This is the first SRACA Queensland Conference to be held in Townsville for 18 years. The THHS values the contributions of the Central Sterilizing Department s to safe patient care at The Townsville Hospital, rural and community facilities. This morning I hope to achieve three things: 1. You will have an understanding of the services provided by the CSD at THHS. 2. You will see how Quality improvement activities have led to service improvements through a specific clinical incident. 3.You will learn about the approved redevelopment planned for the CSD at The Townsville Hospital 1

5 STATE: Here are some pictures of some of our campus facilities. QUESTION: Is there anyone here who lives somewhere other than The Townsville Hospital? The health service provides comprehensive health care to a highly dispersed community that spans across facilities in Ayr, Ingham and Charters Towers, along with smaller facilities in Home Hill, Richmond, Hughenden, Magnetic Island and Palm Island. These services also include two aged care facilities and a number of community clinics and health centres that provide community and primary health care services. 5

11 Highlights for our unit will be when we are redeveloped and the floor space will increase almost double. We will have all height adjustable sinks and tables; automatic loading and unloading sterilizers and washer/disinfectors; an electronic tracking system to enable us to track instruments throughout the whole cycle of use and processing; a 4 chamber Index washer/disinfector; individual task lighting and a fully equipped Loans room. 11

12 The Central Sterilizing Department at TTH supports 12 operating theatres, acute services, multiple oral health sites including Palm Island, Community Health, Townsville RFDS and community services. CSD is embarking on a significant redevelopment project that will include automation of machinery and an electronic tracking system for trays and instruments. Each work unit at The Townsville Hospital has Tier 3 Metrics (service level measure) agreed for their service. These are stretch or safety targets that are identified by the NUM or ND as being crucial to patient safety, operational performance or being a corporate governance requirement. 13 Staff and managers are provided with this type of data that assist in providing information on why there are deviations to the Tier 3 KPI performance Sterilizers: we have 6 steam sterilizers and 3 Plasma Sterrad We are now a 24 hour 7 day a week service since the increase from 10 to 12 Theatres in Planned expansions in theatres, two additional theatres will add to CSD workloads when they open in

15 Initial surgery patient underwent a muscle biopsy on left thigh. Through processes that have been refined and honed through guidelines, recommendations and clinical learnings the CSD team adheres to strict processes to review pack, cleaning and visualisation of instruments CSD noted when cleaning instruments tip of scissors was missing. Instruments visualised with good light and viewer. Theatre notified by CSD staff member who took the scissors up RESILIENT Health Care Check and Challenge Patient was still in EDSU, Patient x-rayed, patient informed by surgeon consent obtained to remove scissor tip Second procedure removal of retained scissor tip. 6mm piece of metal(scissor tip) removed under local anaesthetic Uneventful recovery in DSU. Patient to be followed up in Mackay Outreach clinic in two weeks for wound review,scrub nurse completed prime in consultation with CNC advice sought from Patient safety sac 3 prime lodged as per phone conversation. With the scissors open it is fairly obvious however with them closed it is not so obvious. NUM and Perioperative and critical care ND notified by CNC CNC went to CSD praised CSD staff for noticing and notifying theatre sent to staff involved re review of prime to be organised by CNC 15

16 Workflow options including modernisation through automation e.g. Health Technology machines Systems Equipment Devices Services The Townsville CSD capacity study was conducted in February 2010 and was modelled against the Clinical Service Plan for an increase in Operating Theatres and Procedure Rooms. This indicates an investment need towards a larger wash-line and a reduced sterilizer-line though with automation. 2) The current TTH-CSD area is undersized to meet the proposed surgical service demands (refer to Preliminary CSD Business Solution Design) and will require a staged area allocation and redesign to 16

17 Five Stage Plan Ensure The Townsville Hospital CSD refurbishment is undertaken over five stages and implemented in the four year timeframe to deliver alignment with TTHR Project, as suggested in section 4. Support the Stage 1 implementation of new Batch- Washers, Sterilizers and CSD Plant estimated to cost approximately $1.444M in equipment and funded by both the HTER & QH Sterilizing Programs. This has been good in one way but the deautomation of the line is frustrating but required as part of the staging programme. Provide Stage 1 construction through the relocation 17
