4 Summary of Work Experience and Job Role Work Experience I work in a local NHS Trust hospital and have worked on the Medical Assessment Unit, Accident and Emergency and Intensive Care Unit caring for emergency and critical care patients and providing a compassionate and caring nursing service to the best of my ability. I work in a multidisciplinary team of nurses, healthcare assistants, physiotherapists, doctors, radiographers, and managers. Job Description I currently work in a twenty bedded Intensive Care Unit, caring for critical care patients. I care for patients in a dignified and holistic way, maintaining their privacy, ensuring their cultural needs are met, and provide the patient emotional and psychological support. I am trained to provide many skills which are specific to critical care, as well as having many transferable skills such as such as: Maintaining accurate, detailed reports and records, monitor, record and report symptoms and changes in patients' conditions and record patients' medical information and vital signs as well as modify treatment plans if necessary, consulting with colleagues and superiors to assess, plan, implement and evaluate patient care. I prepare patients for, and assist with, examinations and treatments, and I also request, interpret, and evaluate some diagnostic tests to assess a patient's condition, plan their care and monitor all aspects of patient care, including fluid balance, nutritional intake, pain management, respiratory and cardiovascular function and physical activity and pressure care management. The preparation of rooms, sterile instruments, equipment and supplies, and ensuring that stock of supplies is maintained is also required within my role. Referring of students or patients to specialised health resources or community agencies furnishing assistance, and also consulting with institutions or associations regarding issues and concerns relevant to the practice and profession of nursing. 5 Personal Statement To record my CPD I use an online CPD development website - CPDme.com - I use this on at least a weekly basis to record my activities and reflective notes on my actions. Using the online logging system and downloadable evidence templates, I record all my activities to ensure I have an up-to-date record of my continuing development as a professional. Because I am registered with the HCPC, one of my standards is to keep an accurate record of my professional development. As such, I have found that CPDme is not only simple and easy to use, but it also allows me to take part in with structured and unstructured learning that is then easy to document and capture the evidence of. I often use the mobile app to make quick noted of learning from clinical bulletins supplied from my employer. This assists me in maintaining an accurate record of my learning, but also demonstrates to my employer that I have read the bulletin and interpreted this into a positive change in my clinical practice. This webinar identified techniques and risks of extracting patients from a variety of different situations from cars to homes. What I did learn from this 1 hour webinar was that each situation is dynamic and can change very quickly, even during the movement phase. Information that I learned from this webinar no doubt will benefit my service users from serious road traffic collisions to the elderly person trapped in the bathroom. I identified that there were some clinical presentations that I was not aware of or could identify at my skills level. Attending this three day course has helped me build upon my core clinical examination skills and also has allowed me to identify some clinical presentations that I wasn't previously familiar with. The course has also directed me to take up a full HE level 7 module and use this as part of a formal degree. I believe the skills and knowledge directly learned from this course will directly benefit my patients and service users and allow me to support them in making the right clinical examination leading to the correct signposting of my patients to the most appropriate care. 14 CPD Development Diary Entry Title of CPD activity Worked in a Paediatric Unit - Spotting the ill child This activity was arranged: Myself as part of my job role The category this activity relates to is Working in other departments This development activity has benefited me because I recently had a placement in my local children's hospital completing a short module on spotting the sick child and managing them in a pre hospital situation. As part of this placement I learned the differences anatomically between children and adults and also looked as specific illnesses and presentations that are unique to children. I also learned to identify the peri arrest child when dealing with conditions such as sepsis and meningitis. This course was essential in my practice and really has improved my ability to spot the sick child. I have since found a great resource that I would suggest to my colleagues called Spotting the Sick Child. This was developed by a team of clinicians and academics and follows the following acronym: Communications: Response: Actions: Subséquent Actions: Hospital: Evaluation and Ethics: Discussion: As a pre hospital practicioner, following this model helps me structure and extract vital and reflective components from incidence assigned to me that I could learn from. I now often use this new model to support my colleagues and peers when they are requested to reflect upon something that has occurred in their own practice. The trauma cell and what it does This activity was arranged: Myself in my own time The category this activity relates to is CPD Event This development activity has benefited me because I watched this great webinar which explained in details the role of the trauma cell. This free webinar allowed me to have a greater understanding of the trauma cell and also how this unit supports clinicians under pressure while trying to identify major trauma in casualties. What I learned from the webinar is that it's sometimes very difficult to make a decision alone and contacting the Emergency Operations Center (EOC) and discussing the incident with an Advanced Paramedic is a professional and more reliable approach than making a sole decision. Attending high calibre incidents and inducing stress can sometimes cloud decision making, thus ringing EOC can aid in decision making and ensure that my patient gets taken to the most appropriate care destination, which may not be my nearest Emergency Department. I