3 Introduction Delivering the right care at the right time in the right setting is the core mission of all hospitals across the country. In the Mater Misericordiae University Hospital, we are committed to safeguarding our mission and facilitating our staff to continually improve the quality of care delivered to our patients. We create a policy driven environment, the bedrock on which continual quality and safety measures thrive, through the provision of information and assistance to our staff in close collaboration with the Department of Health & Children, HSE, regulators and research agencies. The aim of this conference entitled New Frontiers, New Challenges is to provide attendees with patient safety and quality improvement information and to provide the opportunity to share knowledge and ideas. We hope that by participating in our conference attendees will have the opportunity to ensure the practical application of quality and safety measures in all aspects of their professional lives therefore improving the patients' experience. I would like to acknowledge and thank the multi-professional (medical, nursing, pharmacy and legal) conference speakers in attendance today and to personally acknowledge the work of the Mater s risk governance, risk management and drug safety committees for their continued work in the area of risk management and patient safety. 6 Biographies Dr. Philip Crowley is National Director of Quality and Patient Safety with the HSE since January Philip qualified from UCD medical school in After training in General Practice he worked for five years in Nicaragua, Central America, setting up chronic disease health programmes and working with grassroots community organisations in health development. He then worked in Newcastle upon Tyne and developed the Community Action on Health initiative using a community development approach to creating links between marginalised communities, professional groups and the health service with a view to tackling health inequalities. He has trained in public health medicine and has worked with the Institute of Public Health in Ireland and as Project Director for the General Practice in a Multi-cultural Society project and the General Practice Health Inequalities project with the Irish College of General Practitioners. Philip then worked general practice in the North inner-city of Dublin until he took up the post of Deputy Chief Medical Officer with the Department of Health and Children which he held until taking up his post with the HSE. In his position of National Director, Quality and Patient Safety, Philip has responsibility for: quality improvement, risk management, service user involvement and advocacy, management of serious adverse incidents, national audit, patient safety programmes and the relationship with HIQA and the State Claims Agency. Mr. Brian Conlan FCMA is the Chief Executive of the Mater Misericordiae University Hospital, a large academic teaching Hospital located in North Inner City Dublin. As a tertiary referral acute hospital with in excess of 600 beds the Mater provides services in the area of Cardiothoracic Surgery including Transplantation, Orthopaedic/Spinal Injuries, Vascular Surgery, General Surgery, Plastic, ENT and Ophthalmic Surgery. The Hospital is a major centre for the treatment of cancer in Ireland. Mr. Conlan is a Chartered Management Accountant, has occupied senior executive positions in Finance within Aer Lingus plc and Team Aer Lingus prior to
7 joining the Mater as Financial Controller in In 2000 Mr. Conlan assumed the position of Deputy Chief Executive and upon legal incorporation became a Director of the Mater company. In July 2005 he was appointed to the position of Chief Executive. Mr. Conlan is a Director of Mater Misericordiae University Hospital and is Chairman of the Hospital s subsidiary companies Suantrai Limited (hospital crèche), Mater Hospital Information Services Limited and Electrical Power Distribution Limited. He is also a Director of the Mater Foundation, Mater Campus Hospital Development (MCHD) Limited, Eccles Street Car Park Limited (ECPL), the Central Remedial Clinic, and the Hospital Procurement Services Group. On behalf of the Dublin Academic Teaching Hospitals, Mr. Conlan chairs the Dublin Hospital Group Insurance Management Committee, and he represents DATHS at the National Clinical Audit Committee, the Clinical Advisory Group to HCAI and AMR and Standards Advisory Group, National Standards for Better Healthcare. From 2003 to 2008, Mr. Conlan was a surveyor with the IHSAB accreditation body completing 8 surveys in public hospitals in Ireland. Ms. Hilary Coates Head of Safety and Learning, Healthcare Quality and Safety Directorate, Health Information and Quality Authority Hilary is the Head of Safety and Learning in the Health Information and Quality Authority. In this role, she leads the Authority s monitoring, inquiry and investigation programmes. Hilary joined the Authority from the World Health Organization s World Alliance for Patient Safety where she worked as a patient safety advisor to the Safe Surgery Save Lives and Clean Care is Safer Care programmes and led on the Alliance s evaluation and monitoring approaches. Prior to commencing work with WHO, Hilary worked as a Programme Director at the Royal College of Surgeons in Ireland where she developed and directed the Quality and Safety, and Leadership programmes. Mrs. Loretta Evans, mother of Colin Evans (RIP) Mrs. Evans will be speaking about her personal experience and interaction with the Mater and the lessons learned.
