2 Medi-Cal Managed Care Introduction The Community Resource Guide was developed by the Cultural and Linguistics department to assist health care professionals in finding health care resources available to Medi-Cal Managed Care (Medi-Cal) members. This guide will help you identify resources, some at no cost, for members to improve and maintain their health. How to use this guide Each service is grouped under 11 main categories in alphabetical order. After the quick reference list, the directory offers service descriptions in alphabetical order. 5 Page 4 of 10 Directory of cultural and linguistic resources Adult Day Health Care of Fresno and Clovis counties Adult Day Health Care offers occupational and speech therapy as well as dietary and nursing services. A registered nurse is present daily to evaluate and provide medical and nursing care. Social work services are available to help participants locate needed services, and counseling is provided to participants and their families. Serves: individuals 18 years of age and older with a physical or mental impairment Hours: Monday through Friday from 8 a.m. to 2 p.m. Agency on Aging Family Caregiver Support Program The Agency on Aging offers services through their Family Caregiver Support Program. This program provides caregivers with practical tools to alleviate stress and avoid burnout, networking opportunities to decrease isolation and share concerns, and stopgap measures to stabilize the provision of care during times of extreme difficulty. Services include support groups, classes and case management. Serves: Fresno and Madera counties; accepts Medi-Cal members Hours: Monday through Friday from 8 a.m. to 4:30 p.m. Alcoholics Anonymous (AA) AA offers a self-help alcoholism recovery program utilizing the 12-step approach. Classes are offered in English and Spanish at no cost individuals/families. Note, the center is only open for meetings. Hours: Monday through Friday from 10:30 a.m. to 4:15 p.m. and Saturday from 9 a.m. to 1 p.m. Asian Pacific Islander Clinic At the Asian Pacific Islander Clinic, case management teams provide comprehensive as well as culturally and linguistically appropriate outpatient and community-based specialty mental health services to transition individuals who meet medical necessity. Services are provided using a wellness and recovery model and include assessments, clinical and rehabilitative services, case management, peer support, and medication assistance. Hours: Monday through Friday from 8 a.m. to 4 p.m.; walk-ins are accepted Big Brothers Big Sisters of Central California Big Brothers Big Sisters of Central California focuses on youth who are filled with promise but whose life circumstances make it hard to achieve success. They offer children facing adversity with a volunteer mentor one who is strong and enduring as well as professionally supported for 1:1 relationships. They also provide recreational activities like sports. Hours: Monday through Friday from 8:30 a.m. to 5:30 p.m.
6 Page 5 of 10 California MENTOR Family Home Agency (FHA) The mission of California MENTOR is to offer adults innovative, quality services and supports that lead to growth and independence regardless of the physical, intellectual or behavioral challenges they face. In an effort to help people shape the direction of their own lives in community-based settings, their philosophy emphasizes partnerships with those they serve as well as their families, employees, mentors, payers and the communities in which they work. Hours: Monday through Friday from 9 a.m. to 5 p.m. California Rural Legal Assistance Foundation The California Rural Legal Assistance Foundation offers immigration services to marginalized immigrant communities. Services include providing outreach and information on the naturalization process as well as extensive community education, advocacy, training, and legal and technical assistance. Serves: Rural California residents Fees: Services are offered at no cost. Hours: Monday through Friday from 9 a.m. to 5 p.m.; appointments are required for assistance regarding the naturalization process. Central California Legal Services (CCLS) The CCLS provides a range of services from advice to court representation for victims of abuse. They assist with naturalization, legal permanent resident card renewals and U-Visa/VAWA applications. CCLS employs attorneys, paralegals and support staff to provide civil legal services at no cost. Other programs offered by CCLS include health services (advice and representation for those having difficulty accessing health care services) and housing assistance (representation for administrative hearings on Section 8 voucher terminations). Fees: Services are offered at no cost. Hours: Monday through Friday from 8:30 a.m. to 4:30 p.m. Central Valley Regional Center (CVRC) The CVRC offers advocacy, evaluation, assessment, information and referral, and coordination of services to developmentally disabled children and adults. Serves: Fresno, Merced, Tulare/Kings, Madera and Mariposa counties Centro La Familia Advocacy Services Family Support Center The Centro La Familia Advocacy Services Family Support Center provides a range of low-cost immigration and naturalization services. They offer help through consultations and assistance with family petitions, green card renewals and citizenship through naturalization. Serves: Remote areas of Fresno County Hours: Monday through Friday from 8:30 a.m. to 4 p.m.
