3 Meaningful Use Stage 2 For eligible professionals: Provide patients the ability to view online, download, and transmit their health information within 4 business days of the information being available to the eligible professional. Provide clinical summaries for patients for each office visit. Use secure electronic messaging to communicate with patients on relevant health information. For hospitals: Provide patients the ability to view online, download, and transmit information about a hospital admission. For both: Use certified electronic health record (EHR) technology to identify patient-specific education resources and provide those resources to the patient
4 Definition under Accountable Care Coordinated Care: Once an ACO begins to benefit from the connected care measures brought forth through Meaningful Use, they can mature toward coordinated care. In coordinated care enough patient data is connected among the multiple organizations in an ACO to allow care coordination through case tracking and identification of health risks. Integrated Care: Further ACO maturity moves us to the stage of integrated care. With integrated care, multiple organizations share data and resources with a fluidly that allows population health management and advanced digital biology to pick up steam. As a result, the patient can begin to exert control over their health with an integrated team of healthcare providers by their side to not only address existing health concerns but to also help prevent future problems through personalized prevention and behavior modification fueled by collaboration and connected data. Accountable Care: In the most mature of organizations we finally see true accountable care. With accountable care healthcare organizations become accountable for outcomes and costs as a team fueled by a high level of patient collaboration. ACOs take full responsibility for a patient s, and a population s, care over time. This creates an environment where full information therapy and customized behavior prescriptions can be added to the more standard clinical treatments to make real shifts in healthcare outcomes.

6 Survey Said: According to a survey from the National ehealth Collaborative (NeHC): 53% of respondents ranked consumer engagement as high or very high in their organization s priorities 33% say their strategies for consumer engagement with health IT are not clearly defined at all. 59% say their strategies for consumer engagement are evolving toward clarity. 8% say their patient engagement strategies are clearly defined 68% of the respondents wanted to use patient engagement to improve health outcomes 66% want to use patient engagement to deliver information to patients 59% want to use patient engagement to enable consumers to take more responsibility for their health and reduce healthcare costs 57% want to use patient engagement to improve consumers experience in interacting with their organization
7 Healthcare progress so far: 53% say they are delivering information electronically to patients 47% are offering a patient portal 43% are providing marketing or educational materials electronically 31% are offering a personal health record 21% are offering remote monitoring to enable patient information to be transmitted to provider via electronic network
8 Common myths about patient engagement Patient engagement is too academic Patient engagement has proven helpful to all groups of people, including both the elderly and the less educated Patients do not want to be engaged An overwhelming 95% of patients that want to be engaged in their care model Informed and involved patients are expensive Research actually shows that engaged patients generally go towards more conservative treatment options.
9 Patients partnering with Providers 55% of patients want to communicate with their doctors via to exchange health information and get answers to questions 40% of consumers said social media did affect their choice of a provider or organization 45% said information online and in social media would affect their decision to get a second opinion 34% said information they found online and in social media would influence their decision about taking a certain medication 32% said this information would affect their choice of a health insurance plan 68% are interested in remote monitoring devices that allow self-monitoring of their condition and electronic reporting of results to their physician (seniors (78%) and consumers with chronic conditions (75%))

12 Using data for a seamless experience Create technologies that acknowledge and respect the patient experience Integration with smartphones mobile health Make self-tracking easier Make following a clinical protocol easier Give clinicians insight into the daily tracking of this data without a patient visit Ask the right questions to collect the right data Patients simply cannot share their data because there is no mechanism for this data exchange Dr. Google is more convenient for questions
13 Inviting Patients into the Care Model ODLs should be very valuable during the course of treatment How do we bring ODLs into the patient record and, in turn, into the care model? Fusion of our patient engagement technologies portal, mobile, social, etc. with the big data discussions that surround ACO and population health ODLs are important to research! 14 Connecting the dots The biggest problem with health care is that we re measuring the wrong things the wrong way. Costs are currently tracked by specialty or department instead of tracking costs by patient with specific condition over the cycle of care Tracking costs using managerial accounting methods would allow providers to correct systemic cost issues by linking healthcare costs directly to the patient, and further into patient behavior, and then directly to process and treatment improvement
