1 JOB DESCRIPTION Position Title: Aboriginal Healthy Babies Healthy Children (AHBHC) Nurse Program: Health Service: Community Health Accountable to: Community Health Service Manager Issued By: Date Revised: August 2015 Date Issued: Classification: PURPOSE AND SUMMARY To ensure the development, provision and maintenance of a network of health and social service providers to ensure that Aboriginal families with children (prenatal to age 6) who are at risk of physical, mental and social problems have access to a range of prevention and early intervention services. DUTIES AND RESPONSIBILITIES 1. Service Responsibilities Under the direction of the Community Health Service Manager, the AHBHC Nurse is responsible for. Individuals and Families A. To ensure that high risk Aboriginal families with children (prenatal to age six) who would benefit from lay home visiting, have access to this service. B. Assessments/screening prenatal and at birth to identify those Aboriginal children at risk and ensuring that Aboriginal families receive further assessments if required. C. To link Aboriginal families with children (prenatal to age 6) who are at risk of physical, mental and social problems to appropriate supports and services in the community. D. To develop plans of care for Aboriginal children and their families. Aboriginal Healthy Babies Healthy Children Worker Page 1
2 E. To provide ongoing liaison service and follow up to ensure family and individual needs are being met. F. To know of and work with community agencies to support healthy Aboriginal families. G. To work with other agency programs and staff to promote a co-ordinated approach. H. To develop an inventory of services available within the community for prevention and early intervention services. Community A. To develop and maintain a network of health and social service providers to ensure that Aboriginal families with children Prenatal to age six) who are at risk of physical, mental and social problems have access to a range of prevention and early intervention services. B. To promote, by programming and personal example, the development of positive role models in the community. C. To develop strong linkages and partnerships with local Aboriginal and non-aboriginal service agencies. Create system and directory of referrals. D. To participate in the development and establishment of culturally sensitive services which address family healing and wellness. E. To provide cultural sensitivity training to Aboriginal and non-aboriginal individuals, groups, etc. Specifically related to the providing of culturally appropriate services. ADMINISTRATION A. To report regularly to the Community Health Service Manager. B. To prepare accurate activity and statistical reports and submit them as required to the Community Health Service Manager. C. To participate in any evaluation developed by Dilico and/or the Aboriginal Healing and Wellness Strategy for the Program. Aboriginal Healthy Babies Healthy Children Worker Page 2
3 D. Maintaining effective communications at all times, written and verbal. E. To attend meetings, workshops, seminars and training established for the Program. F. To undertake any other such reasonable duties as may be assigned from time to time. G. Ensuring that all confidentiality requirements are met as per policies and procedures. Organizational Responsibilities As a representative of Dilico, the Employee is responsible for: a. reflecting and interpreting the Agency Vision, Mission and Core Values in his/her own work with enthusiasm and commitment; b. acting in accordance with relevant legislation and Agency Policies, Standards and Procedures; c. proposing changes within Dilico that would improve the quality of service to Anishinabek children, families and communities; d. developing and maintaining respectful, cooperative working relationships to contribute to the integrated, seamless delivery of services to Anishinabek children, families and communities; e. understanding his/her role and responsibility in maintaining a safe workplace and reducing workplace injuries; f. applying Anishinabek culture, values, traditions and teachings into programming where possible; g. ensuring accuracy, confidentiality and safekeeping of agency records; h. participating constructively in the supervision process with the immediate Manager. 4 QUALIFICATIONS 1. Education A Baccalaureate degree in nursing is required as well as additional training pertaining to working with families, etc. 2. Work Experience Experience working with Aboriginal families and children required. Lay home visiting experience and mental health nursing background will be considered an asset. 2. Skills/Abilities a. sound knowledge of the Anishinabek culture and the principle issues affecting the mental health and health of Anishinabek children, families and communities in the City and the District of Thunder Bay; b. sound knowledge of the positive traditional influences that could promote mental health and wellness of Anishinabek children and families in the District of Thunder Bay. The ability to promote and apply Anishinabek Culture, values, traditions and teachings in the work of the Family Health Team where appropriate; c. developing and maintaining respectful, cooperative working relationships with Anishinabek children, families, communities and service workers to promote an integrated, seamless delivery of service; d. ability to understand or speak an Anishinabek language would be considered an asset; e. effective written and oral communication skills, problem solving, advocacy and assessment skills; f. ability to work within a multi-disciplinary team; g. ability to work within the policies, standards, procedures and the vision, mission and core values of Dilico Anishinabek Family Care; h. ability to provide coverage to all Health programs where appropriate training has been provided and where required qualifications, skills and abilities are met. CONDITIONS OF EMPLOYMENT a. satisfactory Criminal Records Search; b. must possess a valid Standard First Aid/CPR Certificate or be willing to obtain one;
5 Job Description c. current registration with College of Nurses (CNO) and Registered Nurses Association of Ontario (RNAO); d. travel is essential to the First Nation communities serviced by Dilico Health Services; ability to work flexible hours as determined by the Unit Manager or designate in relation to service delivery requirements e. must posses a valid Ontario Driver s Licence and have access to a personal vehicle; NOTE: This job description is not intended to be all-inclusive.
