1 North York Family Health Team Inside this issue: Welcome November 2014 Welcome 1 Coding of Diabetes Associated with higher Quality of Care NYFHT: Highlights & Accomplishments 1 2 Learning Sessions 2 Upcoming Events 3 Reminders 5 The NYFHT is very pleased to announce several new staff members who have recently joined our team. Caroline Pritchard and Patricia Antonini join us as new Registered Nurses, both coming with a wealth of experience in chronic disease management. Nicole Jean-Pierre is a new Registered Dietician, and will be providing support to the Diabetes Education Program. As well, we have two new Nurse Practitioners: Christina Mueller and Amy Allen. In addition to their clinical roles within physician offices, both have been oriented to the Cancer Survivorship Program, and will be providing assistance to this continually growing partnership with the NYGH. Welcome to our team! Coding of Diabetes Associated with Higher Quality of Care A study was published in the CMAJ Open entitled: Identification of undiagnosed diabetes and quality of diabetes care in the United States: cross-sectional study of 11.5 million primary care electronic records According to the study results: "We were able to identify a substantial number of patients with uncoded diabetes and probable undiagnosed diabetes based on existing biochemical evidence using simple algorithms applied to primary care electronic records. Patients with a coded diagnosis of diabetes had a higher quality of care than those with uncoded diabetes." The NYFHT Summer Student program has resulted in significant increases in coding across multiple health conditions and cancers. Analysis of changes in coding compared to other groups almost completed and so far shows major improvements at NYFHT compared to elsewhere in Canada.
2 Page 2 NYFHT: Highlights/Accomplishments Bright Lights Award Congratulations to Dr Michelle Griever, Marjan Moeinedin, Karen Rothschild and all of the North York Family Health team in winning this much deserved Bright Lights Award at the AFHTO conference. Using data to improve transitions of care and care coordination North York Family Health Team Making it easier for teams to access data needed to improve care This award was especially popular and meaningful in that Dr. Josh Tepper, CEO of Health Quality Ontario, spoke immediately after our award presentation which allowed North York Family Health Team to shine as we clearly set a very high standard in accessible and meaningful data to improve quality of care. On another note, we also invited our Ministry consultant to sit at our table for dinner and she made a point to mention how impressive this award was and the good work that the North York Family Health Team accomplishes on an ongoing basis. Learning Sessions with Dr. Margittai or Dr. Becker As you know, Dr. s Margittai (our psychiatrist) and Becker (our endocrinologist) are available to give small group learning sessions at any of our office locations. 3 Page 3 Date: December 11, 2014 Time: 12:30 to 4:30 Location: Ontario Science Centre UPCOMING EVENTS All FHT Meeting Special Message: At this year s All FHT meeting, we will be collecting donations for Covenant House, a charity which serves street youth. According to Covenant House, they anticipate requiring 225 gift bags for their youth. Sixty-eight percent (68%) of their youth are male and 32% are female between the ages of 10 and 24. Given the higher ratio of male to female, the are asking for larger sizes in clothing and footwear (i.e. XXL, XL) and greater volume of male-specific items. With List: The following is a wish list of the items they would like for their youth: Backpacks Gift Cards Movie passes Fast food vouchers McDonalds, Tim Hortons, etc. TTC Tokens Wallets (male & female) House slippers House coat/robe Chocolate bars/christmas Candy (nut free/no gum for stockings) Chapstick/Lip Balm Other gifts that promote Christmas cheer are also appreciated All donations are to be unwrapped. Please do not pre-package your donations into individual gifts. They will divide the donations they receive evenly among the youth. Thank you for your support! 5 Page 5 Reminders Initial Appointments with Registered Dieticians: In order to improve access for all patients, as well as more efficiently utilize the valuable time of our Registered Dieticians, patients referred with dyslipidemia, hypertension, or a positive family history of coronary disease will be advised to attend a group Heart Health Class prior to being offered individual counselling. Patients who decline attendance of the Heart Health class and prefer to be immediately seen for individual counselling are encouraged to contact a private Registered Dietician. Contact information for such services will be provided to patients. Collaborations: CCAC Please continue to engage your office s dedicated CCAC Coordinator. If you have not already had them at an office meeting, this is a great way to learn more about how to leverage this relationship. You can ask your CCAC Coordinator to bring a list of all your patients who are currently receiving their CCAC services, or you can ask for information on certain programs that CCAC offers. Please notice when this initiative has been helpful (you could even send us an of gratitude to the Clinical Manager!). We will reflect on this effort with CCAC in future.
