1 REFERRAL PROCEDURES FOR NHS ENGLAND ARDEN, HERFORDSHIRE AND WORCESTERSHIRE AREA TEAM As per your GOS Contract it is your obligation to be aware of correct referral procedures. At the end of a sight test if you feel a referral is necessary you need to refer in line with this guidance, as outlined in clauses 31 and 100 of the GOS Contract. GENERAL NOTES All referrals need to be legible and should provide all the necessary information about why you are referring. All referral routes are decided by the patient s GP practice and by the Clinical Commissioning Group (CCG) the GP Practice belongs to, not the patient s residence address or the practice that the sight test is undertaken at. Referrals are based on 5 categories: Routine/Urgent/Emergency/2 Week Waits (2WW) and that have been commissioned by the CCG. 1. Routine needing an appointment for a condition that is chronic and not immediately sight threatening. 2. Urgent needing an appointment for a condition that is not immediately sight threatening. 3. Emergency needing an appointment on the same day. 4. Suspected cancers and Wet ARMD. 5. Unlike your GOS contract which is commissioned by the Area Team, are commissioned by CCGs outside of the GOS contract. These include schemes such as Intra Ocular Pressure Referral Refinement Scheme (IOPRRS), Post-Operative Cataract, Pre-Operative Cataract and Primary Eyecare Acute Referral Service (PEARS). Referral processes are different for each geographical area and are detailed on the following pages:

All referrals are triaged by Ophthalmologists within 2 working days. If the referral is not appropriate thy will onward refer to the Hospital Eye Service within 48 hours or same day if urgent. The patient's GP will be advised if they have referred the patient on. All patients are telephoned to book a convenient appointment or they can call on An appointment will be allocated within a maximum of 4 weeks for all referrals. Once the patient has been seen they will write to the patient's GP with the outcome. Wet ARMD please send as a fax referral to or as an (if you have nhs.net ) to amd.fasttrack@nhs.net Direct Referral to Hospital Service Cataract Operation GP informed 4-6 week Post-operative Assessment by Optometrist refer back to hospital if necessary and/or for a the second eye operation Glaucoma Suspect Referral Refinement involving second reassessment of IOPs, visual fields and optic discs refer directly to hospital if glaucoma confirmed or now highly suspected GP Eye Referral Scheme where the GP refers non-urgent cases to an optometrist that otherwise would have been referred to the hospital. Optometrist refers on to hospital those deemed to be urgent (this scheme soon to be changed to a PEARS). Children s scheme where orthoptists screen out at year 1 children and refer to optometrists. Optometrists then examine them and refer to the hospital the few deemed requiring further examination. HOLOMS: Optometry Ocular Hypertension Monitoring the patient is seen by the optometrist every 6 months instead of the hospital. The hospital will see the patient after 3 years unless the optometrist finds significant changes before.
