2 Dear Friends, MCFI is a community resource for children, teens, adults and older adults with special needs and their families. The agency was founded more than 75 years ago and has experienced extensive growth through progressive programming in support of people with disabilities and other challenges in Wisconsin. In 2014, the agency completed a reorganization which places MCFI and its affiliates under a parent organization, Centers for Independence. The reorganization allows the agency to consolidate administrative and support functions and promote cost efficiencies. In 2014, MCFI and affiliates nearly 24,000.* More than 3,000 MCFI staff members and volunteers accomplish MCFI s mission through the delivery of over 50 programs under the organization s core service lines: Behavioral Health, Children s Services, Employment Services, Long-term Care, Neurorehabilitation, and Nutrition Services. MCFI core programming has been accredited by CARF, earning its highest rating, and signaling that the agency is committed to continually improving services, encouraging feedback and serving the community. Children s Services Every day more and more children are diagnosed with developmental delays. Our Children s Programs serve nearly 600 children, from birth to age 21. Nearly all More Than 600 CHILDREN annually of the children live in Milwaukee, where the poverty rate for children under 18 has reached 42.1 percent. MCFI has been able to respond to the growing needs of children who are considered developmentally at risk of failure by integrating medical care, wellness, education, nutrition and support for their families. We help kids succeed both academically and personally. Highlights of our Children s Programs are: SmartBaby 100 percent of babies born to mothers enrolled in the SmartBaby program over the past year were full-term and of normal birth weight. Birth to Three Over 80 percent of the children with significant developmental delays showed improvements in motor, language, feeding, cognition and social/emotional skills. Pediatric Skilled Nursing Serves children with special health care needs. In 2014, children between the ages of 1 and 4 enrolled in this program area were on track to master essential skills, based on national assessment tools. This is the only community-based day skilled nursing program in the state accredited by the Joint Commission. School for Early Development and Achievement A Marquette University researcher, using four different assessment tools, found that SEDA students executive functions have improved using our curriculum. SEDA is a unique laboratory school emphasizing developmentally enhancing curriculum for at-risk children. *This total includes affiliates MCFI Home Care, School for Early Development & Achievement, New Health Services and ilife.

3 Behavioral Health This program has begun to provide primary care services to our behavioral health clients, appropriately re-branding from TLS Behavioral Health to the Whole Health Clinical Group. The focus on clinical care more effectively supports recovery in both behavioral health and physical health. The agency was recently awarded a 4-year federal grant to support the integrated health services approach. Over 2,400 Employment Services MCFI Employment Services received two competitive State of Wisconsin, Department of Workforce Development grants to train and hire unemployed and underemployed (1,000) and those with disabilities (50) to work in health care occupations such as Certified Nursing Assistants. The agency introduced a new training program called TalentUp this year. The purpose is to provide industry-based training, soft skills education and work readiness activities to prepare for entry into the workforce. The program is targeted to with barriers to work who do not qualify for traditional funding streams and would otherwise be unable to secure trainng and work preparation activities. TalentUp is offered as a training program in the food processing and preparation industry. Long-Term Care The Long-Term Care program, Center for Healthy Aging, provides clients with socially enriching, cognitively stimulating and wellness oriented activities. The program has achieved positive results in helping seniors maintain or improve functioning. Client assessments indicated Over 300 NEARLY 1,000 that 59 percent maintained or improved cognitive functioning and that 62 percent maintained or improved their physical functioning. The average age of the participants was 75 and 46 percent were taking medications for dementia or Alzheimer s.

4 Neurorehabilitation over 450 Our Neurorehabilitation inpatient and outpatient programs focus on recovery and regaining independence after accidents, injuries, or strokes resulting in brain injuries. In 2015, 78 percent of clients participating in our outpatient programs showed improvement in functionality using a national assessment tool. In August, MCFI opened a new kitchen at the Innovations and Wellness Commons, a central city redevelopment project. The agency will provide jobtraining opportunities at the facility and accelerate local economic development. Nutrition Services MCFI is Milwaukee s largest nonprofit food management company, currently operating three main production kitchens and five smaller food preparation kitchens within Milwaukee County. Fran Wiesner MCFI Nutrition Services is a partner in the summer months with the Hunger Task Force, which serves more than 825,000 meals at parks and playgrounds. MCFI more than 40,000 meals as part of the program this year to children who rely on this food resource. Howard L.
