5 DM DRAFT I. Call to order and approval of the November 4, 2015 Board Meeting minutes. A. Approval of the November 4, 2015 Board Meeting minutes. Action: Ms. Warner moved to approve the November 4, 2015 Board Meeting minutes. Ms. Rudd seconded the motion and it was passed unanimously. II. Old Business: A. There was no old business. III. New Business: A. Community Transportation Coordinator Evaluation Mr. Vanselow said the evaluation was conducted January 7, The evaluation committee consisted of: Ms. Janice Flowers, Mr. Rob Forbus, and Mr. Bryan Taylor. Mr. Vanselow reviewed the evaluation information. The operator for the county is First Transit, Inc. for trips. The CTC will seek to have more coordination contracts. Ms. Lohr said the CTC is allowing 60 minutes pick-up window time on the front, and on the return trip the pick-up time window time was 30 minutes. She changed the return trip pick-up window time to 60 minutes due to the distance of some trips. Ms. Warner said if someone is a no show for their trip it counts as two no shows to include the return trip. She said this was to be changed to one no show and asked if it was in fact changed to one no show. She noted the number of no shows is high. She asked if trips are denied after a number of no shows. Ms. Lohr said in the TDSP there is a process for suspension from the system if there is a pattern of no shows. Ms. Rudd asked if 2,695 no shows was a correct number. Ms. Lohr said the number is accurate and represents scheduled trips for which clients did not show. Ms. Boyer Rutherford noted the no shows only represented approximately 3% of the trips. Mr. Vanselow reviewed the results from the ride along trip done by Mr. Rob Forbus. Mr. Forbus said the driver was professional and friendly. Ms. Janice Flowers also rode the system for two trips. Both trips went very well. Regarding passenger satisfaction, of 61 respondents, 74% rated the system very good. The only operator providing transportation is First Transit. Mr. Vanselow reviewed the remainder of the evaluation. Mr. Volpi noted on accidents and road calls the miles were not consistent. Mr. Vanselow said he would check the information. Mr. Volpi said the surveys indicated there was not a number posted for the helpline. Mr. Vanselow said those are the calls that go directly to Tallahassee, and Tallahassee was not able to provide the numbers. Ms. Hale asked if the pickup window is from the time the person gets to the doctor s office, or from the time that they are finished at the doctor s office. Mr. Vanselow said the pickup Bay County TD Minutes Page 2

6 DM DRAFT window is the time the person is picked up at their house or original pickup point. If the original pickup is 8:00 a.m., according to the Service Plan they can pick up 7:00 a.m. to 9:00 a.m. However, the CTC is operating under a 60 minute window meaning if the pickup time is 8:00 a.m., they can pick up at 7:30 a.m. or 8:30 a.m., allowing a one-hour window. He said the Service Plan had a two-hour pickup window under the previous provider, and that remained in the Plan. Action: Mr. Forbus moved to approve Ms. Hale seconded the motion and it was passed unanimously. B. Community Transportation Coordinator (CTC) Report Mr. Gene Keen, the operations manager for First Transit, reported to the Board. He said the second quarter covered the period including October, November, and December He reviewed the second quarter report. He said reschedules were estimated due to a staff change. Action: Information Only C. Regional Planning Council Quarterly Report Mr. Vanselow reviewed the quarterly report and explained that the report is submitted to the Commission for the Transportation Disadvantaged. He said staff attended the Transportation Disadvantaged annual training. Action: Information Only D. Commission for the Transportation Disadvantaged Correspondence Mr. Vanselow said the next scheduled business meeting is April 8, The draft allocations for the counties was included in the agenda package. He said there is a possibility of Bay County receiving an additional $134, for trips. The CTC will be applying for two competitive grants. One grant is for existing services and the other grant is for additional transportation beyond what is already being provided. Mr. Keen reviewed accidents that occurred in October, November, and December There was one commendation for the quarter. There was general discussion. Action: Information Only III. Other Business A. Annual Board Training Mr. Vanselow said the main funding source for Transportation Disadvantaged is the Transportation Disadvantaged trust fund from the state of Florida. Other funding sources are federal, local, fares, and copays. Mr. Vanselow said the Board s role is to advise the CTC, develop the Service Plan, improve deficiencies, and determine the level of service. He said the board members are advocates for the riders. He stated that the TD meetings are held in accordance with the Florida Sunshine Laws. Bay County TD Minutes Page 3

