2 Note from the Humanitarian Coordinator After the devastating earthquake of 12 January 2010 and the cholera epidemic that emerged in October 2010, the humanitarian community continued working throughout 2011 to provide assistance to all affected people. Despite these efforts, enormous humanitarian challenges still persist, such as the vulnerability of the Internally Displaced Persons (IDPs) still living in camps, the localized cholera epidemic that re-emerged with important outbreaks with the onset of the rainy season, the vulnerability of the population affected by food and nutritional insecurity and the consequences of the hurricane season which runs annually from June to November. Although the victims of the 2010 earthquake have been slowly returning to their communities, early in 2011 there were still about 1.2 million people displaced across the country. Towards the middle of the year the return movement accelerated but at the end of the year there were still about 550,560 IDPs in camps. Despite these challenges and the scale of identified needs requiring an urgent response, the level of humanitarian funding remained below expectations, while projects to bring sustainable solutions are slowly implemented. During this year, the Consolidated Appeal Process (CAP) was funded at 58.9% in addition to important contributions granted outside the CAP. Similarly, the Emergency Relief and Response Fund (ERRF) did not receive the same enthusiasm from donors as in 2010, just after the earthquake (US$81.6 million). In 2011, only US$471,647 were received from three donors. Thanks to a balance of US$ 13,872,180, in addition to these new contributions, the ERRF responded to the cholera epidemic in an effective, coordinated and timely manner. The Fund provided US$8,378,592 to fund 22 projects directly related to the cholera epidemic response. This significantly contributed to reduce mortality. Free access to curative and preventive health care was ensured through the strengthening of the community empowerment approach and sensitization on the need to change behaviors. Two Fund Evaluation/Audit missions were conducted in 2011 and their conclusions have been particularly instructive. Indeed, I have decided to strengthen the governance of the Fund to ensure greater transparency and efficiency by setting up, with reviewed and clarified terms of reference, the Advisory Board that I chair myself and the Review Board supervised by the Head of OCHA Office in Haiti. At the end of the year, the ERRF secretariat, which suffered a destabilizing turnover that hindered its continuity and longterm vision, was given a permanent staff. Please allow me to remind you that Haiti remains very fragile and plagued by deep structural poverty. Therefore, I urge the donor community to devote special means to help meet the humanitarian needs that still remain high for the IDPs, worsened by outbreaks of epidemics and the potential impact of natural disasters, notwithstanding the support to state structures and to all partners involved in the country s socio-economic recovery. 3 Executive Summary Following the earthquake of 12 January 2010, the hurricane season and the cholera outbreak in October of that year, Haitian communities fell into a state of extreme health, social and economic vulnerability. The year 2011 was marked by the continuous efforts of the international community in general and the humanitarian community in particular, to respond to the urgent needs of IDPs in camps across the country, to the temporary displaced persons as a result of the hurricane season and to the cholera epidemic victims. Social and economic recovery interventions were initiated, although timidly. Throughout 2011, the ERRF did not receive the same enthusiasm from donors as in the previous year. The fund recorded only $471,647 from three donors: Brunei Darussalam with $61,338, Republic of Congo with $400,000 and Moldova with $10,309. Despite this under-funding and the weak socioeconomic situation, the ERRF provided rapid and flexible funding to in-country humanitarian actors to address urgent and unforeseen humanitarian needs. The fund served as additional funding to initiate, accelerate, enhance or complement the overall emergency response in Haiti. In addition, the Fund focused its action on responding to the devastating cholera epidemic which affected approximately 473,649 people. Funding was part of a joint effort of the humanitarian community in support of the efforts of the Haitian Government and, especially the Ministry of Public Health to reduce the impact of the epidemic at the community level. By the end of year, the cholera mortality rate decreased from 2.4% in November 2010 to 1.4% in September Central Emergency Response Fund (CERF), made available to Haiti to assist in responding to the cholera epidemic. This need was highlighted in the CAP for The ERRF filled critical gaps by funding 22 projects for a total of $ 8,378,592, all aimed at providing an urgent response to the cholera epidemic. Three projects were implemented by one UN agency and IOM, totaling $1,068,738, while 19 projects were implemented by NGOs and the Red Cross and Red Crescent for $7,310,854. The clusters that benefited the most from the Fund within the cholera response framework were Water, Sanitation and Hygiene (WASH) with 10 projects for $ 3,787,762 and Health with 6 projects for $2,247,044. Other clusters such as the Multisectoral, Logistics and Protection received respectively $1,452,620 for three projects, $693,365 for two projects and $197,800 for one project. The Department of the West had the highest number of people in camps and received the bulk of funding from the ERRF ($3,959,870 for 10 projects); while the Artibonite department, where the first cholera outbreak was reported, received $1,606,465 for four projects. The remote Department of Nippes, with little presence of humanitarian partners received $1,216,028 for three projects. The South East and Central departments received $330,650 and $300,000 respectively. Two projects were funded nationwide with $965,579. In order to avoid duplicating efforts, the ERRF played a complementary role to the Rapid Response allocation of $10,371,212 of the 2