8 Ms. Angela Tysall SRN. RM. Clinical Nurse Specialist in Asthma, Ear care and Minor Illness. Extended Independent Nurse Prescriber. Currently working as Project Manager in the National Advocacy Unit and the HSE lead in Open Disclosure. Also work as a complaints review officer for the HSE - 13 years experience as a midwife - 2 years experience as a practice nurse - 5 years experience as senior sister, team leader and then acting manager of a NHS nurse lead Walk-in Centre - 6 years management experience in a GP OOH service in the North West of Ireland Ms. Jennifer Brown, Clinical Pharmacy Services Manager (MMUH) Jennifer is a Chief II Pharmacist and has managed the clinical pharmacy service in the Mater Misericordiae University Hospital for 10 years. This service has medication safety at the core of provision. As part of a Pharmacy Department medication safety team, in conjunction with the hospital Drug Safety Committee, Jennifer has worked on the development and implementation of the award winning MMUH Medication Safety e learning Programmes. Ms. Hilda Dowler Hilda is an Assistant Director of Nursing for Nursing Quality and additionally is the Key Performance Indicator Co-ordinator in the Mater Misericordiae University Hospital. In her current role Hilda works in collaboration with nursing personnel identifying 'performance improvement' initiatives creating an environment on which quality and safety can thrive, improving the quality of care delivered to patients. In partnership with Professor Tanya McCance, Professor for Nursing Research and Development, which is a joint appointment between the University of Ulster and the Belfast Health and Social Care Trust, Hilda coordinated a research project
9 titled Identifying Key Performance Indicators in Nursing & Midwifery in the Mater Misericordiae University Hospital. Hilda s professional career includes working in the area of occupational health in both the public and private sectors. Hilda trained in Connolly Memorial Hospital, Blanchardstown and completed a post grad qualification BSc in Occupational Safety and Health Management. Hilda is currently undertaking an MSC in Quality & Safety in Healthcare. Ms. Una Cunningham Previously the SLT Manager at the Mater and Chairperson of the Irish Association of Speech and Language Therapy Managers, Una has been seconded into the role of Local Programme Manager for the implementation of the National Clinical Care Programmes at the Mater. Una is also a member of the National Stroke Programme team with a specific remit for development of early supported discharge services nationally. Professional interests include, change management and community hospital service interfacing. Dr. Deirdre Madden, Senior Lecturer in Law in University College Cork Dr. Deirdre Madden is a Senior Lecturer in Law in University College Cork with research interests and publications in the area of medical law. She was been a member of the Medical Council since 2004 and has been a member of the Fitness to Practice Committee, Chairperson of the Standards in Practice Committee and Chairperson of the Ethics Working Group of the Council. In 2005 she was appointed by the Minister for Health to write a final report on post mortem practice and procedure, subsequently known as the Madden Report. Dr. Madden was also appointed Chairperson of the Commission on Patient Safety and Quality Assurance which published its Report, entitled Building a Culture of Patient Safety, in July Dr. Madden is a current member of a number of national working groups and international research projects related to medical law and ethics. She is also a regular conference speaker and media commentator on medical law issues.
10 Mr. Philip Nolan, Partner, Mason Hayes + Curran Philip is a Partner and Head of the Commercial Department at Mason Hayes & Curran. Philip advises both public and private clients in relation to communications, information technology, intellectual property and competition law. Philip has also developed a focused regulatory advisory practice which includes public procurement law, data protection law and energy law. Philip has co-authored four editions of Business Law published by Oxford University Press in conjunction with the Irish Law Society's Law School. He also co-authored a book entitled Irish Information Technology Law published by Cavendish. Philip is actively involved in a number of international organisations including the Association Internationale des Jeunes Avocats (AIJA) and the International Technology Law Association. He previously served as President of the Technology, Media and Intellectual Property Commission of AIJA. Philip has served as a member of the Editorial Board of the Gazette of the Law Society of Ireland and the Curriculum Development Unit of the Law Society of Ireland.
