2 What is a Promotora? A Bilingual, bi-cultural community outreach worker Deployed in low-income, hot spot areas throughout Utah Who assist patients families in finding necessary resources; connect to health care providers; gain insurance coverage; help with health literacy through education programs

3 Why did Holy Cross(HCM) create this Objective: Role? To ensure that families are healthy, safe, and can navigate complex health & social systems by providing a variety of educational programs and increasing individuals abilities to access community resources and services.

4 Why Did Intermountain Choose to Partner with HCM Intermountain Hot-spotting data in Park City identified a need HCM has known track record of success with this role in other areas The community was familiar with this roll via a Promotora at the People s Health Clinic We believe this role will help connect patients to the appropriate level of care at the right time

5 The Promotoras have been characterized as bridge builders who play a central role in community integration and are key to every program at Holy Cross Ministries. As community outreach workers, the Promotoras have assisted over 1,844 to access basic resources this year. The Intermountain-HCM Promotora located at the Bonanza clinic began mid-august and is taking referrals from Round Valley & Heber Valley clinics, the hospitals, and the OB/GYN practices

6 Success Story It began when a newly diagnosed ten year old diabetic was brought into the emergency department in Heber because of low blood sugar. She was admitted to the hospital and treated. There was teaching done and she was to follow up with Primary Children s. The next day, the interpreter who has translated for the family and knows them well, came to CM because she wanted to make sure the patient did not fall through the cracks. Interpreter felt that we should somehow come together as a group to help this family as it has been extremely overwhelming, especially for the patient s mother. Interpreter arranged a meeting which included herself (as translator), the Promotora, care manager Heber Valley Clinic, Certified Diabetic Educator and the nurse at Wasatch County Health Department. The patient s mother was willing to meet with everyone & miraculously, we were all able to meet with Mom. Each person was able to explain their role and how we all wanted make sure that the family s needs were being met. There were issues with mom understanding medications and diet so it was arranged for them to meet with diabetic educator. It was also felt that in order to help mom navigate through all of this that perhaps some counselling support would be helpful. The Promotora made arrangements with someone who speaks Spanish for this. The RN at the Health Dept. took on the task of making sure the meter she was using to test her daughter s BG was covered by Medicaid. This RN had also been in contact with the social worker from PCMC to let them know that mom has felt her needs aren t met when she goes down with her daughter to see the endocrinologist. PCMC has acknowledged this and will pay more attention to the needs of the family. The CM from Heber Valley also contacted the social worker at PCMC just to introduce herself as the liaison for the clinic and also let the patient s PCP know what the team has been doing. He suggested that she should be seen for a WCC(well child check.) She saw him on 10/13. Full Circle!