7 Page 6 of 10 Clinica Sierra Vista Garland Health Center The Clinica Sierra Vista Garland Health Center offers conventional HIV blood testing as well as chlamydia, syphilis, gonorrhea and herpes testing. Hours: Monday through Friday from 8 a.m. to 5 p.m. Community Behavioral Health Center The Community Behavioral Health Center is a 61-bed facility that provides adult inpatient psychiatric care and 24-hour inpatient hospitalization services to children, youth and adults. Their mission is to provide a multi-disciplinary approach while ensuring optimal mental health for patients (as well as their families and community) through education and treatment. Experienced staff provide voluntary and involuntary psychiatric assessments, adult care, recreation therapy, group and individual therapy, medication management, milieu management, and other treatment modalities. Community Link Fresno Rainbow Pride Community Link offers services to the LGBTQ community including: The Fresno Youth Alliance: a supportive and social group for younger members The Gray Alliance: a social, support and advocacy group for older members News Link: the Central Valley's no cost monthly gay newspaper Pink Pages: the Central Valley's only gay, lesbian, and bisexual resource and business directory The Fresno Rainbow Pride: an annual LGBTQ pride parade and festival It's a Queer Thang: a monthly radio show with news, entertainment and music from the LGBTQ community Bowling and volleyball: less-than-competitive volleyball games (March to October) and two bowling leagues (October to March and May to September) Serves: Central Valley residents Comprehensive Alcohol Program (CAP) residential The CAP offers substance abuse treatment services. The types of care offered include residential short-term treatment (30 days or less) and residential long-term treatment (more than 30 days). with co-occurring mental and substance abuse disorders Fees: Services are offered on a sliding fee scale where income and other factors are considered. Hours: Monday through Friday from 8:30 a.m. to 5 p.m.
8 Page 7 of 10 Decision Home of Fresno Decision Home of Fresno is a starting point for men who are or have experienced substance abuse problems. Family-oriented activities such as group and peer discussions involving the 12-steps and time with their families are offered. Hours: Office Monday through Sunday from 8 a.m. to 5 p.m. Center 24/7 Encourage Tomorrow Encourage Tomorrow provides after-school and lunch-time youth development programs including cultural awareness lessons, service-based learning, career development, college preparation, self-esteem boosting activities, health and nutrition education, life skills and mentoring cultivation, and job skills and shadowing opportunities. In addition, tutorial services are offered in home or at their main center. Hours: By appointment only Eoc Health Services Clinic Community Health Center The Eoc Health Services Clinic Community Health Center provides education, counseling and treatment services to men, women and teens who are at-risk for unintended pregnancy and sexually transmitted infections (STI). They offer confidential and comprehensive medical services to the low-income, uninsured and underinsured through services like breast and cervical screening for women and testicular screening for men, physical exams, and HIV/AIDS and STI testing. Hours: Monday through Friday from 8 a.m. to noon and 1 p.m. to 5 p.m. Evangel Home, Inc. Evangel Home, Inc. offers temporary emergency shelter as well as meals, clothing, counseling and group support to single women with children who have been victims of domestic violence. Hours: Center 24/7 Assessments Monday through Friday from 9 a.m. to 5 p.m. Family Healing Center The Family Healing Center offers services to victims of child abuse, domestic violence, sexual assault and family violence. Hours: Monday through Friday from 9 a.m. to 5 p.m.
9 Page 8 of 10 Fresno Barrios Unidos Fresno Barrios Unidos is an organization that transforms communities by empowering youth and families through advocacy, education and wellness. Programs offered include: El Joven Noble ( The Noble Youth ): a youth development and support program Teen Success: a program that empowers teen mothers Fresno County Department of Public Health Community Health Division The Fresno County Department of Public Health Community Health Division offers standard hepatitis C and HIV testing, but they also offer rapid oral and blood HIV testing. Additionally, hepatitis prevention/education, HIV test counseling, HIV/AIDS prevention education, partner notification, and treatment information is provided. Hours: HIV testing Monday through Thursday from 8:30 a.m. to 11:30 a.m. Fresno LGBT Community Center The Fresno LGBT Community Center is open to the public. They offer access to information and local and national LGBTQ resources including local publications such as News Link and the CCA Newsletter. They offer reference materials relating to the LGBTQ civil rights movement as well as printouts of recent court decisions affecting the LGBTQ community. Hours: Tuesday through Saturday from noon to 5 p.m. Fresno Rescue Mission The Fresno Rescue Mission offers an emergency family shelter that provides 14 to 30 days of temporary shelter to families as well as food, clothing and personal care; referral to other agencies that can assist them; and love/hope through the teachings of Jesus Christ. To assist them in starting over in an apartment, families are given food packages and other needed supplies when they leave the shelter. Hours: Monday through Friday from 8 a.m. to 5 p.m.
