Organisational Area Ireland East Hospital Group Context/ The reform actions necessary to enable the Government to deliver whole system health Background reform, as undertaken within the Programme for Government, are laid out in Future Health: A Strategic Framework for Reform of the Health Service The establishment of Hospital Groups is an integral part of this reform, which will see public hospitals reorganised into more efficient and accountable hospital groups that will harness the benefits of increase independence and greater control at local level. The Government s decision regarding the establishment and operation of the new hospital groups is informed by two reports The Establishment of Hospital Groups as a Transition to Independent Hospital Trusts and The Framework for Development Securing the Future of Smaller Hospitals. These reports fulfil important commitments in Future Health for the reform of the health service. The overarching aim of the wider health system reform programme is to deliver a single-tier health system based on Universal Health Insurance (UHI), underpinned by the principle of social solidarity, with equitable access based on need and not on ability to pay. In preparation for the introduction of UHI, a new financing system, (Activity based funding will be introduced. The alignment of ABF with the national clinical programmes will provide Hospital Groups with the tools to provide effective and efficient services within a clear national framework. Initially the Hospital Groups will be established on a non-statutory administrative basis with an Interim Group Board appointed by the Minister for Health overseeing the delivery of high quality, safe patient care. In the first year of operation each Hospital Group will develop a strategic plan for future service configuration. These plans will describe the provision of more efficient and effective patient services; how the services will be reorganised to provide optimal care to the respective populations; and the plans to achieve maximum integration and synergy with other Hospital Groups and all other health services, particularly primary and community care services. Each Group will have a single consolidated executive management team, with responsibility for performance and outcomes, within a clearly-defined budget and employment ceiling. The executive management team will have autonomy to reconfigure services across the group, including the deployment and redeployment of all staff, subject to an agreed policy framework and approval process. In addition each Hospital Group will have a primary academic partner which will ensure the capability of the Group to delivery on healthcare teaching, training and research and innovation agenda in an integrated manner. The establishment of Hospital Groups and, subsequently, Hospital Trusts, will enable hospitals to provide care in the right way at the right location. This will be done in a manner that ensures a safe, high quality service for all, maximising and capitalising on the strengths of both larger and smaller hospitals, with better outcome for patients. Following an agreed period of time rigorous evaluation of each group will be undertaken to ensure it is fit for purpose to function as a Hospital Trust. Legislation will be put in place to enable Independent Hospital Trusts to be established taking account of any changes to Groups which may emerge from the review process. 3 Reporting Arrangements Key Working Relationships The Group Assistant Director of Nursing / Group Sepsis will report directly to the Group Chief Director of Nursing & Midwifery or designate of the Ireland East Hospital Group The ADON, on behalf of the Hospital Group will represent the programme nationally and be a point of contact between the National Sepsis Management Team and the Hospital Group. The ADON will also: Work, in a leadership capacity, with the National Lead, Group Chief Director of Nursing and Midwifery, local executive management teams (in particular the Directors of Nursing, Clinical Directors and local sepsis team) and local change groups. While providing intensive programme and change management support to sites, the ADON will be required to work collaboratively with other interdependent clinical programmes and their clinical and nurse leads; the relevant Divisions of the HSE Directorate to include the Quality Improvement Division to ensure a joined up approach to the implementation of the National Clinical Guideline for Sepsis Management: No 6. Liaise with the National Clinical Lead for Sepsis and any local Sepsis Management Teams Scope of the Role The Ireland East Hospital Group includes the following hospitals: Mater Misericordiae University Hospital St Columcille s Hospital Our Lady s Hospital, Navan Cappagh National Orthopaedic Hospital St Vincent s University Hospital Royal Victoria Eye and Ear Hospital Midland Regional Hospital Mullingar St Michael s Hospital, Dun Laoghaire Wexford General Hospital National Maternity Hospital St Luke s General Hospital, Kilkenny Sepsis, severe sepsis and septic shock are used to describe the systemic inflammatory response of patients to infection, as a continuum of progressive and life threatening severity. Sepsis is the clinical syndrome defined by the presence of both infection and a systemic inflammatory response. The aim of the National Clinical Guideline on Sepsis Management: No 6 is to facilitate the early recognition and appropriate treatment of sepsis in Ireland in order to maximise survival opportunity and minimise the burden of chronic sequelae. There are recognized improved outcomes associated with early appropriate therapy. The guideline includes a care pathway for sepsis and an adaptation of the National Clinical Guideline on Sepsis Management: No 6 for patients with severe sepsis/ septic shock. This guideline has been produced in response to growing national and international awareness of the increasing incidence of sepsis. The Guideline Development Group recommends the Surviving Sepsis Campaign Guidelines and the Sepsis 6 bundle as the guide to the management of sepsis in Ireland. This guideline outlines: Broad measures that should be put in place for sepsis screening and subsequent Group Assistant Director of Nursing / Group Sepsis Page 3 of 8