7 DM DRAFT B. Ms. Warner said she manages the respite center at the Council on Aging. The respite center is a daycare for seniors with dementia. The patients live at home with their families and the families care for the seniors 24-7 and the families need a break. The clients come to the daycare 2 to 3 days a week. Ms. Warner said some of the clients need transportation to the daycare. Ms. Warner said the Council on Aging has been paying for the transportation. They have six clients that ride the bus to and from the respite center. They pay for the transportation out of a local grant that the Council on Aging receives from the Faith Foundation; however, they will not be able to do that anymore as those funds are gone. Ms. Warner said Ms. Angela Bradley sent her a copy of the application. Ms. Warner sent the application to the care givers of the people who have their loved ones riding on the bus. She said the application is complicated and is eight pages long. The application asks for proof of income with the completed application. Ms. Warner said the non sponsored transportation does not have income guidelines. She said the caregivers do not have the income. Ms. Warner said the second issue is that there is a requirement for the application to go to the client s doctor, and the doctor has to fill out two pages. On the application the doctor is asked if the client has been diagnosed with a cognitive, mental, physical, or other disability. She said the Council on Aging has to mention if the client couldn t ride the trolley. Then the doctor has to say whether or not the person can communicate addresses, destinations, phone numbers, read and/or monitor time, follow instructions, or deal with unexpected situations. Ms. Warner said none of the questions apply to their clients. She said the caregiver at the home puts the client on the bus and the bus goes to the respite center. Staff at the respite center take the clients off the bus. The clients don t have to navigate anything or answer questions. Ms. Warner said the transportation was provided previously by Tri County. Tri County for a time contracted with the respite center. The respite center provided transportation on their own buses, and Tri County paid the respite center out of non sponsored funds for the trips. Then Tri County decided to take the service back and Tri County transported the clients. Then the respite center had a fund come become available and the respite center started transporting the clients. The respite center no longer has the funds available. She said the application is just submitted and nobody talks to them to ask what kind of transportation is being applied for. Ms. Warner said the first application has been completed by one of the caregivers. The caregiver sent it to the doctor and it was completed. The application was turned in. The time allowed to process the application is 21 days. The applicant, Ms. Missy Pizza, had received her answer, was at the meeting and Ms. Warner asked her to speak. Ms. Pizza said her uncle goes to the respite center and he rides the van when she is not able to take him. She said that now she takes him back and forth because of where she is working. She takes him earlier than the van would pick him up, and he stays longer so she can work longer. She said that would not always be the case because she is switching jobs. She said she relies on the transportation three days a week. She said she filled the application out. The doctor sent the application back filled out. Ms. Pizza said she received a call. Her uncle has dementia and is not able to take care of himself. He has moved in with Ms. Pizza. She said the transportation allows her to work without worry about her uncle. At first she was not aware of all his impairments and tried to send him on the trolley to the senior center. The doctor had suggested her uncle Bay County TD Minutes Page 4

8 DM DRAFT needed to interact with other people; however, it did not work out. Her uncle was very belligerent at the center and wanted the driver to allow him off the van at the wrong place. He needs someone with him at all times so he doesn t get off the van at an inappropriate point. Ms. Pizza said Bay Area Transportation contacted her and approved her uncle for the transportation. Bay Area Transportation asked where they lived, i.e. if they lived in the city. Ms. Pizza said they don t live in the city. They live between Lynn Haven and Panama City in an area in the county. Bay Area Transportation said they would have to schedule between 9:30 a.m. and 2:30 p.m. Ms. Pizza said the hours that her uncle goes to the program are 8:00 a.m. to past 4:00 p.m. The schedules are not compatible. Ms. Pizza s work hours weren t within those hours. Bay Area Transportation agreed to accommodate the hours. However, the uncle needed a personal attendant and Bay Transportation does not provide attendants so he could not ride the van. The option is that a personal care attendant could ride in a car behind the van and follow them to the center when they drop him off the personal care attendant could leave. Ms. Pizza found this ridiculous in that if the attendant could drive to the center the attendant could drive the uncle also. Ms. Pizza said she is looking for assistance to allow her to continue working and allow her uncle to go to the center. Ms. Warner said the respite center was invaluable in that it deterred needing a nursing home for the clients. Ms. Lohr said there are six clients currently riding at various times. She said there was a lot going on and the situation is delicate. She thought the application does need some work. She said TD states that riders are eligible based on disability, age, and no other means of transportation. Ms. Lohr said their drivers are not personal care attendants and they cannot provide attendants. Ms. Warner said her drivers cannot act as personal care attendants either. Ms. Lohr said the physician stated that the client required a personal care attendant. Ms. Boyer said if there is a modification to that, or if the physician modifies his statement stating that he needs a personal care assistant for certain situations but not all. Ms. Lohr said they may need to say that on the application. Ms. Boyer agreed that the application needs some work. Ms. Rudd was concerned as to whether the Board approved the application. She had issues with it stating that proof of income is required. She asked where in statute it says proof of income is required for the disadvantaged program. She said it does not say that proof of income is required. Ms. Lohr said in the TDSP where it states that regarding transportation based on no other means of transportation, that it means you cannot afford transportation. She further stated that then the LCB will adopt something that says we are going to base whether or not they can afford it on a threshold that is 150% below the federal poverty line. Ms. Rudd said the Board has not done anything like that in the Service Plan. Mr. Vanselow said the only thing the Board did was adopt eligibility. The eligibility stated in order to determine application meets program criteria you must submit a completed application. In order to be eligible the applicant must have no other means of transportation, and at least one of the following. He said that was where it was unclear in Bay County TD Minutes Page 5

