2 Report Information Virgin Australia Just Culture Policy Virgin Australia is dedicated to providing and maintaining a safe and healthy workplace for team members, contractors, visitors and guests as is practically achievable. Virgin Australia has a 'no blame' policy for the reporting of accidents and incidents. This means that individuals are not punished for actions, omissions or decisions taken by them based on their experience and training which results in a reportable event. The purpose of reporting is to learn from the experience and implement appropriate processes to ensure the safety of team members, contractors, visitors and guests and serviceable equipment. Whenever an accident, incident or personal injury occurs or whenever working conditions, which are potentially hazardous to persons, equipment or buildings are detected, they must be reported and investigated. Policy Upon being involved in incident that results in an injury or illness (no matter how insignificant), all Virgin Australia Group team members and contractors must immediately or as soon as is reasonably practicable, report the incident verbally, to their direct supervisor, leader or manager and complete and submit an Injury / Illness Report (ereport or paper-based report) within 4 hours but not later than the end of shift. For the complete Virgin Australia Group Incident Reporting and Investigation Standard refer Virgin Australia Volume SSM5: HSE Management System, Section 11 - Incident Reporting and Investigation. Report Submission The Injury / Illness Report is to be completed by all staff members including contractors in the event of an injury or illness. Any guest injury or illness must be reported using the Injury / Illness Report. Note: Virgin Australia Airlines only: selected Supervisors will be notified by when a team member has submitted an Injury / Illness Report. page 02

3 Methods of Reporting You are able to submit two different types of reports: 1. ereport completed online and the preferred type of reporting (further instructions related to using the ereporting portal are provided in the ereporting ). 2. Paper-based Report completed by those team members and contractors with limited access to computers. Figures 1 and 2 illustrate the two different methods of reporting. 4 Company ereports It is important to select the correct company when completing an ereport. VAA Reports VAI Reports VANZ Reports Operational Personnel Reports submitted by all operational team members and contractors relating to: Australian domestic operations International operations including sectors to/from: Christmas Island Cocos Keeling Islands Honiara Phuket Non-Operational Personnel Reports submitted by all team members and contractors based in Australia. Operational Personnel Reports submitted by all operational team members and contractors relating to: International operations including sectors to/from: Abu Dhabi Kuala Lumpur Los Angeles Non-Operational Personnel Reports submitted by all team members and contractors based in: United Arab Emirates United States Operational Personnel Reports submitted by team members and contractors relating to: Pacific and Trans-Tasman operations Non-Operational Personnel Reports submitted by all team members and contractors based in New Zealand and Pacific Islands. There are a number of different types of reports available in the Safety and Security ereport system. Not all reports are available in all companies. Please contact your Supervisor of company liaison should you require assistance in report selection. page 04

5 Injury / Illness Report To complete a new report: 1. Login to Safety Reporting: VINE > Safety Reporting > Click the company logo. 2. Click Injury / Illness Report from the list of available reports. Note: the active report is indicated by an orange highlighted area. 3. Complete the following fields: a. General Section: Date and Time of Incident Airline Flight Number Location (Port) Flight phase Aircraft Registration Specific Location Enter the date and time of the injury or illness in the format: dd mm yy hh:mm. Select from the drop down list the airline: Virgin Australia Virgin Australia International Virgin Australia New Zealand Virgin Australia Regional Airlines Alliance. *Specifying the airline will ensure your report is submitted correctly. If the injury or illness occurred while attending to an aircraft, enter the Flight Number. Select from the drop down list or enter the location where the injury / illness occurred, for example, BNE, SYD, MEL, enroute. If the injury or illness occurred while attending to an aircraft, select from the drop down list the flight phase, for example, take-off or landing. If the injury or illness occurred while attending to an aircraft, select from the drop down list the Aircraft Registration. Specify the exact location where the injury or illness took place for example, Bag Room, Tarmac, Cabin, Galley, Office. page 05

