1 Collin County Economic Development Corporation Joycie Turner 2016 Scholarship Award Application Forms WHAT IS THE JOYCIE TURNER SCHOLARSHIP? The CCEDC awards non-renewable scholarships to graduating high school seniors living in Collin County based on several criteria listed below. WHO IS ELIGIBLE FOR THE SCHOLARSHIP? Any current high school senior that plans to attend an accredited institution of higher learning, or a two-year technical training school. WHAT ARE APPLICATION REQUIREMENTS? The following criteria will be considered by the Joycie Turner Scholarship Committee for the purpose of awarding scholarships: 1. GPA /Academic Awards/Honors/Special Recognitions 2. Community Service/Volunteerism 3. School Activities 4. Two Letters of Recommendations: a. Community leader, Church Leader, or Employer. b. Teacher, Guidance Counselor, or Principal. 5. A Typed Essay (minimum of 300 words in length no more than 500) 6. Return of Completed Application Forms NO LATER THAN Tuesday May 3, LATE APPLICATIONS WILL NOT BE ACCEPTED. WHEN AND WHERE WILL RECIPIENT NAMES BE ANNOUNCED? Winners will be notified by phone call and followed by an official letter. HOW WILL SCHOLARSHIP AWARDS BE DISTRIBUTED? All recipients must submit a letter of acceptance from their institution of choice and a copy of their fall class schedule to Frank Gray, Jr. or Sonya Lamey as proof of acceptance in order to receive their scholarship checks. 1

2 APPLICATION INSTRUCTIONS 1. Reference Joycie Turner Scholarship Checklist (Page 3) to ensure that all of the required forms are included in your packet. Each section must be COMPLETE for award consideration. You must follow the instructions. Incomplete packets will not be accepted. 2. Type or print all information on Joycie Turner Scholarship Application (Pages 4, 5 and 6). Please include a head-shot only photo with your application. 3. Applicant should use the enclosed Letter of Recommendation Forms to obtain recommendations in the following categories: Community Leader, Church Leader, or Employer (page 7) Teacher, Guidance Counselor, or Principal (Page 8) 4. The GPA Request/Confirmation Form (Page 9) must be signed by the applicant where indicated and then given to school Guidance Counselor for completion. Envelope must be sealed. 5. ALL 2016 Joycie Turner Scholarship applicants are required to submit a 300 word (minimum) essay (typed/printed and double spaced). If essays are less than 300 words, they will not be considered for award. 6.
