1 Florida Local Government Information Systems Association (FLGISA) FLGISA Scholarship Application Packet 2016 The Scholarships of the Florida Local Government Information Systems Association (FLGISA) The FLGISA is proud to announce the following College Scholarships. A total of $8,000 is available for the children of FLGISA member employees who are high school seniors and entering college in the fall, as well as currently enrolled undergraduate students. There will be a total of eight (8) scholarships offered; one in each region and two others at large. FLGISA Northwest District Scholarship: Awarded to a child of an employee in a member organization in the Northwest Region. The child must be a High School senior or currently enrolled undergraduate student at an FLGISA Northeast District Scholarship: Awarded to a child of an employee in a member organization in the Northeast Region. The child must be a High School senior or currently enrolled undergraduate student at an FLGISA Central East District Scholarship: Awarded to a child of an employee in a member organization in the Central East Region. The child must be a High School senior or currently enrolled undergraduate student at an FLGISA Central West District Scholarship: Awarded to a child of an employee in a member organization in the Central West Region. The child must be a High School senior or currently enrolled undergraduate student at an FLGISA Southeast District Scholarship: Awarded to a child of an employee in a member organization in the Southeast Region. The child must be a High School senior or currently enrolled undergraduate student at an FLGISA Southwest District Scholarship: Awarded to a child of an employee in a member organization in the Southwest Region. The child must be a High School senior or currently enrolled undergraduate student at an NOTE: Two people in each district will be on the selection committee. If there is a conflict of interest, the committee member must excuse themselves if the scholarship application is for a family member (child, stepchild, niece, nephew, and grandchild). Page 1

2 FLGISA Scholarship Application Packet 2016 FLGISA Executive Board Scholarship Award: Awarded by the FLGISA Executive Board to a child of an employee in a member organization in the Any of the 6 regions. The child must be a High School senior or currently enrolled undergraduate student at an accredited college or university who has exhibited academic excellence and community spirit. This student will be awarded a scholarship toward college expenses to assist in their educational experience and growth. FLGISA President s Scholarship Award: Awarded by the current FLGISA President to a child of an employee in a member organization in Any of the 6 Regions. The child must be a High School senior or currently enrolled undergraduate student at an accredited college or university who has exhibited academic excellence and community spirit. This student will be awarded a scholarship toward college expenses to assist in their educational experience and growth. Eligibility: Scholarships will be granted to high school seniors or currently enrolled undergraduate students who are children of employees which work for FLGISA member organizations who will be attending an accredited college or university. The following minimum criteria must be met: U.S. citizen or permanent resident graduating from high school or currently enrolled at an accredited college or university. If awarded a scholarship, student must be enrolled full time. Proof of enrollment at post secondary institution required to release award funds cumulative grade point average, ability and motivation to compete on a collegiate level. Preference will be given to candidates who demonstrate strong educational motivation and group leadership skills through their activities in school, church, youth groups, community volunteer effort and/or work experience. Financial need and letters of recommendation are additional criteria in the selection process. Previous scholarship recipients may reapply, but new applicants will be given a greater amount of consideration. Scholarship Rules: Scholarship will be given for the fall of The awards will be given to the institution in all cases, and will be sent directly to the institution financial aid offices within 30 days of receipt of proof of student s full or part time enrollment at the school. When the institution requires the student to pay fees first before they certify registration, the school will be authorized to reimburse the student. If a recipient is unable to begin school in the fall and plans to enter school in a future semester, the recipient must notify the FLGISA in writing by June 30, 2016 for an extension. An extension is provided for up to one calendar year. If the scholarship is unused by the end of that year, the scholarship is forfeited and will be used to fund another scholarship in subsequent years. It is also forfeited if no extension is requested by June 30, Recipients will be recognized at a time and location to be determined. Page 2

Please mail or drop off in person your completed application form, high school or college transcript, student personal statement, and letters of recommendation to: FLGISA Scholarship Committee Attn: Rebecca Brennan Applications must be received by Sunday May 15, Final selections will be made by June 15, Letters of Recommendation Choose two (2) people to write letters of recommendation for you. It is strongly recommended that one letter come from a teacher or other school employee and one letter come from an employer or friend (non family related) who knows you through work or volunteer activities. Please enclose the letters with this application. Transcripts of Grades Your application must include high school or transcripts and SAT scores. If you have taken any college courses, include a transcript of your work thus far. Student Personal Statement Write a statement (no longer than 500 words) explaining the reasons you are applying for a scholarship, your plans for the future, and your short and long term goals. You should also include any other information about yourself, your family and background, your educational achievement, etc. which will help the Scholarship Committee to evaluate your potential. What is FLGISA? The Florida Local Government Information Systems Association is an organization for CIO's, IT Managers, and technology decision makers from the state's local government agencies. We welcome all jurisdictions to become a member. The FLGISA can benefit your organization no matter how big or small. Some Specifics FLGISA Vision: FLGISA is the Florida Local Government source for leadership, innovation and collaboration in technology. FLGISA Mission: To assist and educate local government technology leaders to collaborate, innovate and communicate, for the benefit of the communities we serve. Collaboration: To promote opportunities for our membership to work jointly with Federal, State, Educational, Non profit, or For profit organizations on initiatives or projects that benefit the citizens of Florida by improving service delivery and lowering costs. Education: To advance our profession by providing opportunities for continuous skills improvement in the areas of leadership and technologies through conferences, district meetings, and the Certified Public Technology Leadership (CPTL) program. Innovation: To introduce opportunities to transform how local government delivers services via technology by providing a forum for creative thinking, vision, and a culture of productive imagination. Communication: To engage the exchange of ideas and information for the mutual benefit of the members by providing forums to share experiences and expertise and to promote opportunities for mentoring by and between members.
