2 SAMPLE COURSE CROSSWALK TO CONNECTICUT HOSPICE AGENCIES Updated July 207 Relias Learning offers online learning, staff compliance training and continuing education for post-acute care facilities (Assisted Living, Long-term Care, Skilled Nursing, Home Health, Hospice, and Rehabilitation organizations). Relias Learning Training Crosswalks are based on published accreditation standards. They are designed to assist organizations in the selection of courses from the Relias Learning libraries to ensure and demonstrate staff competence according to the training standards. General staff compliance trainings should be selected to ensure the organization meets OSHA and other regulatory requirements. Staff training is only one element of a successful survey and the crosswalks are not meant to take the place of a careful review and evaluation of your program to the accreditation standards. The courses listed in the crosswalk are a representative sample of courses available in Relias Learning libraries. It may not be necessary to choose all courses listed for a specific training standard; it is recommended that courses are chosen that closely align with the mission and services offered and the specific needs of the persons served. The following constitutes Relias Learning, LLC s recommendation of course work for each listed Crosswalk Measure. Relias Learning does not make any guarantee that such course(s) will be accepted by the accrediting body(ies). 2

3 Connecticut Hospice Agency Training Criteria These categories are based on 9.33: Licensure of Home Health Care Agencies The regulations below were originally published by the State of Connecticut, and can be found at: r_iv/9-33._home_health.pdf Orientation All staff Pursuant to 9-3- D7(a)(), an agency orientation policy for all employees must include but not be limited to review of the following: Note: The orientation dates, content, and name and title of the person providing the orientation must be documented in the employee's personnel folder. Organizational structure of the agency Agency patient care policies and procedures Philosophy of patient care Description of client population and geographic area served Agency personnel policies and job description Applicable state and federal regulations governing the delivery of home health care services 3

4 Orientation All providers of care/all service providers Pursuant to 9-3- D72(b)(2)(B)(vi), the agency must provide orientation and training for all providers of care and services to the hospice philosophy of patient care. The hospice program is responsible for educating all unlicensed personnel assigned to provide services to hospice patient families regarding hospice goals, philosophy and approaches to care. Additional options, as applicable: Hospice 0: Orienting to Hospice Care Bloodborne Pathogens Essentials HIPAA: The Basics Infection Control Patient's Rights 0.6 Preventing, Recognizing, and Reporting Abuse 4

5 Orientation Volunteers Pursuant to 9-3- D72(b)(2)(P)(ii), the hospice program must provide orientation, to its volunteers consistent with the duties and functions to be performed Suggested options, in addition to agency orientation: to the Hospice Volunteers library Hospice 0: Orienting to Hospice Care HIPAA: The Basics Infection Control Patient's Rights 0.6 Preventing, Recognizing, and Reporting Abuse Annual In-Service All/applicable staff Pursuant to 9-3- D7(a)(2), the agency s in-service education policy must provide an annual average of at least one () hour per month for each employee serving patients. Pursuant to 9-3- D72(b)(2)(B)(vii), for hospice employees, six (6) hours of the annual in-service education requirements in accordance with Section 9-3-D7(a)(2) must address topics related to hospice care. Suggested options (6 hours): Cultural Competence and Humility in Palliative Care Everyday Ethics for Hospice and Palliative Professionals and Volunteers Giving a Complete Bed Bath to the module library Infection Control and Prevention 5

6 Annual In-Service All/applicable staff (cont.) Pursuant to 9-3- D7(a)(2), the agency s in-service education policy must provide an annual average of at least one () hour per month for each employee serving patients. Pursuant to 9-3- D72(b)(2)(B)(vii), for hospice employees, six (6) hours of the annual in-service education requirements in accordance with Section 9-3-D7(a)(2) must address topics related to hospice care. Nutrition and Hydration at the End of Life Oral Hygiene - Unconscious Individual Suggested options (6 hours) (cont.): Perineal Care for Home Health & Hospice to the module library Quality of Life Issues: Challenges and Opportunities The agency must ensure, as part of its coordination of inpatient care agreement with an inpatient setting, that all direct service staff receive in-service education including two (2) hours specific to hospice care. Hospice in Long Term Care: Care Coordination Hospice in Long Term Care: Comfort, Pain & Symptom Control in Hospice Suggested options (2 hours): to the module library Hospice in Long Term Care: Hospice Philosophy of Care Hospice in Long Term Care: Patient's Rights, Dignity and Model of Care 6

7 Annual In-Service All/applicable staff (cont.) Pursuant to 9-3- D7(a)(2), the agency s in-service education policy must provide an annual average of at least one () hour per month for each employee serving patients. The in-service education must include current information regarding drugs and treatments, specific service procedures and techniques, pain and symptom management, psychosocial and spiritual aspects of care, interdisciplinary team approach to care, bereavement care, acceptable professional standards, and criteria and classification of clients served.
