Well September seems to have crept up on us by surprise with a few welcome rain showers so far. It s that time of year again for extra layers, umbrellas and sensible shoes! We wish everyone a happy September and good luck with all the back to school chaos and the beginning of fall activities. Looking for Parksville Network Lead We are still looking for one Parksville MOA who would like to take on the role of Network Lead. This involves meeting every couple of months for about an hour to give feedback on the needs, wants and concerns of MOA s; provide assistance in planning upcoming events, offer creative input on newsletters and suggestions for educational development. As you are aware, Dr. Jayaraman has closed down his practice in Qualicum Beach. I m sure that you have all been receiving a large volume of queries from his former patients. To address the increased number of patients looking for physicians, the Division is pleased to announce the launch of a TEMPORARY Referral Mechanism. The initiative aims to create a referral process that is easier for physicians and patients by using a centralized process. Patients will be triaged by a team and referred to the most appropriate family physician. Note that we have a list of physicians who have indicated that they are able to add patients to their practice, the area of focus and the number of patients that they can accommodate.
2 While our initial focus is on complex, urgent, unattached patients, the long term aim is to help support attachment for all patients. This Temporary process will be utilized as a prototype for the development of the long term Centralized Referral process. We will start with Dr. Jayaraman s patients and expand as capacity allows. If you would like to refer Dr. Jayaraman s high needs patients, please provide the hotline information to them. We are basing this on the definition as supplied by the GPSC (General Practice Service Committee) that relates to the GP14074 fee code. Frail in Care (CSHA - Canadian Study for Health and Aging, Clinical Frailty Scale score of six or more in residential care new admissions only with exceptions for extenuating circumstances such as sudden departure from practice of existing MRP FP Significant Cancer Palliative Moderate to High Needs Complex Chronic Conditions Severe Disability in the community Mental Health and/or Substance use New Mother and Infant(s) (intake can occur at any time during pregnancy and up to 18 months of age.) The Hotline will launch on September 8, 2015, and we are expecting that it will continue until the end of October. We will keep you informed with updates. SmartSex Resource Walk-in service at the Youth Clinic (<25yr olds) for appointments with a nurse. Location: Parksville Family Place 494 Bay Avenue, Parksville, BC, V9P 2G
3 Hours: Tues & Thurs: 3:30 p.m. - 6:00 p.m. Services: STI testing and treatment, HIV testing, Birth control, Emergency contraception, Pregnancy counseling, Pregnancy test, Free condoms, Needle supply, Pap testing, Emergency contraception and harm reduction supplies available. Medical Equipment Loan Cupboard Recently a patient strained his ankle and asked where he could get crutches that he could use for a couple of weeks. Pharmasave, Life Support in addition to other local pharmacies offer medical equipment rental services; however did you know that Parksville and Qualicum Legions have a Medical Equipment Loan Cupboard where people can borrow items? You don t even have to be a member! Parksville Legion 146 Hirst Ave, Parksville (250) :30 a.m.-11:00 a.m. - Monday to Friday The equipment cupboard is located at the back of the legion building. There is a sign on the door which says medical equipment loan pick up/drop off. Apparently they have an extensive list of items available. Everything that your Doctor could possibly order was how it was described! Qualicum Legion 180 Veterans Way Qualicum Beach (250) :00 p.m.-2:00 p.m. - Monday to Friday There are lots of items available, crutches, wheelchairs, walkers etc. Items are signed out on a 3 month loan basis, with extensions available. Transportation Services A MOA was asked recently about the transportation available to seniors who need to get to Doctors appointments when they don t have any method of transportation themselves. Here are a few by donation options in the area:
4 Cancer Society Volunteer Drive Program Medical Appointment Transportation Program (SOS) (250) x 222 This program only covers trips between Qualicum Beach and Parksville to Comox, Courtenay, NOT Nanaimo OR Victoria. Wheels for Wellness (250) Web: donbuchner@wheelsforwellness.com All services usually require hours notice to arrange a pick up. Group Medical Visits Other medical communities on Vancouver Island have been taking a group approach to Doctors appointments. If there are patients in the practice who suffer from similar chronic conditions e.g. diabetes, the Doctor meets with the group of patients where they can discuss similar topics and set goals. This format makes patients feel supported and more in control of their health knowing that there are other people in the same situation. Group Medical Visits (GMVs) are a rewarding and time-efficient way of caring for patients. The Practice Support Program (PSP) team can work 1:1 or with small groups of physicians and MOAs to plan the GMVs. It is a new way of offering care hence a lot of thought needs to be given to identifying the optimal group visit model best suited to one s practice. Because PSP want the GMVs to be sustainable they review the fee structure and identify a break-even point to ensure GPs are not losing revenue. It is important to secure space and find a regular time that is convenient for both GPs and patients. A process to easily identify your target patients needs to be determined and then the patients would be asked if they would be willing to try this new format. Tools such as sample letters to patients, group expectations, a consent document and evaluation of the visit are provided. The PSP team wants the GMV to be a great experience for all involved! Practice Support Program The Practice Support Program (PSP) provides training and support for physicians to enhance the delivery of patient care. The Program s objectives are to enhance provider and patient experience and to promote sustainable practice change through the implementation of quality
