Ask for the rate code for CLSA18 Rates are $ % tax for attendees Session 1 Wednesday 8:30-10:00 AM Diagnosis and Follow-up of Multiple Myeloma and Related Disorders: The role of the laboratory Patients with multiple myeloma (MM) and related plasma cell disorders often present with a myriad of non-specific symptoms, making diagnosis challenging. Traditional screening methods are insensitive, missing a significant percentage of these patients. The laboratory can play a key role in helping the clinician obtain a more accurate diagnosis with appropriate follow-up by adopting updated MM screening protocols recommended by the International Myeloma Working Group (IMWG) & National Comprehensive Cancer Network (NCCN). Compare traditional myeloma testing methods with current recommended protocols and describe how kappa and lambda free light chain analysis play a key role. Discuss the updated criteria for diagnosis of Multiple Myeloma and new recommendations for inclusion in routine practice. Explain why it is necessary to monitor both intact immunoglobulin paraprotein and serum free light chains in multiple myeloma. Dr. Ann L. Updates to this program will be available online at Session 2 Wednesday 8:30 10:00 AM Dealing with Preanalytic Error Preanalytic error is a common problem for all laboratories with potential impact to the healthcare facility as a whole and, more importantly, the patient. There are several things to consider in recognizing and resolving these errors. This discussion will focus on the most common preanalytic errors that plague laboratories including hemolysis, pseudohyperkalemia and short draws as well as offer suggestions on troubleshooting and finding solutions for these laboratory challenges. Describe the troubleshooting process for preanalytic errors that occur in the laboratory path workflow. Discuss the most common errors including hemolysis, pseudohyperkalemia and short draws (quantity not sufficient) and how to resolve them. Identify factors that contribute to preanalytic error, e.g. Decentralized phlebotomy, IV and syringe draws, training and how to minimize the impact. Or is it something more sinister? Different specimen issues will be presented in reference to how they affect clinical chemistry results. Through discussion of examples and case studies technicians and scientists will (re) learn to recognize when chemistry results have gone bad. Name at least four specimen issues that contribute to inaccurate results. Explain the difference between contamination, IV contamination and diluted by fluid (s). Recognize when common chemistry results are inaccurate. There will also be examples of common PT failures and corresponding corrective actions. Identify the role of proficiency testing in the laboratory accreditation process. List different grading processes and identify the information provided on an evaluation report. Identify potential consequences of PT failures and the appropriate corrective actions to address them. Amanda Weiss, MT ASCP Program Coordinator at WSLH Proficiency Testing Session 5 Wednesday 2:00-3:30 PM Latent TB Screening with New Tools: IGRA Use and Results Interpretation. Latent tuberculosis infection (LTBI) is a condition in which a person is infected with Mycobacterium tuberculosis but does not currently have active tuberculosis disease. An estimated 10 to 15 million persons in the United States have LTBI. Because 5 to 10 percent of persons with LTBI are at risk of progressing to active disease, identification and treatment of LTBI are essential for the elimination of tuberculosis. Describe currently available tests for latent tuberculosis infection (LTBI), focusing on the newer, blood-based Interferon Gamma Release Assays (IGRA s) Explain the benefits and limitations of LTBI screening with both either TST or an IGRA and what are the current guidelines in utilization of either strategy Interpret results when screening special populations: low-risk healthcare workers as well as high-risk immunocompromised patients or suspects in a TB exposure Identify the availability and pricing expectations for these types of tests in Alaska. Session 8 Thursday 8:30-10:00 AM Cost-Effective Utilization of Urine Drug Testing for Chronic Pain Management Patients The costs associated with traditional urine drug testing models for pain management can be excessive due to inappropriate testing performed, such as reflex confirmation testing for all positive screen results that are not clinically useful. This presentation will describe cost effective strategies that eliminate unnecessary testing and inappropriate interpretations for pain management patients. Compare traditional workplace drug testing model with chronic pain management monitoring. Describe factors affecting costs associated with chronic pain management patient oversight. The presentation will focus on what the changes are relative to the previous edition and what impact these changes may have on day to day specimen collection. Identify important changes to the CLSI Standard for venous blood collection. Understand the path of workflow for venipuncture. Discuss how revisions impact venous blood collection. It should promote appropriate, timely diagnostic testing, including specimen collection, and pathogen identification and accurate, timely reporting of results to guide patient treatment. Laboratory involvement in stewardship optimizes antimicrobial use and reduces resistance. List lab and microbiology tests that contribute to optimizing stewardship. Discuss the general principles of diagnostic stewardship as they relate to preanalytic, analytic and postanalytic activities. Consideration of the roles in a multi-disciplinary C. difficle program including environmental services, nursing, laboratory, infection prevention, clinicians, education providers, pharmacy, and administration.