4 action once sepsis is detected. Key local, regional and national measures, including balancing measures that should be monitored to track improvements in sepsis management This National Clinical Guideline is part of a suite of guidelines that relate to the acutely deteriorating patient. National Clinical Guidelines include: National Clinical Guideline No. 1 National Early Warning Score (NEWS) National Clinical Guideline No. 4 Irish Maternity Early Warning System (IMEWS) National Clinical Guideline No. 5 Communication (Clinical Handover) in Maternity Setting National Clinical Guideline - Paediatric Early Warning System (PEWS) in process National Clinical Guideline - Clinical Handover in acute hospitals in process Purpose of the Role The Acute Hospitals Division, National Clinical Effectiveness Committee and National Lead Sepsis require an ADON working at Hospital group level to drive and support the implementation of the National Clinical Guideline on Sepsis Management. The ADONs role involves working across acute hospitals within the group engaging with staff through formal and informal mechanisms. He /She will provide innovative multidisciplinary leadership, advice and guidance based on current best practice, and drive the implementation of the Sepsis Management Guideline through multidisciplinary collaboration, education, awareness raising, audit and feeding back successes, barriers and audit data to the National Sepsis Team. The purpose is to inform and guide healthcare staff in the recognition, treatment and appropriate referral of patients with sepsis with the intention of: Improving sepsis recognition Ensuring that staff are educated to recognise patients with sepsis have Sepsis 6 performed within one hour of sepsis recognition, identify patients with severe sepsis/septic shock and escalate care as appropriate Reducing mortality and morbidity from sepsis in Ireland Reducing the economic burden of sepsis Principal Duties and Responsibilities Work in collaboration with the national clinical lead, other nurse leads for sepsis management and with the Hospital Group Management Team and staff to create awareness and recognition of the principles underpinning the National Clinical Guideline for Sepsis Management at national, regional and local level. Lead out on the clinical, professional and practice contribution of multidisciplinary staff to the ongoing implementation and evaluation of the key drivers primarily (but not exclusively) focusing on the following areas: Early identification of septic patients Ensuring best practices in the ED and the AMAU Seamless transitions of care. Support the implementation of guidelines and pathways for the management of sepsis. Present reports to the National Sepsis Team and Steering Committee that will form part of an annual report to the Director of the Clinical Strategy and Programmes Division. Identify Key Performance Indicators for Sepsis Management in conjunction with the Sepsis Management Team and establish an audit schedule to monitor trends. Agree a reporting schedule with the National Lead for Sepsis Management. Group Assistant Director of Nursing / Group Sepsis Page 4 of 8
5 The presence of a sepsis screening form, signed by the treating doctor, in the patient s case notes for the admission due to or complicated by sepsis, will be a key performance indicator as a measure of guideline compliance. Reporting on this KPI will start in This is the responsibility of the treating physician, but its implementation will be supported by education and audit provided by the website and, in part, by the sepsis ADON. Liaise with hospitals and community services regarding sepsis management communicating the benefits of programme solutions and managing stakeholder buy in to the solution and provide advice and support. Provide advice and support to the National Clinical Lead on sepsis management issues escalated to the local Sepsis Management Team /committees. Liaise with the National Clinical Lead, other group lead nurses to promote alignment and integration of National Clinical Guideline on Sepsis Management. Attend National meetings and forums at the request of the National Lead Sepsis, and work collaboratively on sepsis at the group /regional and national basis. Link with the relevant Divisions in the Health Services Directorate, the Quality Improvement, the Special Delivery Unit, the Office of Nursing and Midwifery Services Director (ONMSD), Hospital Groups and Community Health Care Organisation areas to support and promote an integrated approach to the implementation National Clinical Guideline on Sepsis Management in a manner that enhances both quality and patient safety. Link with appropriate hospital committees such as Infection, Prevention and Control and key personnel to provide advice and direction in relation to education and training to support the implementation of the National Clinical Guideline on Sepsis Management. Participate in national programme meetings and key meetings between the programme and sites. Provide an education role by creating/contributing to nursing & multidisciplinary forums/learning community that captures learning from existing local good practices. Use this learning to educate others and to update the overall model for the programme. Represent the Hospital Group regarding nursing issues related to Sepsis Management and maintain good collaborative relationships with external stakeholders e.g. CSPD. ONMSD, Relevant Government Departments, State Agencies, Regulatory Bodies, Higher Education Institutions. Have a working knowledge of the Health Information and Quality Authority (HIQA) Standards as they apply to the role for example, National Standards for Safer Better Healthcare and the Prevention and Control of Healthcare Associated Infections, Hygiene Standards etc. Maintain accurate and contemporaneous records / data on all matters pertaining to the planning, management, delivery and evaluation of this service in line with HSE requirements. To support, promote and actively participate in sustainable energy, water and waste initiatives to create a more sustainable, low carbon and efficient health service. 6 Eligibility Criteria Qualifications and experience Candidates must on the latest date of receiving completed application forms for the office: a) Be registered in the General/Children s/mental Health or Midwifery Division of the Register of Nurses & Midwives maintained by An Bord Altranais agus Cnáimhseachais na héireann (Nursing and Midwifery Board of Ireland) or be entitled to be so registered and b) Have had experience in Nursing Management at CNM2 level or above and c) Have had satisfactory nursing experience and d) Candidates must possess the requisite knowledge and ability (including a high standard of suitability and management ability) for the proper discharge of the duties of the office. and e) Candidates must have undertaken training in management appropriate to the post. Desirable f) Possess a Quality Improvement Qualifications at Level 8 or higher (or other relevant equivalent qualification) in a relevant other suitable field. Other Requirements Candidates must have access to transport as the post will involve frequent travel.
