Formulates 2 nursing diagnoses which are actual or high risk, based on client data, and reflect the long term care involved with chronic illness. Each statement uses the PES format. (12) b. Prioritizes the nursing diagnoses and identifies rationale for priority.(3) a. Goals are consistent with the problem, one for each diagnosis (3) b. Outcome criteria are related to the goal, written in realistic, behavioral terms and include specific, measurable outcomes. Includes long term criteria applicable to the pediatric client and family with chronic illness. (17) a. Develops a minimum of 6-8 interventions which are realistic, comprehensive, and individualized. Each order includes an action verb, what, when, how, how much and frequency. Scientific rationale and references are included. (25) Evaluation refers to goal using the outcome criteria. If criteria are in progress, determines the likely reassessment that will occur at home or at a future visit (both positive and negative reassessment predictions). Indicate a specific rationale for each priority utilizing Maslows. 3. Assign a number to each priority. Example - High Priority life threatening according to Medium Priority: Explain why it is a medium priority Low: Give the rationale for assigning this low priority. Will include both the general goal and the specific expected outcomes. 1. CLIENT GOAL a. Reword or paraphrase the Problem. b. The goal must be written as a positive statement. c. The goal is a general, broad nonmeasurable statement. Example: The client will: as evidenced by: (AEB). Write the E.O. immediately after the AEB. 2. EXPECTED OUTCOMES - a. Remember Expected outcomes measure the goal attainment they are not interventions. b. Write at least three E.O. that are specific, measurable, individualized and realistic. (Note: RUMBA) R = Realistic U = Understandable M = Measurable B = Behavioral A = Attainable PLANNING AND IMPLEMENTATION Nursing Orders Rationale for Nursing Orders 1. The column will begin with the words The nurse will written at the top. 2. However, a specific individual may be identified. 3. Each order will contain: What--action verb (activity) How--conditions How much--when necessary Where-- When--identified by time or activity How often--identified by frequency (e.g. Q.3 hours starting at 0900) or by activity (e.g. TID after each meal) 4. Always include the independent actions of the nurse when utilizing doctors orders in the nursing care plan. 5. The column will end with the student s signature, date and time. The date is not necessary after each order. 1. To develop this area a reference or reference text is necessary 2. Sources must be cited. 3. Rationale should indicate specifically how and why a particular Nursing Order will assist in goal achievement. 4. Should be based on biological, psychological, sociological, cultural and communicating principles. (BPSCC) 7. NANDA nursing problems are preferred, but others can be used with valid data. c. Expected outcomes that are long-term may have a specific time and date. It is not necessary to write the date after each E.O. d. Date and time at bottom of column for overall goal.