10 Page 9 of 10 Gay and Lesbian Center Bakersfield The Gay and Lesbian Center seeks to improve the wellbeing of the LGBTQ community with cultural and educational programs and an extensive referral network. They offer community building socials and other activities like picnics, dinner outings and camping trips. They also offer: About Kern: a program that brings together lesbian, gay, bisexual, transgender and ally professionals from Bakersfield and Kern counties together to celebrate and promote equality in the workplace 2030+Q-: a peer support and networking group for individuals under 40 years of age who identify as LGBTQ or straight allies Serves: Bakersfield and Kern counties Fees: Services are offered at no cost. Hours: Tuesday through Thursday from 5 p.m. to 7 p.m., Friday from 4 p.m. to 7 p.m. and Saturday from 3 p.m. to 6 p.m. Heritage Adult Day Health Care Heritage Adult Day Health Care offers therapeutic services to seniors. Transportation to and from the facility is offered at no cost, and breakfast and lunch are provided. Patients typically stay at the center for five to six hours. Nurses are on staff to monitor each patient s health, social workers provide 1:1 sessions, and nutritionists provide consultation. Every week, patients are taken on field trips to various stores to purchase hygiene products. ; accepts Medi-Cal members Hours: 7:30 a.m. to 4:30 p.m. Kings View s Blue Sky Wellness Center The Kings View s Blue Sky Wellness Center offers mental health services to adults by focusing on key mental health recovery principles including dispensing hope, having freedom of choice, modeling self-determination and personal responsibility, and emphasizing that consumers can and do heal from a mental health diagnosis. They offer self-help and support groups as well as a variety of activities such as computer access, library time, a weekly clothes closet and a gym for personal use. Hours: Monday through Saturday from 8:30 a.m. to 4:30 p.m. Madera Rescue Mission The Madera Rescue Mission is committed to the needy, destitute and homeless. They not only meet temporary needs but also offer long-term help and solutions to those who truly desire a change in their life. Serves: Madera County residents Hours: Monday through Friday from 8 a.m. to 5 p.m.
11 Page 10 of 10 Marjaree Mason Center The Marjaree Mason Center offers individual and group therapy for adults and children affected by domestic violence. They provide emergency and long-term safe housing, legal and financial assistance services, support services, and children's services to victims of domestic violence as well as community education. 24-Hour Crisis Hotline: Hours: Monday through Friday from 8:30 a.m. to 5 p.m. Noami s House Noami s House provides overnight shelter to homeless women 365 days a year. They also offer services at no cost through the combined resources of the Collaborative including hot meals, medical and dental care, a day shelter, laundry and clothing assistance, case management, storage, transportation, health and personal living education, and information and referrals. Hours: Assessments Monday through Friday at 3:30 p.m. and Saturday and Sunday at 12:45 p.m. or 5:45 p.m. The ARC Fresno and Madera counties The ARC offers programs to empower individuals with developmental disabilities to attain greater independence. They offer programs including adult day programs, community integration, community living, supported living, community employment and advocacy. Serves: Fresno and Madera counties Hours: Monday through Friday from 8 a.m. to 4:30 p.m. Valley Adult Day Health Care Center The Valley Adult Day Health Care Center is a health care facility providing services to seniors and adults with disabilities and/or medical/mental health conditions or impairments. ; accepts Medi-Cal members Hours: Monday through Friday from 8 a.m. to 4 p.m. Valley Caregiver Resource Center The Valley Caregiver Resource Center offers caregiver services including support groups, education workshops, respite care, legal consultation and emotional support. Hours: Monday through Friday from 8 a.m. to 5 p.m.
