Miller, CCIM, 2003 and 2015 CCIM Chapter President and a long time real estate veteran and Vice President of Commercial Title Insurance Services. Dave Miller has been in real estate for 42 years, 36 years in Commercial Title Insurance Sales. He began his career in California as a roofer, starting McCallon & Miller roofing in In 1976 Dave went into the Mortgage business. Prior to finishing at USC, in 1980 he opened Mortgage Consultants Company. In 1981 Dave was recruited into the title industry by Ticor Title in Los Angeles to work with attorneys and developers. Throughout the 80s and 90s Dave worked in the title and brokerage fields. The year 2000 brought him the title of VP/National Accounts and Commercial Sales Manager for Chicago Title; a position he still holds today. Dave has served on numerous boards and committees over the years including First Business Bank, NAIOP, CCIM, ACMLA, ICSC, BIA (CA), NACORE and the Men's Art Council (Phoenix Art Museum). In 2015 Dave was a recipient of the Phoenix Business Journal inaugural l Landmark Leader Award. He is the only person to ever receive the CCIM designation based on his title company experience. Dave enjoys fishing, golf, sporting clays, and played rugby for 30+ years. He is married with two sons and resides in Phoenix, Arizona. Background The DAVID S. MILLER ENDOWED Scholarship honors DAVE MILLER as a CCIM, PAST PRESIDENT and trusted expert in the real estate and title fields. The DAVID S. MILLER ENDOWED scholarship was created in 2016 by the CENTRAL ARIZONA CCIM Chapter with matching funds provided by the CCIM Foundation. Named endowed scholarships honor those who have shown great dedication, commitment, outstanding achievement and service to the commercial real estate industry. About the CENTRAL ARIZONA CCIM Chapter The Central Arizona CCIM Chapter promotes the highest ethical standards in commercial real estate, provides quality educational programs to members and related professionals, serves as a forum where intelligence and success can be shared and celebrated, and finally, enhances the awareness and recognition of the CCIM designation to the business community. About the CCIM Foundation The CCIM Foundation is a 501(c)(3) non-profit organization. The Mission of the CCIM Foundation is to advance and foster commercial real estate education through scholarships, programs and initiatives. Scholarship Program Guidelines - $1000 toward a CCIM Course The DAVID S. MILLER ENDOWED SCHOLARSHIP was created to support qualified real estate professionals in their efforts to achieve the CCIM designation. Recipient should exemplify the high caliber of professionals that comprise the CCIM Institute.

2 Scholarship Selection Criteria - Includes the following: Each applicant should be able to demonstrate a commitment to a commercial real estate career. Each applicant should be able to demonstrate active involvement as a dues paying member with the CENTRAL ARIZONA CCIM Chapter. Scholarship recipients should be prepared to volunteer on a CENTRAL ARIZONA CCIM Chapter committee for two years after receipt of the scholarship. Recipient of this scholarship should have a need for financial assistance. Scholarship Value Recipient will receive $1,000 to offset the cost of one of the following CCIM designation courses: CI 102, CI 103 or CI 104. Procedure - Applications are submitted directly to the CENTRAL ARIZONA CCIM Chapter. Deadline for Submission to CENTRAL ARIZONA CCIM Chapter: November 5, 2018 Recipient Notification - notification will be sent on or before December 1, 2018, notifying those selected. 2

I also agree that I will not be reimbursed for course(s) already taken and that this scholarship cannot be applied retroactively. I understand that if I fail this course, I will be responsible for reimbursing the Foundation for the full cost of a failed course; failing the course renders the scholarship null and void.