6 b. Injured/Ill Person s Section Name Contact number / Address Gender PNR (Guest only) Enter the first and last name of the injured or ill person. Enter the contact phone number and / or address of the injured or ill person. Enter the address of the injured or ill person. Provide your work address for personal injury or illness reporting. Select from the drop down list the gender of the injured or ill person. Enter the Passenger Number Record if the injured or ill person is a guest. page 06

Injured / Ill Staff Section: Home Port Reported to Supervisor Supervisor Name Hours of Shift Worked Person Type Supervisor Staff Number Years Worked in Current Role Division No. Days since last full day off Employment Type Select from the drop down list your home port or office location. Note: for guests or visitors select N/A Guest. Select yes or no from the drop down list. All injuries or illnesses must be immediately reported to your manager or supervisor. Enter the first and last name of your Manager or Supervisor. Enter the number of hours you have completed of your current shift or work day. Select from the drop down list the injured or ill person s primary role at Virgin Australia, ensure you correctly identify guests, visitors, staff members and contractors. Note: Administration covers ALL office workers in Virgin Village, Airport Terminals, Training Facilities and other office locations. Enter your Manager or Supervisor s work address. Entered the injured person s staff number. Note: for guests or visitors enter N/A. Select from the drop down list the injured or ill person s number of years in the current role. Select from the drop down list the Division in which the injured or ill person works. Note: for guests or visitors select N/A Guest. Select from the drop down list the injured or ill person s number of days since the last full day off. Select from the drop down list the injured or ill person s type of employment. Note: for guests or visitors select Guest. page 07

8 d. Injury / Illness Section: Occurrence title Occurrence description Bodily Location Equipment Involved Injury Cause Nature of injury Describe any Contributing Factors? Witness (if any) Enter a brief description of the injury or illness (up to 80 characters). Provide a clear, concise and factual account of the occurrence. Select from the drop down list the part of the body affected by the injury or illness. Examples may include hands and fingers, lower back, knees and upper leg, respiratory system, digestive system and more. If the injury or illness occurred due to any form of contact with equipment, select from the drop down list or enter the equipment involved. Examples include Aircraft Stairs, Baggage, Engineering Tools & Equipment, Overhead Locker, Escalator/Travelator and more. Select from the drop down list the cause of the injury or illness. Examples may include: hitting an object with a part of the body, being hit by a moving object, manual handling, contact with a substance, contact with electricity, falls from height, falls from the same level, noise and more. Select from the drop down list the nature of the injury or illness. Examples may include: bruising, cuts, electric shock, food poisoning (suspected), hearing loss, allergic reaction, dizziness and more. Enter any factors that may have contributed to the injury or illness. Examples may include turbulence, damage to equipment and more. Enter the first and last name and phone number or of any witnesses. page 08

9 e. Treatment Section: Treatment provided What preventative action has been taken to prevent recurrence? Medical Volunteer s Medical Outcome Medical Assistance Did Virgin Australia equipment, facilities, people or process contribute to the injury/illness? Did the incident have the potential to cause serious harm as a result of any of the following? Enter the details of any treatment provided. Examples may include first aid, treatment with ice, physio and more. Enter the details of preventative action that has occurred immediately after the incident. Examples may include isolation of an area, signage or cleaning of a spill, applying an unserviceable tag to equipment. If an external Medical Attendant was engaged, provide the details (first and last name and place of work) of the Medical Attendant. Examples may include a Doctor, Nurse, Physio, Doctor(s) on Board and more. Select from the drop down list the medical outcome of the injury or illness. Select from the drop down list the medical assistance provided. Select from the drop down list Yes or No. Select from the drop down list one of the following if it applies: Escape of a pressurised substance Escape of gas or steam Collapse of a structure Items falling from heights Electric shock Implosion, explosion or fire. page 09

10 f. Reporter s Section: Reporter s Name Reporter s Workplace Reporter s Entered Entered by Enter your first and last name. Enter your home port or workplace location. Enter your work address. Defaults to the date of submission. Defaults to the name of the person submitting the report. 4. Click Submit, located at the bottom of the form. 5. Click OK on the confirmation window. 6. Close the Internet Explorer window to exit the ereporting Portal. page 10
