3 Housing Congratulations on your assignment to Camp Shelby, Mississippi. We would like to provide you with some useful housing information to prepare you and your family for your move. We know that moving can be stressful and want to make your housing experience one you will remember in a positive manner. MARRIED SOLDIERS OR SOLDIER WITH DEPENDENTS May Qualify for Government Leased Housing. Government Housing on post is not available at Camp Shelby. This memorandum will provide guidance to assist you in a smooth transition. Housing guidance requires the Housing Office to assist Soldiers in locating off post private rental housing. We provide you the rate of your Basic Allowance Housing for the local area. We then add an additional 20% for out of pocket expenses to your BAH which could be used to pay things like rent, rental insurance, utilities. You are not given an extra 20%. This 20 percent is what the Army states is acceptable for one to expect to pay out of pocket above your BAH rate. We will provide you with the average cost of utilities, subtract it from the BAH plus 20% amount. You should search for a home that does not exceed this amount. You should then begin searching for housing in the local area for private rental. If you are able to find housing for that amount (BAH plus 20% minus the cost of utilities) or less you are required to rent housing off post. (If there is any housing available for this amount or less you are required to rent whatever housing you select.) If for some reason you or the Housing Office cannot locate any housing (houses or apartments) for this rate or less we will submit an application and request government leased Housing for you. Leased Housing is explained in paragraph 3. The Government Leased Housing Program are homes that the Army Corps of Engineers has leased in the communities surrounding Camp Shelby. As Soldiers arrive they complete the required housing documentation and we insure that you are eligible for Family Housing by following the guidance above. Once we determine we cannot find housing as describe above we submit an application to the Mobile District for approval to offer you a leased home on the program. Once approved you are shown homes, once you select your home we will do a walk through inspection, complete all necessary paperwork. The date you receive your keys to your leased house your BAH is stopped. Your BAH is forfeited. You pay for telephone and cable and any other amenities. If you do not like the homes we have available you are authorized to seek private rental housing. We can assist you with this search as well. (Note: Certain breeds of dogs are not allowed, including but not limited to Pit Bulls, Rottweiler s, and Doberman Pinchers, Wolf Hybrids or any canine breed with dominate traits towards aggression) 3

4 Housing SINGLE SOLDIERS WITHOUT FAMILY MEMBERS OR SOLIDERS THAT DO NOT QUALIFY FOR LEASED HOUSING If you are not authorized Family Leased Housing we will provide you with several resources. We will help you as much or as little as you desire. We will give you property management listings, websites and current rental listings. We are able to review listings with you and set up appointments for you with property managers. Please call us days prior to your arrival to Camp Shelby, MS. You will need to fill out the DD1746 Housing Application and it back to us along with a copy of your orders. This will allow us to project and prepare for your arrival. This is especially important if you require four bedroom or larger homes. There are a limited number of these homes throughout the area and we may need additional time to prepare for your arrival. (Note: Some aggressive breeds of pets and large animals are prohibited with many property managers if you rent on your own) Keep in mind that there is no perfect formula for qualifying for government leased housing because it is all based on the available rental market when you arrive to Camp Shelby. The ADDP provides authorized private sector dental benefits under two distinct components for Active Duty Service Members (ADSMs) who are either: Referred from a military dental treatment facility (DTF) (DTF-referred) or reside and work (duty location) greater than 50 miles from a military DTF as part of the Remote Active Duty Dental Program (Remote ADDP). United Concordia pays claims for authorized dental care. Treatment must be provided by a United Concordia network dentist in order to be eligible for payment. Eligibility Who is eligible? The ADDP is available to ADSMs of the Uniformed Services to include: o U.S. Army o National Guard and Reserve Component sponsors called or ordered to active duty for more than 30 consecutive days and Early Activators o Family members of active duty uniformed services personnel o Family members of National Guard and Reserve service members o Line of Duty (LOD) service members** Note: ** LOD documentation must be submitted to United Concordia prior to receipt of private sector dental care. Submission is NOT considered authorization. The Government will review the LOD paperwork and United Concordia will notify ADSMs of the determination. Eligibility Who is not eligible? o National Guard and Reserve service members who are not on active duty for more than 30 days o Former spouses o Parents and parents-in-law o Disabled veterans 13