9 DM DRAFT the application. He said one is that you don t live on a bus route; two, six years of age; three, a disability prevented you from using bus routes; and four, applicant income level falls below poverty guidelines. Mr. Rudd said that it said one of them. Mr. Vanselow said the CTC had discussions with him prior to the last meeting about applications. He said they wanted to make sure is that it is consistent with the TDSP. He said the current application may not be consistent with the TDSP. Ms. Rudd was not sure the rider needed to get the doctor s certification. She thought that was only necessary under ADA. Ms. Lohr said the doctor s certification was to verify the disability, not that they couldn t use the fixed route. Ms. Warner said they qualified under the age qualification. Ms. Rudd said they don t need to go to the doctor if they qualify under age qualifications. Ms. Lohr said the application said under #3, if you are applying for transportation disadvantaged service, and have a disability, medical verification section of this application is required and must be completed by a medical professional. Ms. Boyer said it should have said, if you are applying because you have a disability. Ms. Rudd said the same thing applies with the income. She said it appeared that the client has to qualify for all reasons to get it. Ms. Lohr said it all had to be filled out and it has to go to the doctor because that is what it says. Ms. Lohr said the application form must be completed by a medical professional if you are applying for transportation due to a medically verified condition only such as cognitive, mental, physical, or other disability. Ms. Rudd suggested getting some examples from other areas in order to get something that is simpler. She said the Commission requires an application process. Ms. Warner said her transportation ends February 29, 2016 because they have no more money. Ms. Lohr asked that the names of the clients needing transportation be sent to her. Ms. Warner said she would send the names to Ms. Lohr. Ms. Lohr asked Ms. Pizza if her uncle is okay. Ms. Pizza said the uncle needs an attendant. Mr. Vanselow said sometimes they can hire an agency. He did not know how that would be put in words on an application. Ms. Warner asked for a better application because dementia is involved. Mr. Vinski said maybe the medical part is needed for some cases. Mr. Vanselow said the medical portion is on the application because if somebody says, do you need a personal care attendant to go to the doctor on the 12 th floor, they are going to say no. Then they will get into the building and just wander around. He asked how that can be prevented and still allow going to a daycare center. Ms. Lohr said this one was physician verified. Ms. Rudd suggested to Ms. Lohr that clients be considered on a one-on-one basis. Ms. Lohr asked to work on the application further and discuss the matter at the next meeting. Mr. Vanselow said if an application is done soon they could work with some agencies to determine if it is acceptable. He said in the meantime to communicate with clients that are having trouble to avoid people not applying before a new application is developed. C. Grant Review Bay County TD Minutes Page 6

10 DM DRAFT Mr. Vanselow called on Ms. Culbreth to speak on grants. Ms. Culbreth, from Bay County, said the Bay County CTC had applied for FY 2016, for the FDOT 5339 rural and small urban grant. In the grant the CTC requested five standard cutaways for their demand response system. Ms. Rudd said the CTC also made application for Section 5311 rural funds. She said there are rural funds available for areas outside the urban area. V. Public Comments: There were no further comments. VI. Adjourn Bay County TD Minutes Page 7

11 AGENDA ITEM III-A SUBJECT: Membership Certification BACKGROUND: In preparation for the Annual Membership Certification, the board is asked to review the draft membership certification to determine which members desire to continue serving and solicit recommendations for new members for vacancies. The Membership Certification will be presented to the Bay County Transportation Planning Organization for approval prior to sending to the Commission for Transportation Disadvantaged. Members whose term is ending are eligible to continue serving if willing and able. REQUESTED ACTION: A motion and vote to recommend the Membership Certification to be presented to the Bay County TPO for approval. Our Commitment - Mobility

