9 NPSG The hospital implements a standardized approach to hand-off communications, including an opportunity to ask and respond to questions

10 Elements of Performance NPSG Interactive communication that allows for the opportunity for questioning between the giver and receiver of patient information Up-to-date information regarding the patient s care, treatment, medications, services, and any recent or anticipated changes

11 Elements of Performance NPSG A method to verify the received information An opportunity for the receiver of the hand-off information to review relevant patient historical data Interruption during hand-offs are limited to minimize the possibility that information fails to be conveyed or is forgotten
