #: List the phone number for the Principal Investigator. PI Fax #: List the fax number for the Principal Investigator. PI List the address for the Principal Investigator. Co-PI Last Name: List the Last Name of the Co-Principal Investigator. If there is more than one Co- Principal Investigator, please use the UCSD Proposal Signature Page 2, which was designed and created solely for that purpose. Co-PI First Name: List the First Name of the Co-Principal Investigator. Some agencies such as NASA and some Non Profit Agencies do not allow Co-Principal Investigators. Co-PI Title: List the official Payroll Title of the Co-Principal Investigator. If you are not certain what the title is, please either contact your Department/ORU Business Office or the University s Payroll Department. Co-PI Department/ORU: List the Department/ORU in which the Co-PI has their academic appointment. This may or may not be the same as that of the PI. Co-PI Mail Code: List the department mail code for the Co-Principal Investigator Co-PI Phone #: List the phone number for the Co-Principal Investigator. Co-PI Fax #: List the fax number for the Co-Principal Investigator. Co-PI List the address for the Co-Principal Investigator. Office of Contract and Grant Administration 2

3 Agency Information: UCSD Proposal Signature Page Agency Name: List the name of the funding agency that the proposal will be submitted to. If the proposal is for a subaward contract or subaward grant, please provide the name of the subawarding agency rather than the prime agency. For example, if the proposal is being submitted to another University, that University would be our agency, who happens to be receiving their funding from another (i.e. "prime") funding source. Please also include, (in parenthesis), the name of the Prime funding agency. Contact Name: List the name of a contact person at the agency to which the proposal is being submitted. This is very important both for mailing purposes and for any clarification regarding the proposal. Contact Phone: List the phone number for the Contact Name at the agency. Street Address: List the complete street address of the agency the proposal is being submitted to. Do not use P.O. Boxes since courier delivery services, such as UPS and FedEx will only deliver to a street address. City, State, Zip: List the city, state, and zip code of the agency the proposal is being submitted to. If applicable, include the following information: List any additional information that the agency may require about the University, such as the Award # the PA/RFA/RFP, etc. #, and any Other required agency information, such as DUNS Number, Taxpayer Identification Number, Institutional Assurance Numbers for Human and/or Animal Subjects, name of the University s Cognizant Federal Audit Agency, etc. Proposal Title: Proposal Title: List the title for the proposal. Be as brief and concise as possible. The Principal Investigator will usually furnish this information. Be sure to check the funding agency guidelines for any possible requirements on length and content, as some agencies are very specific about this information. 4 Project Dates / Total Costs Requested: Project Begin Date: List the anticipated starting date for the project. This is the date that should be used to estimate project expenses for the budget. Agency guidelines may determine the earliest start date that can be selected. Project End Date: List the anticipated ending date for the project. This is the date that would be at the end of the entire project. Total Costs Requested: List the total costs for the entire project. This is the sum of the total direct costs plus the total indirect costs. Signatures: Principal Investigator Signature: The Principal Investigator will sign their name. Date: Enter the date when the form was signed. Co-Principal Investigator Signature: The Co-Principal Investigator will sign their name. Date: Enter the date when the form was signed.
