2 Registered Nurse () Role Description Matrix Primary Care Nurse Task Force Registered Nurse () Role Description Matrix Background: The Registered Nurses Association of Ontario (AO) is leading a ground-breaking task force that is bringing together key stakeholders to review the role of Primary Care Nurses (Registered Nurses and Registered Practical Nurses) in the delivery of quality and accessible primary care. Recommendations will aim at optimizing the utilization of the 2,873 s and 1,412 RPNs currently working in primary care by maximizing their scope of. The purpose of this matrix is to summarize the current roles and responsibilities of nurses practicing in a number of primary care settings and was compiled through existing job descriptions. Given the broad role of Primary Care Nurses, there may be elements that are incomplete or missing. Initiates health assessments including: o health history, o identification of illness screening needs, o immunization requirements/ recommendatio ns, and o periodic health exam visits for healthy clients and screening maneuvers (i.e. PAPs) under medical directives. Observes and records outcomes Analyses the findings of a health assessment Systematically assess patients physical and psychosocial needs. Conducts patient assessments. Measures and records patient vitals and prepare patient and room for the examination by the physician. Assess immunization status. Perform blood pressure follow-ups assessing response to medications, educating patients and promoting health through blood pressure management. Incorporates objective findings such as laboratory and diagnostic findings, specialist reports and past history in a comprehensive assessment. Applies and adapts recognized clinical guidelines to assessments. Conducts psychiatric nursing assessments and provide support as necessary, Conducts risk assessments in collaboration with the client and other service providers and develops appropriate health plans. Assesses the presence of or the need for advanced care directives or do not resuscitate documents. Assesses health education requirements and readiness of the client as a component of an overall health assessment. Establish a work plan including project charter, targeted priorities and accountabilities. Acts as a resource to clinical teams to support effective consultation and collaboration. Provides leadership for the review, planning and or implementation of existing or new clinical service delivery initiatives such: Ontario Telemedicine Network (OTN), Lung Health Service Delivery, Health and lifestyle change strategies that support selfmanagement such as Stanford chronic Disease Self- Management, Health Provides case management for clients. Monitors, evaluates and adjusts the health plan based on effectiveness of interventions and/or changes in condition or environment, in collaboration with the client and team members. Monitors client progress with respect to risk reduction and self management plans. Provides pertinent front-line information to facilitate health planning in the Champlain LHIN.