13 AGENDA ITEM III-B SUBJECT: Transportation Disadvantaged Service Plan (Minor Update) BACKGROUND: The planning agency staff, Local Coordinating Board (LCB), and the Community Transportation Coordinator (CTC) jointly develop the Transportation Disadvantaged Service Plan (TDSP) every five years and a Minor Update is completed annually, as required by Rule (4), Florida Administrative Code. Several Federal Transit Administration (FTA) grants now require Human Services Coordinated Transportation Plans. The TDSP will serve as the Human Services Coordinated Transportation Plan for Bay County. Services approved for funding must be identified in the TDSP as a need and included in the goals and objectives. The annual TDSP update will be presented at the meeting for LCB review and approval. * TDSP Update will be handed out at the meeting RECOMMENDED ACTION: A motion and roll call vote to approve the Transportation Disadvantaged Service Plan. 14 AGENDA ITEM III-C SUBJECT: Community Transportation Coordinator (CTC) Report BACKGROUND: The Local Coordinating Board shall on evaluate services provided by the CTC and seek innovative ways to improve cost-effectiveness, efficiency, and safety. In order to fulfill this requirement the board has requested that the CTC provide a quarterly summary of coordination activities and a financial report of sponsored and nonsponsored trip costs in Bay County. The current report reflects some changes that were requested at the last board meeting and is a work in progress trying to summarize services provided. Share any additional information in an effort to keep the Local Coordinating Board informed of the progress being made to the Demand Response System. REQUESTED ACTION: For Information. 16 AGENDA ITEM III-D SUBJECT: Transportation Planning Organization Report BACKGROUND: West Florida Regional Planning Council as designated staff to the Bay County Transportation Planning Organization (TPO) submits a quarterly report to the Commission for the Transportation Disadvantaged on planning grant tasks. The third quarter report is enclosed for board review. REQUESTED ACTION: For information. Our Commitment - Mobility

17 PLANNING RELATED GRANT AGREEMENT TASKS QUARTERLY PROGRESS REPORT Designate Official Planning Agency: BAY COUNTY TPO Invoice #: 3 County: Bay County Contract #: G0208 Reporting Period: January 1, 2016 to March 31, 2016 I. Program Management A. When necessary and in cooperation with the local coordinating board, solicit and recommend a community transportation coordinator, in conformity with Chapters 287 and 427, Florida Statutes. Such recommendation shall be presented to the Commission by Planning Agency staff or their designee. (Task 2) Tri-County Community Council gave notice to cease operations as the CTC for Bay County as of 12/31/2013. Bay County voted to become the CTC during the emergency period 1/1/2014 to 6/30/2014. Bay County Board of County Commissioners (BCC) voted to remain the CTC for a 5 year period beginning July 1, Bay County BCC was approved by the CTD as the CTC at the May 14, 2014 Business Meetinging. Current MOA is from July 1, 2014 to June 30, TASK COMPLETE B. Develop and maintain a process for the appointment and reappointment of voting and non-voting members to the local coordinating board (LCB). (Task 3) The process is in place and a current roster is sent with each invoice. The annual membership certification is scheduled for June C. Prepare agendas for local coordinating board meetings consistent with the Local Coordinating Board and Planning Agency Operating Guidelines. Ensure that operator payments are addressed as a standard agenda item. (Task 3) The agenda for the November LCB meeting was consistent with the CTD Guidelines and included operator payments. D. Prepare official minutes of local coordinating board meetings regardless of a quorum) and submit an approved copy along with the quarterly report to the Commission. For committee meetings, prepare minutes in the form of a brief summary of basic points, discussions, decisions, and recommendations to the full board. Keep records of all meetings for at least five years. (Task 3) The approved minutes of the November 2015 and the draft minutes of the February 2016 LCB meetings are included with the invoice. 07/01/2015

18 E. Provide at least one public hearing annually by each local coordinating board, and assist the Commission, as requested, in co-sponsoring public hearings. This public hearing must be in addition to the local coordinating board meetings. It may, however, be held in conjunction with the scheduled local coordinating board meeting (immediately following or prior to the local coordinating board meeting). (Task 4) The Public Hearing is scheduled for June F. Provide staff support for committees of the local coordinating board. (Task 3) Staff assisted with the TDSP development. G. Develop and update annually by-laws for local coordinating board approval. Approved by-laws shall be submitted to the Commission. (Task 5) Annual by-laws were approved at August 2015 LCB meeting and are included in the invoice. TASK COMPLETE H. Develop, annually update, and implement local coordinating board grievance procedures in accordance with the Commission guidelines. Procedures shall include a step within the local complaint and/or grievance procedure that advises a dissatisfied person about the Commission s Ombudsman Program. A copy of the approved procedures shall be submitted to the Commission. (Task 6) Local coordinating board grievance procedures were approved at August 2015 LCB meeting and are included in the invoice. TASK COMPLETE I. Provide the Commission with a current membership roster and mailing list of local coordinating board members. The membership roster shall be submitted with the first quarterly report and when there is a change in membership. (Task 3) The current roster is included with the invoice. The annual LCB certification is scheduled for June J. Provide public notice of local coordinating board meetings and local public hearings in accordance with the Coordinating Board and Planning Agency Operating Guidelines. (Task 3) A copy of the advertisement for the meeting and public hearing are included with the invoice. K. Review and comment on the Annual Operating Report for submittal to the local coordinating board, and forward comments/concerns to the Commission for the Transportation Disadvantaged. (Task 7) The Annual Operating Report was reviewed at the November 2015 meeting with the LCB. The LCB voted to authorize the Chair to sign the Face Sheet. The signed Face Sheet is included with the second quarter invoice. TASK COMPLETE 07/01/2015

