Do not include jurisdictional monies spent on the project prior to the submission of this application. In kind contributions may be included as part of your jurisdictional commitment, but a heavy reliance on such funding is discouraged. B. Implementation Schedule Attach an IMPLEMENTATION SCHEDULE delineating major tasks to be accomplished and the time frame for accomplishments must be submitted with your grant application This project can be completed within 12 months of notification of award. * Please note: RTA will continue to provide for bus signage, concrete aprons, benches, cap and benches, shelters, concrete bus pads, and trash receptacles at locations meeting RTA Customer Amenity Program criteria. (See copy attached.) D. Project Summary Briefly SUMMARIZE the proposed project in 500 words or less. Include: 1) a specific description of the project, including its relationship to transit, 2) an explanation of how this project will positively impact transit, and/or Montgomery County, and 3) an explanation of why RTA funds are essential for the success of the project. 5 Detailed Instructions for Completing Application I. APPLICANT INFORMATION, Application Page 1. A. Certification. Each application must be signed and dated by the chief elected official, administrator or authorized individual of the jurisdiction who will be responsible for project compliance of all rules in the grant. Please note that the RTA reserves the right to request any applicant to provide a legal opinion that such project would constitute a legally permissible expenditure of public funds. Such opinion, if required, shall be at the applicant s expense. Also, RTA has an overall Disadvantaged Business Enterprise (DBE) goal of 10 percent. Please indicate how you plan to meet the goal under Section IV (E) of the application. Appropriate documentation shall be provided to the RTA for review and approval. B. Name of jurisdiction. Give complete name and address of the jurisdiction. C. Contact Person. Provide the name, title, telephone number, fax number and e- mail address of the person who RTA can contact if there are questions about the application. D. Name and Address of Public/Private Entity. If a public or private entity, physically located in the jurisdiction named in Section A, is applying jointly, provide their name and address. Grant funds may be requested by a jurisdiction in cooperation with/for a public or private entity. A letter of intent or resolution from this party confirming their involvement should be included with the application. E. Name of Project. Choose a name to describe your project. F. Indicate Priority for this Project. If more than one grant application is submitted, the applicant is to prioritize all of its applications and note that order here. G. Amount of RTA Funds Requested. List the amount of funding being requested. RTA requests, in accordance with the Ohio Revised Code, Section , that any expense over $100,000 be publicly bid, and contracts under that amount receive three (3) quotes. If a jurisdiction s bidding rules are more restrictive than that set forth in ORC 303, the more restrictive requirements shall be followed. Appropriate documentation shall be provided to the RTA for review and approval. II. PROJECT FUNDING, Application Page 2. A. Jurisdictional Funding. Indicate the total amount of cash and invoice cost funding that will be provided by your jurisdiction from the award date onward, excluding any costs incurred prior to the date the grant award is made. B. External Funding. Indicate the amount of external funding committed to this project. Attach a detailed summary of all private, non-profit or other government

6 sources, and include letters of award notification from each. Include estimates for funding not yet awarded. C. Program Budget Implementation Schedule. An attachment will be required to outline all major expenditures for the project, and the funding source for each. If the applicant can check both boxes, then this project meets two (2) important policy requirements: project leverages private, non-profit, and/or other government monies, and the project does not increase RTA s operating expenses. III. PROGRAM CRITERIA, Application Pages 2-3. A. To determine ways in which the proposed project matches RTA s program guidelines and criteria, applicants shall answer all nine (9) questions as completely as possible. If the question does not apply, write N/A. Since additional space may be required for some answers, please feel free to provide questions and answers on separate paper. Use all the space you need to complete each response. The following discussion may assist you in responding to the questions. Where possible, please estimate the impact of the proposed project in terms of increased passenger safety, jobs created or retained due to improved access to transit, or other relevant impact measurement. Also, you may estimate indirect benefits. 1. Improve safety/accessibility of transit Does the proposed project improve safe access to/from an existing bus stop or to/from a specific facility served by a bus route? 2. Increase transit ridership/market effectiveness Does the proposed project improve ridership for the RTA s system by serving a facility that employs or houses a transit-dependent population? Does the proposed project serve a particular transit market segment such as senior citizens, visitors to a major medical facility, employees/customers of a shopping plaza or major employment site, students to a school, etc? 3. Reduce RTA operating costs Does the proposed project reduce RTA s operating expenses by streamlining an existing route alignment or help facilitate the reduction of running time of a particular route? 4. Improve mobility of persons Does the proposed project make it possible for citizens (especially those with disabilities) to safely and conveniently utilize the RTA fixed route system? 5. Leverage external funding Does the proposed project have the potential to receive funding from outside sources, or enhance an existing project proposal?

Link transit to current or planned major developments Does the proposed project provide connections such as bikeways, pathways, parking lot access, etc. between transit service and major developments such as shopping centers, office parks, industrial or medical facilities, etc?. Encourage partnership applicants Does the proposed project encourage joint efforts among two or more entities? This could be two (or more) jurisdictions or a public or private entity, working together with its jurisdiction, etc.? IV. PROJECT DESCRIPTION, Application Page 4. A. Project Location. The address of the proposed project is provided. If the project is within RTA s current service area, please check the box. Photos of the proposed project site must be provided. B. Type of Project. Mark where appropriate. The footnote reminds the community that RTA will continue to provide amenities in accordance with the RTA Customer Amenity Program, for example, RTA shelters may be placed at bus stops which have a minimum of 50 daily boarding s, etc. C. Implementation Schedule. Applicants must attach a separate page listing major tasks and the projected time frame for completion of each task, and provide evidence of ability to finish the project within the required time schedule. If the applicant can check the box, this project meets one of the policy guidelines: project can be completed within 12 months of notification of grant award. D. Project Summary. In 500 words or less please describe the project and explain how it will make a positive (transit) impact within the RTA service area and why RTA funding is needed. Use additional paper and attach to the grant application if necessary. E. Additional Comments. In 500 words or less please attach any comments you wish.