5 improvement strategies in primary care. The program is delivered by Regional Support Teams (RSTs) across the province. In 2015/16 an integrated model of practice support will be executed to better meet the needs of Divisions and physicians. This model will build an infrastructure allowing for greater flexibility in the delivery of PSP quality improvement and practice support, and increase the involvement of Divisions in co-planning of PSP services. Local collaborative PSP-Division planning tables are being established to facilitate this change. One service offering under this model will include PSP supported small group learning sessions (SGLS) that will promote physician designed and led change in quality improvement. Grounded in best practice guidelines SGLS topics will be chosen by the Division to reflect their local quality improvement needs. Session topics may include clinical quality improvement, office efficiency, and EMR optimization. SGLS will be co-created with Divisions and RSTs and endorsed by the collaborative planning table in order to prioritize resources for quality improvement and practice supports. In recognizing accreditation of SGLS, when developing individual SGLS s, the collaborative planning should consider: 1) topic to be addressed based on an absence or deficit in knowledge or skills (a gap between current practice and best practice activities); 2) focus on key patient populations (eg. Priority populations and/or patient access through increased office efficiency, PSP content areas, EMR optimization support, and/or other emerging best practices and; 3) commit to evaluate, share and spread learnings with partners. This was offered previously in the Adult Mental Health module by the Practice Support Program (PSP). PSP ran this module in various locations around the Oceanside area. Your Doctor may have participated and the PSP team is trying to determine which MOA's attended too. Unfortunately there was not a lot of response. This is an invaluable course and we would love to offer it again to people who are eligible. Please contact oceansideadmin@divisionsbc.ca and let us know who has taken it and who hasn t from your offices. Emergency Pool of MOA s There is an experienced MOA who is ready to work. Contact us if you know any family doctors, specialists, long term care facilities or OHC admin looking for someone. Bonnie Just finished a temporary 6 months assignment. She has both the MOA & Medical Terminology certifications, in addition to years of experience in medical office administration both from the Mainland and Vancouver Island. MOA of the Month In every newsletter we highlight an Oceanside MOA to give you a better idea of who is part of the Oceanside Team. This month it s Ingrid Morton. Ingrid Morton is a married mother of two active boys. She worked in a busy accounting office for many years before children, and began working as a MOA in a very part time capacity when they boys were toddlers. Ingrid is now working 2 days a week at the Qualicum Associates Clinic with Dr. Clair Biglow and Dr. Ashraf El Karsh. She enjoys all aspects of her job, particularly patient interaction. Ingrid feels that being a good listener is the most important part of her job
7 and does her best to "keep all of the plates spinning" to ensure that all of the patients and Doctors needs are met. She is always on the lookout for ways to make work flow smoother and quicker and always asks the question, How can we make this process less complicated? Ingrid enjoys hiking, camping, beachcombing, gardening and all kinds of adventures with her family. They recently decided that they were ready for a "bigger" adventure, and planned a multi-day trip to beautiful Rugged Point Marine Park on BC's West Coast. After packing an astonishing amount of gear! They drove 4.5 hours north through Campbell River, then Zeballos, and finally arrived in Fair Harbour. There they were picked up by water taxi and made the 45 minute trip to their destination, where they set up camp and lived on the beach for 4 days. They were gifted with amazing weather, a spectacular wilderness destination, white sandy beaches, and a huge red snapper from a passing boat which made for a delicious supper! Ingrid is now busy planning more adventures, and looks forward to exploring more of BC's West Coast.
8 Oceanside MOA Recipe Book Our Watermelon Salad recipe was a great success from last month. 3 of our readers made it for recent pot lucks and it went down a treat. Send us your favourite recipes. If we can gather enough we will put together an Oceanside MOA Recipe Book. Olive Cheese Bread...mmmm! An old fav of mine A stick of softened butter 1/2 cup of mayonnaise 1 1/2 cups grated Monterey Jack cheese A bunch of chopped black olives A bunch of chopped green olives Sliced green onions Dash of pepper Mash into a spread, spread it onto 2 halves of French bread, bake at 325 for 20 minutes. Slice, serve, and swoon! Walking Groups in Oceanside MOAs in Qualicum Beach have started a weekly lunchtime walking group on Tuesdays. Oceansidemoa@hotmail.com and let us know if you are interested in joining us. We meet on Fern Rd W/Beach Rd at approx. 12 noon and walk for 30 minutes. Come and join us if you have time to spare.
