6 Experience Providing Similar Services Healthy Start of North Central Florida has been subcontracting with providers throughout North Central Florida to provide home visiting and care coordination for more than 25 years

9 Experience Providing Similar Services (continued) Florida School of Traditional Midwifery-Licensed Midwives provide in home prenatal care, birth and postpartum services to women annually.

10 Population to be served Alachua County residents that deliver in Alachua County In 2016 there were 2,753 Alachua County resident births in the County. UF-Health Shands: 1,158 births NFRMC: 1,523 Birth Center: 20 Home births: 52 All women invited to participate (approximately 2,700) Year 1 Goal: 70% will consent to participate and 60% will complete the home visit

11 Program Approach Expectant mothers informed of NHVP through OB/GYN & Licensed Midwives Childbirth classes & hospital tours Countywide marketing campaign Family Partner will meet with each Alachua mother prior to discharge and get consent to participate in NHVP RN will visit mom and baby at home within 7 days of discharge Licensed midwives currently provide a postpartum visit and will add NHVP assessments & education to current visit

12 Program Approach (continued) Family Partner will: Meet with all Alachua mothers who deliver at UF-Health Shands and NFRMC and develop rapport Educate mom/family about benefits of NHVP; estimated length of visit educational information that will provided assessments that will be conducted incentives available information on short term case management available (if needed) Obtains signed consent to participate in program. Schedules date and time for home visit Provide nurses business card, which includes her picture

13 Program Approach (continued) For infants in NICU: Family Partner will meet with family at the hospital within 3 days of baby s birth Schedule a call with the RN within 3-5 days after that meeting RN will conduct a home visit within 7 days after baby is discharged from NICU

14 Program Approach (continued) RN/Licensed Midwife completes the following: Postpartum and newborn assessment Includes mother s and baby s physical, mental and social health history and service needs Education and referrals on postpartum and infant care Understanding & knowledge of self-care, infant health, immunizations, infant sleep position, infant dietary intake, home/fire/water safety, follow-up appointments

15 Program Approach (continued) If family needs additional services or resources they can be connected to the following through short-term case management by the Family Partner and she will follow-up after 14 days to ensure linkages to services: Parenting education Breastfeeding support & education Nutritional support & information Child development education Child Care Assistance with basic needs Other assistance or resources as needed

18 Collaborators and Partners Healthy Start of North Central Florida: Lead contractor Subcontract with hospitals and Florida School of Traditional Midwifery Employ the Family Partners North Florida Medical Regional Center: Hire, train and supervise the RN s who will visit mothers who deliver at NFMRC UF-Health Shands: Work with the Family Partner who will recruit program participants at Shands UF-Health Shands HomeCare Center: Hire, train and supervise the RN s who will visit mothers who deliver at UF-Health Shands Florida School of Traditional Midwifery: Train and supervise the licensed midwives to add NHVP information and assessments to their postpartum visits Subcontract with HSNCF so midwives can receive a stipend for participating in the NHVP Licensed Midwives: Conduct home visits with their patients