mentored a student paramedic undertaking his Dip in Paramedic Practice. Along with my colleague, we supported the student in all aspects of Paramedic practice in both a clinical setting and also supporting his theory and knowledge around presenting patients. This two year mentoring phase also helped me keep my knowledge unto date as the student was very inquisitive and on occasions asked me questions that I indeed did not know the answer to. This then directing my self study to identify the correct answers. I identified from some research that individuals took part in a diversity of learning and development but did not take time to document this and also accurately record the learning that had taken place. This presentation was the first I used to demonstrate the issues that I had identified. Learning and development alone had identified that there was a clear lack of reflection. In simple terms, what had you done, why and what did you learn that could translate to better patient care, improvement in your own practice, or a more holistic package for the service user. CPDme was a clinical study / trial to identify the user base and also look at some of the trends around PD, L&D and other methods of professional development. The results were the birth of CPDme and the supporting of staff from all vocations to accurately record, reflect and keep a succinct record of their continuing professional development. 24 How I have met the required HCPC Standards Meeting HCPC Standard 1 I have my Trust supplied CPD/PDP folders that I have used to record my CPD activities - I have a file that contains all my certificates gained in my professional life, and that is specific to my nursing career with regard to activities undertaken, courses, seminars, in house training etc. I have recently joined CPDme.com where I have uploaded my evidences and created a more manageable and concise portfolio. CPDme.com makes managing my evidence easy and structured. They provide online forms that I can easily complete and submit, and also upload attachments such as certificates or authorised witnessed skills log forms which are also provided within their site. Using this website also provides me with pointers, news and study articles, links, videos etc which I can use for my study and discussions, and then use as evidence for inclusion in my portfolio. Having this membership means that I no longer feel alone and lost. I have plenty of advice, pointers and support to help me develop further. Meeting HCPC Standard 2 I am involved in a field where I am encouraged to both attend courses set by my employer and also become involved in inter departmental experience, student mentorship, and also undertake independent learning and study. I already have attended a mixture of seminars, lectures and a series of compulsory skill update sessions, but now also include self study and research as part of my development, as well as peer discussion and debates, as these events also have the requirement for me to read, absorb, and then reflect and debate on the content, and look at how the information gathered can help me in my role, and how it will subsequently help improve the service I provide to my patients. This is part of everyday life, reading articles and journals of interest, watching certain fact based programmes of interest etc, but not necessarily something I would have previously considered as part of my development. Now, I believe these things are equally as important as attending a traditional lecture. Meeting HCPC Standard 3 My CPD has benefitted both myself as a nursing professional, work colleagues and students as a peer and also the patients that I care for. My knowledge and skills have improved greatly since I started to record and reflect more on my prior and current training and education. I have chosen the examples below to demonstrate how my CPD has enhanced the quality of service I deliver to the public

25 How I have met the required HCPC Standards Example 1 University Lecture; Anatomy I have recently attended a university based lecture on Anatomy, that covered a variety of elements that I found incredibly useful for my future examinations and observations, as well as refreshing my existing knowledge. I can now make more concise and descriptive notes for my colleagues, and I can also envisage the internal working behind injuries when assessing patients, and also explain things to them a little more descriptively. This knowledge has given me more confidence in my service also, both with the patient, and with my superiors. Example 2 Mentorship Course I attended a mentor/supervisor course, which gave me insight on how to approach student learning, how to supervise tasks properly, learnt about my own learning techniques, and subsequently, how to help enhance and guide others learning. Example 3 Self Directed Study I have also researched various areas for my involvement, such as researching the regional variance of diet, and the poor diet and associated health issues in our immediate area. This helps me to conduct more in-depth patient interviews, and give more targeted advice and also handout more relevant information, support group data etc to my patients, thus improving the service myself and the hospital provide, and also the service received by the patients and their families. These 3 examples are also shown in my CPD evidence included in this portfolio. I also use reflection as an integral part of my everyday work, and some of these reflections are also included in my development log. 26 Knowledge and Skills Framework 1: Communication Communication has many forms and is a two-way process. It involves identifying what others are communicating as well as communicating yourself and the development of effective relationships. 2: Personal and People Development This dimension is about developing yourself and contributing to the development of others through both formal structured and informal ad-hoc methods. 3: Health Safety and Security This dimension focuses on maintaining the health, safety and security of everyone in the organisation and anyone who comes into contact with it. It includes tasks that are undertaken as a routine part of work, such as moving or handling. 4: Service Improvement This dimension is about improving services in the interests of the users of those services and the public as a whole. The services might be services for the public (patients, clients and carers) or be services that support the smooth running of the organisation (such as finance, estates).