The project aimed to reinforce and harmonize public communications between the humanitarian actors, the media and the government of Haiti. Haitian media became partners in emergency public information by synchronizing their communication with all partners. ERRF s added value to the project: Haitian population was better informed about the cholera outbreak and the risk of potential natural disasters and the nature of the available emergency assistance they could expect and how and when to access it. 8

ERRF s added value to the project: The Fund contributed to the implementation of well adapted sustainable solutions, and allowed communities to increase their access to safe drinking water, and their knowledge about the cholera epidemic through mass awareness campaigns. Overall, the activities helped to reduce cholera in the country. 9

11 Summary and analysis of achievements In addition to the huge humanitarian needs caused by the earthquake of 12 January 2010, the country was struck by a cholera epidemic in October 2010, affecting approximately 473,649 people. This increased the vulnerability of communities living in extreme poverty. During 2011, the humanitarian community focused on responding to this new and devastating epidemic. The ERRF provided funding to 5 clusters (Health, Logistics, Multisector, Protection and WASH) for 21 projects amounting to $8,080,810 to respond to the urgent needs brought by the cholera epidemic. in caring for cholera victims. It established 4 CTC in areas where none existed, distributed 1,518 cholera prevention kits and sensitized 11,400 people. The International Medical Corps (IMC) was granted $300,216 to implement a project in the department of Artibonite (St. Michel) and South (Les Cayes). The project established 2 CTC and 4 ORP, treated 570 cholera cases and trained 211 health care providers in cholera management. It also raised the awareness of 55,534 people on good hygiene practices and provided cholera kits containing 12,843 aquatabs for water purification. 1. Health The Humanitarian Coordinator (HC) approved six projects for a total of $2,247, Funding enabled cholera affected communities to access free health care through the CTC and ORC. In addition, medical facilities and support supplies were provided as well as the necessary inputs and equipment while communities awareness was raised on issues such as good hygiene practices, water treatment and better knowledge of the cholera epidemic. The project implemented by The Alliance for International Medical Action (ALIMA), with a budget of $549,883, focused on health care activities for 3,178 cholera patients and awareness building for 6,200 people. To date, each community office is working on a database, that the nurse in charge at each community follows for each target community in Kenscoff, Croix des Bouquets and Petionville in Port-au-Prince. The Netherlands Red Cross (NRC) implemented a project of $330,650 in the South East Department. The project aimed at strengthening the capacity of 88 health workers The International Organization for Migration (IOM) project in Artibonite, with a budget of $299,959, installed 45 ORP and provided treatment to 265 cholera patients. In addition, the project trained 134 persons on cholera management, provided a mobile team for epidemiological monitoring and awareness building in the high risk rated communities. In addition, 250,000 people were sensitized on good hygiene practices and knowledge of cholera. With $ 350,331, the Unité de Recherche et d Action Médico-Légale (URAMEL) provided basic health care to about 24,700 people around the Champs de Mars in Port-au- Prince. URAMEL installed three ORP and assisted approximately 1,670 cholera cases, referred 265 cholera cases to treatment centers, and sensitized 24,000 people. The project also dealt with 30 cases of sexual violence. With a budget of $416,005.30, Viva Rio s project implemented water and sanitation activities in the poor neighborhoods of Bel Air in Port-au-Prince. The project created 60 Cholera Treatment Units (CTU) and 10 ORP and trained 207 community volunteers on cholera. The project treated 677 cholera cases 10

