2 PCMH Transforms Traditional Care The Patient-Centered Medical Home (PCMH) transforms the traditional care practice model by replacing episodic care based on illnesses and patient complaints with proactive, coordinated care and a long term healing relationship. 4 Standard 2: Identify and Manage Patient Populations A. Patient Information i2itracks interfaces with PM and EHR systems, accessing all collected demographic data.