14 Medical and Dental Care ADSM Dental Readiness United Concordia will track dental readiness classification for all remote ADSMs who utilize the ADDP. Civilian dentists will report the dental readiness of the remote ADSM on each claim submitted. Remind your dentist to include your Dental Readiness Classification on the claim form. ADSM Benefits The ADDP will only pay for covered dental benefits, non-covered dental benefits specifically authorized by the Government, and those dental benefits that require and have received authorization prior to receipt of dental care. If you elect to receive non-covered services, you will be responsible for payment. Routine Care As a remote ADSM, you may personally coordinate your routine covered dental procedures (e.g. examinations, cleanings, fillings) as long as: o Dental treatment is less than $750 per procedure or appointment or o The cumulative total is less than $1,500 for treatment plans completed within a consecutive 12-month period AND o The procedure must be a covered benefit as listed in The Active Duty Dental Benefit. (Available on the ADDP website): o An Appointment Control Number (ACN) provided by United Concordia must be obtained prior to receipt of care for all courses of treatment, regardless of cost. 14

15 Medical and Dental Care Specialty Care As a remote ADSM, you must receive authorization prior to receiving: o Specialty dental care (e.g., crowns, bridges, dentures, periodontal treatment)* o Dental care in excess of $750 per procedure or appointment o Dental care with a cumulative total greater than $1,500 for treatment plans completed within a consecutive 12-month period o Dental care from a non-network dentist o For implant specialty services, you will also need a Command Memorandum from your unit signed by the unit commander or designated representative. This should be submitted by the civilian dentist as an attachment with the completed Authorization Request Form. o Orthodontics is not an approved service for Remote Active Duty Service Members. Emergency Care Emergency dental care procedures under the ADDP for remote service members consist of the following: o Emergency dental care does not require an authorization or Appointment Control Number (ACN). o Emergency dental care includes any treatment necessary to relieve pain, treat infection, or control bleeding. Root canal treatment may be needed to relieve pain and infection, and is considered emergency dental care. o Crowns, bridges, and denture services are not considered emergency dental care and, therefore, are not covered. As such, ADSMs who elect to receive non-covered services as part of an episode of emergency dental care are responsible for payment of these services. It is recommended that you use a United Concordia network dentist for emergency dental care. Although this is not required, any follow-up care with a non-network dentist will not be authorized and you will be responsible for payment. 17 Medical and Dental Care Exceptional Family Member Program (EFMP) What is the program about? It's all about helping your family navigate the medical and educational system, so you can have more peace of mind about your family member's care. We assist by: o Identifying and enrolling family members with special medical or educational needs. o Finding out what services are available at your present or new duty station. o Supporting your family with information, referrals and non-clinical case management to access services. EMFP offers two important kinds of help The EFMP helps families in two big ways: by making sure special needs are considered during assignments, and by easing access to assistance wherever you are. o Assignment coordination: While military mission is always the driving force behind a service member's assignment, the Exceptional Family Member Program helps make sure that family members' documented needs are considered during relocations. This is important because access to appropriate medical and educational services may be limited in overseas and remote locations. With assignment coordination, you can be more assured that your family member's needs will be considered, so you can focus more clearly on missionrelated responsibilities. o Family support: This part of the program helps families identify and access programs and services. Family support providers can put you on the path to empowerment by: Providing information and referral services for both military and community services Helping you find and navigate programs in new locations. Giving "warm handoffs" to the Exceptional Family Member Program at new locations. Finding ways to bridge gaps in programs, services and supports Informing you about available local school and early intervention services Providing non-clinical case management, including individualized services plans Offering opportunities for families with special needs to connect Helping families understand what is offered, how to determine eligibility and how to apply for benefits and entitlements How do you get started? You get started by contacting Ms. Rita Kora at or rita.kora.civ@mail.mil More information on EFMP is available at and includes enrollment and screening information. 17