12 and referred 30 cases to the CTC. 20,000 people benefited from awareness building activities. 2. Logistics With the hurricane season underway and the cholera epidemic at its peak, the humanitarian community was facing a critical shortage of storage and transport means of supply and humanitarian inputs. The HC provided $693,365 for two projects, one implemented by the World Food Programme (WFP) for $469,998 and the other by Handicap International (HI) for $223, The WFP project made two helicopters available to 33 humanitarian organizations for the transportation of passengers (2,838) and freight (510 tons of food and NFIs).. HI implemented a project to increase the storage capacity of humanitarian inputs and other medicines required to respond to the cholera epidemic. The funding enabled the Department of the West, particularly the Petit and Grand Gôave, access to 730 cubic meters of storage space. Furthermore, the centralized management of needs enabled beneficiary partners to pay less attention to the inventory of incoming stocks and to focus more on the operational response to the epidemic. 3. Multisectoral The HC allocated $1,452,620 for 3 multisectoral projects focusing essentially on WASH and Health activities. Ananda Marga Universal Relief Team (AMURT) received $496,873 for capacity building for communities on the cholera epidemic.the project built 75 public latrines, protected 12 water sources, set up 15 hygiene committees and trained 135 community hygiene promoters. It conducted a good hygiene practices awareness campaign that reached about 72,000 people in 100 villages in the Artibonite. Première Urgence/Aide Médicale Internationale (PU/AMI) implemented a project of $520,300 in the commune of Carrefour in Martissant and Fontamara areas in Port-au-Prince. The medical care received benefitted 262 cholera patients across 43 ORP. The project trained 86 community hygiene agents who sensitized 34,493 persons and distributed cholera prevention kits in 9,549 households. Care centers received 18,004 Oral Rehydration Salts and 565 homes and 300 latrines were decontaminated. Viva Rio implemented a project with a budget of $435, that enabled some 35,000 people to gain access to safe water through six rehabilitated standpipes connected to the metropolitan network, the daily chlorination of approximately 12m³ of water, and the construction of 30 latrines. The project enabled school and health facilities to use methane gas produced by three bio-digesters using household waste. 4. Protection Internews received $197,800 to strengthen the coordination capacity of Government, humanitarian partners and the media. It assisted in harmonizing messages on cholera and other natural disasters affecting the country and, in turn, the media became a full partner in the dissemination of emergency related messages. Ultimately, the Haitian population was more aware of the dangers of cholera and natural disasters through this project, and also on the nature of the assistance that could be expected and especially how and where to get it. 5. Water, Sanitation and Hygiene Structural poverty of the population, overcrowding of IDPs in camps following the earthquake, limited access to water in sufficient quantity and quality, poor environmental sanitation and poor hygiene practices were a 11

13 breeding ground for and facilitated the rapid spread of the cholera epidemic. The cluster received $3,489,981 (43% of overall funding for the year 2011) to fund 9 projects. Project activities allowed communities affected by cholera to access water in sufficient quantity and quality, to rehabilitate/ construct water sources or supply water by water trucking and establish ORPs. All projects implemented included an important input on community awareness building in good hygiene practices. Agence de Coopération Technique et de Développement (ACTED) implemented a project of $509,417 project in the Bas Artibonite area. The project rehabilitated a potable water conveyance system, 26 water sources, distributed 10,000 cholera prevention kits, and built 30 latrines and 70 hand washing stations. It set up 20 chlorine production units, 30 water chlorination posts and 50 ORP. A total of 180 community hygiene promoters were trained and equipped and approximately 20,000 were sensitized on good hygiene practices. CARE International received $372,526. The project maintained 10 ORP, created 10 new ones and equipped 20 operational posts. The project has trained and equipped 60 agents and 6 supervisors and sensitized about 382,000 people on good hygiene practices related to cholera. Cooperazione Internazionale (COOPI) received $171,953 for a project implemented in Port-au- Prince in the IDP camps of Tabarre. The project provided 1,124m³ of water through water trucking. This activity was phased out with the drilling of seven wells equipped with hand pumps and the repair of four other pumps. The community has benefited from 8 ORP as well. The project trained 10 water committees, collected 1,296m³ of solid waste and maintained 41 latrines, 92 showers and 14 stations for hand washing and drained public areas at risk. Food for the Hungry (FH) received $300,000 for a project in the localities of Kenscoff, Bellevue Mountain, the Cadets, Sile, Caradeux and Marre-Roseau in Port-au-Prince. The project distributed 1,000 prevention kits, trained 212 technicians and 24 community water chlorination mobilizers. It has raised the awareness of about 100,775 people. International Rescue Committee (IRC) was granted $492,583 used to set up 105 water chlorination points, train 300 community mobilization brigades, and distribute 4,990 cholera prevention kits. The project treated some 6,580 cholera cases and sensitized about 140,000 people. International Organization for Migration (IOM) has implemented a project of US$297,781 at national level. The project aimed to reduce public health threats posed by damaged sanitation infrastructures in IDPs camps. It repaired 452 toilets in 17 camps and decommissioned 104 toilets in 11 camps. About 60,000 persons were sensitized on the link between non functioning sanitation facilities/environmental pollution and public health risks Mercy Corps implemented a project of $300,000 in the Centre department in Hinche and Mirebalais. 56,000 households benefited from the chlorine produced by 84 production units in the communities while 52,924 other households received aquatabs. Awareness raising activities reached approximately 636,000 people. Première Urgence/Aide Médicale Internationale (PU/AMI) received $500,000 for support and awareness activities. The project established 43 ORP, distributed 12,000 cholera prevention kits and disinfected approximately 8,462 latrines, and sensitized 35,000 people. Solidarités International (SI) implemented two projects in the department of Nippes with funds 12