19 L. Review the transportation disadvantaged service plan, and recommend action to the local coordinating board. (Task 1) The annual TDSP update is scheduled for June 2016 LCB meeting. M. Report the actual expenditures of direct federal and local government transportation funds to the Commission for the Transportation Disadvantaged no later than September 15th. (Task 8) Actual Expenditure Report (AER) was completed and submitted to the CTD in September TASK COMPLETE II. Service Development The planning agency shall develop the following service development items. A. Jointly, with the community transportation coordinator and the local coordinating board, develop the Transportation Disadvantaged Service Plan (TDSP) by preparing the planning section following Commission guidelines. (Task 1) Staff submitted an ammended TDSP for Board approval at the November 2015 LCB meeting. The annual TDSP update is scheduled for June 2016 LCB meeting. B. Encourage integration of "transportation disadvantaged" issues into local and regional comprehensive plans. Ensure activities of the local coordinating board and community transportation coordinator are consistent with local and state comprehensive planning activities including the Florida Transportation Plan. No activity this quarter. C. Encourage the local Community Transportation Coordinator to work cooperatively with regional workforce boards established in Chapter 445, Florida Statutes, and provide assistance in the development of innovative transportation services for welfare transition program. The regional Workforce Board is represented on the LCB. III. Technical Assistance, Training, and Evaluation The planning agency shall provide technical assistance and training for the local coordinating board, and shall assist the local coordinating board in monitoring and evaluating the community transportation coordinator. A. Provide the local coordinating board with quarterly reports of transportation disadvantaged planning accomplishments and expenditures as outlined in the planning grant agreement and any other activities related to the transportation disadvantaged program including but not limited to consultant contracts, special studies, and marketing efforts. (Task 9) The quarterly report was included in the February 2016 Agenda. 07/01/2015

Attend at least one Commission-sponsored training, including but not limited to, the Commission's quarterly regional meetings, the Commission's annual training workshop, or other sponsored training. (Task 10) Staff attended the 2015 FPTA/CTD Annual Training and Expo October 25-28, TASK COMPLETE C. Attend at least one Commission meeting each year within budget/staff/schedule availability. 07/01/2015 Staff attended the October 26, 2015 and January 21, 2016 CTD meeting. D. Notify Commission staff of local transportation disadvantaged concerns that may require special investigations. N/A E. Provide training for newly-appointed local coordinating board members. (Task 3) Annual board training and TD review was accomplished at the February 2016 LCB meeting and is also provided as needed. TASK COMPLETE. F. Provide assistance to the community transportation coordinator, purchasing agencies, and others, as needed, which may include participation in, and initiating when necessary, local or regional meetings to discuss transportation disadvantaged needs, service evaluation and opportunities for service improvement. No activity this quarter. G. To the extent feasible, collect and review proposed funding applications involving "transportation disadvantaged" funds consistent with Chapter 427, Florida Statutes, and Rule 41-2, Florida Administrative Code, and provide recommendation to the local coordinating board. No grants received this quarter. H. Ensure the local coordinating board conducts, at a minimum, an annual evaluation of the community transportation coordinator. The local coordinating board shall evaluate the coordinator using the Commission's Evaluation Workbook for Community Transportation Coordinators and Providers in Florida (at a minimum, using the modules concerning Competition in Use of Operators, Cost-Effectiveness and Efficiency, and Availability of Service) and local standards as defined in the Transportation Disadvantaged Service Plan. (Task 2) The CTC Evaluation Committee was approved and appointed at the November 2015 LCB meeting. The CTC Evaluation was conducted in January Bay County met or exceeded all standards evaluated. TASK COMPLETE I. Assist the Commission for the Transportation Disadvantaged in joint reviews of the community transportation coordinator. N/A
