1 Incident, Injury, Trauma and Illness Policy Policy Number: 08 Approval Date: 1 October 2015 Review Date: 30 September 2018 Aim The Service and all educators can effectively respond to and manage accidents, illness and emergencies that occur at the service to ensure the safety and wellbeing of children, educators and visitors. Implementation All staff and visitors to the Service are required to follow this policy in the event that a child is: Injured; or Becomes ill; or Suffers a trauma Parents will be notified of an incident/injury/trauma/illness as soon as possible but usually immediately, once First Aid has been provided. For issues not requiring notification to the Regulators, a standard Service template is used to record the issue. For issues that require the Regulators be informed an Incident, Injury, Trauma and Illness Record should be completed. This record may be found here: Incident,%20injury,%20trauma%20and%20illness%20record%20word%20version.p df If unsure of whether the issue will require notification, err on the side of caution and complete ACECQA s Incident, Injury, Trauma and Illness Record as linked above. These forms must be completed as soon as possible and always on the day of the issue. These forms must be shown to the parent/caregiver collecting the child and signed by this person. Only team members with a current, approved First Aid qualification (including a current, approved CPR qualification) may administer First Aid. There will always be at least one person with current First Aid qualifications onsite and available to the children whenever there is at least one child in attendance. This includes current, approved courses in First Aid, CPR, Anaphylaxis and Asthma. All incidents, injuries, illnesses and trauma must be reported to the Director (Approved Provider/Nominated Supervisor) as soon as possible. First Aid kits
2 First Aid kits are provided throughout the service and their location identifiable by appropriate signage. The locations of kits are: i. Back verandah, by the shower (accessible to team members for Giraffes and Lions children as well as themselves). ii. In the kitchen, on top of the freezer (accessible to the cook). iii. In the staffroom, on top of the fridge/freezer (accessible to team members and visitors). iv. In the Elephants bathroom cupboard (accessible to team members for Nursery and Elephants children as well as themselves). v. In the hall cupboard, Nursery building (accessible to team members for Nursery children as well as themselves). Administration of First Aid If there is an accident, illness or injury requiring first aid, the following response procedure will be implemented: Team member notifies the Responsible Person and refers person to someone with current qualifications (as noted above), ensuring any safety issue that could have caused the issue is resolved First aid qualified educator reviews child s medical information including any medical information disclosed on the child s review form, medical management plan or medical risk minimisation plan before attending to the injured or ill person. If the illness or incident involves Asthma, an allergy or Anaphylaxis, a second team member or visitor will be present to witness any administration of medication and remain present for the duration of its administration. A Medication Record will be completed: Team members not required to administer First Aid will continue to care for children in the vicinity of the incident, illness or injury If an ambulance is required, a team member will wait on the footpath on Cressy Road, ensuring access to the property is clear and ambulance officers are shown the quickest way inside to the person in need For any issue requiring Regulator notification, needing medical assessment, affecting the head (no matter how minor) or serious enough to cause surprise to the parent upon arrival, the child s parent will be notified as soon as possible If child presents as ill (temperature of 38 or more, has had two cases of vomiting/diarrhoea or generally not well enough to participate in the program) parents/approved caregivers will be called and requested to collect the child within sixty minutes. 3 Notification of serious incidents and complaints The Approved Provider will notify the Regulators within 24 hours of any serious incident at our service (s. 174). This includes an injury or trauma to, or illness of a child for which the attention of a medical practitioner was sought or ought reasonably to have been sought or the child attended, or ought reasonably to have attended a hospital. If the attention of a medical practitioner was sought or the child attended hospital in connection with the injury, trauma or illness the incident is a serious one and must be notified. To decide if an injury, trauma or illness is a serious incident when the child did not attend a medical practitioner or hospital, we will consider the following issues: Was more than basic first aid needed to manage the injury, trauma or illness? Should medical attention have been sought for the child? Should the child have attended a hospital or an equivalent facility? If our service only becomes aware that the incident was serious afterwards, we will notify the regulatory authority within 24 hours of becoming aware that the incident was serious. We will notify the Regulator via the NQA ITS portal, The Approved Provider will also notify the regulatory authority in writing within 24 hours of any complaints alleging that the safety, health or wellbeing of a child is being compromised at the service. First Aid Kit Guidelines Any First Aid kit at the service must: Not be locked. Not contain paracetamol. Be appropriate for the number of employees and children and adequate for the immediate treatment of injuries at the service. Be in a place that takes an employee no longer than two minutes to reach, including time required to access secure areas. Be constructed of resistant material, be dustproof and of sufficient size to adequately store the required contents Be capable of being sealed and preferably be fitted with a carrying handle as well as have internal compartments.
4 Contain a list of the contents of the kit. Be checked quarterly using the contents list to ensure the contents are as listed and have not deteriorated or expired. Have a white cross on a green background with the words 'First Aid' prominently displayed on the outside. Be easy to access and if applicable, located where there is a risk of injury occurring. Be provided on each floor of the Service. First Aid kits must be taken on excursions/lockdowns/evacuations and be attended by First Aid qualified educators. A well recognised, standardised first aid sign to assist in easily locating first aid kits. Signage will comply with AS 1319:1994 Safety Signs for the Occupational Environment. All team members accessing a First Aid Kit share in the responsibility to report stock usage and re-order supplies when getting low, this is done by noting what is needed on the Cressy Road Team Facebook page and including #CressyRoadS in the original post. Work Health and Safety (OHS) requirements Serious injury or illness is a notifiable incident under the Work, Health and Safety legislation. Serious injury or illness means a person requires: immediate treatment as an in-patient in a hospital, or immediate treatment for: o the amputation of any part of the body o a serious head injury o a serious eye injury o a serious burn o the separation of skin from an underlying tissue (such as degloving or scalping) o a spinal injury o the loss of a bodily function o serious lacerations or A serious illness includes any infection to which the carrying out of work is a significant contributing factor, for example an infection that can be linked to providing treatment to a person or coming into contact with human blood or body substances. A dangerous incident is also notifiable under the legislation. Dangerous incidents include: an uncontrolled escape, spillage or leakage of a substance an uncontrolled implosion, explosion or fire Incident, Injury, Trauma & Illness Policy Cressy Road Cottages Page 2 of 3
5 an uncontrolled escape of gas or steam an uncontrolled escape of a pressurised substance electric shock the fall or release from a height of any plant, substance or thing the collapse, overturning, failure or malfunction of, or damage to, any plant that is required to be authorised for use in accordance with the regulations the collapse or partial collapse of a structure the collapse or failure of an excavation or of any shoring supporting an excavation the inrush of water, mud or gas in workings, in an underground excavation or tunnel The Approved Provider/Nominated Supervisor must notify WorkCover by telephone or in writing (including by facsimile or ) as soon as possible after the injury, illness or incident. Records of the incident must be kept for at least 5 years from the date that the incident is notified. The Approved Provider/Nominated Supervisor must ensure the site where the incident occurred is left undisturbed as much as possible until an inspector arrives or as directed by WorkCover.