14 amounting to $843,508. The second project reinforced the achievements of the first in the municipalities of Miragoane and Fond de Nègres while expanding its work in the towns of Plaisance du Sud and Baladère. The first project of $ 543,508, distributed 1,950 cholera prevention kits and established 72 water chlorination points that treated 3,613m³ of water when the water trucking for the CTCs of Fond des Nègres received 474m³ of drinking water for 1,376 recorded patients. Training activities benefitted 60 community mobilizers, 144 chlorination technicians and 54 disinfection committees that disinfected 1,247 houses. Awareness building efforts reached over 50,000 people. The project also rehabilitated 13 water sources, built 7 public latrines and established 8 ORP. The second project, amounting of $300,000, implemented by SI, built 85 chlorination points after training and equipping 38 chlorinators that cleaned approximately 1,360m³ of water. The project then trained 27 brigade members, rehabilitated 2 water supply systems and 12 water sources. It also distributed 1,848 cholera kits, disinfected 64 houses and raised the awareness of approximately 58,000 people. 13

15 Project Monitoring The turnover of ERRF managers did not allow the continuity of action in Only six out of 19 projects were evaluated. However, once the ERRF Manager was hired, he trained the staff of the Project Audit Project audits remain the main issue in managing the Fund. A total of 13 projects from 2008 to 2009, whose audit was conducted in 2010, have still not been paid the last installment. The quality of work was unsatisfactory and in 2011 the reports were reviewed several times. In March 2012, the final versions were sent to Geneva. Partners expect that the balances of their projects would be disbursed soon. The 2011 project audit has not yet begun. Klynveld Peat Marwick coordination section, including those in the field, on the use of the assessment format, and all projects funded in 2011 were evaluated with the support of the Coordination Section. Goerdeler (KPMG) has an ongoing audit contract for 41 projects, including 40 projects in 2010 and 1 in It should be noted that for 2010 and 2011, there are respectively 5 and 18 projects to be audited. Currently there are efforts (UNDP/OCHA CO) to hire a firm to audit these projects and those that will be funded and implemented during Gender Consideration The narrative reports of the projects implemented in 2010 and 2011 did not disaggregate in a systematic manner data on women, men, girls and boys. Consequently, in 2012, an effort will be made to require the ERRF partners to provide this data in their project proposals and interim/final reports. To this end, a results sheet was developed to bring the organizations receiving ERRF funds to provide disaggregated data on men, women, children and other vulnerable categories. Since August 2012, the ERRF Haiti will implement the Gender marker. All the projects will take into account the gender approach. For this, OCHA Policy Development and Studies Branch (PDSB) will deploy a GenCap Advisor from Geneva to train humanitarian actors in Haiti. It is planned to reach about 60 persons in Port-au-Prince (West department) and 20 persons in Gonaïves (Artibonite department) from UN agencies, international and national NGOs and Gouvernment structures like The Direction de la Protection Civile (DPC), Direction Nationale de l Eau Potable et de l Assainissement (DINEPA). Conclusions and Recommendations Conclusions In addition to mobilizing assistance to earthquake survivors, the humanitarian community responded to the cholera epidemic that caused damage in a greatly unfavorable human, social and economic environment. 14

16 The response to the cholera epidemic was quick and efficient despite the very difficult humanitarian context. Although the short-term results were satisfactory and cholera cases under treatment were diminishing, it remains no less true that the sustainability of actions will depend solely on the handling of the crucial questions of holistic human development and cultural change. The ERRF support to cholera response is expected to be limited in 2012 due to the closure of the Fund. Recommendations The recommendations contained in the 2010 report still remain valid. Nevertheless it should be recalled that measures have already been taken to improve the performance of both management and programs. Project Proposals The quality of project proposals has improved and better describes: Direct beneficiaries; Quantified results; And implementation and monitoring strategy The cross-cutting issue of Gender must go beyond the statement of good intentions and be translated into concrete actions through project rationale, results, indicators and activities implemented. A new template, which follows the official ERF format, was developed to reflect this need. This template includes better guidance on the content of sections and also adds new sections that drive partners to provide the maximum information for a better understanding of the project. Reporting The narrative reporting very often lacks supporting figures on the results achieved. To correct this, a record summarizing results was introduced with the purpose of obtaining detailed results, indicators and gender disaggregated data. Monitoring Audit It is recommended to contract a firm to rapidly audit not only the 23 projects of 2010 and 2011 It is recommended to strengthen project monitoring activities without presuming to carry out an Evaluation - to identify project strengths and weaknesses and make the required adjustments, if necessary. A monitoring format was developed to help nonspecialists in Monitoring and Evaluation. Training was given to the OCHA Coordination Section staff in Port-au-Prince and also in the field. They support the project monitoring activities. but also those that will be implemented in Gender 15
