They are based at The East Riding Community Hospital and can be contacted on Health Visitor A health visitor covers the practice and is based at Manor Road. The contact number is We are a training practice for student doctors. HOW TO REGISTER Please bring some form of identification ie passport, driving licence or home office ID and proof of current address. You will be asked to complete a registration form. PRACTICE BOUNDARY The practice will take new patient registrations from: Beverley, Walkington, Bishop Burton, Cherry Burton, Etton, Lockington, Lund, South Dalton, Holme on the Wolds, Beswick, Kilnwick, Watton, Leconfield, Arram, Aike Tickton, Weel and Woodmansey. Patients who are already registered and live in one of the above places may remain registered with the practice if they move out to one of the following: Dunswell, Thearne, Hutton Cranswick, Middleton-on-the-Wolds and Routh. APPOINTMENTS A range of appointments are available Monday to Friday between 8.30am and 5.30pm, later on Wednesdays. These can be made by telephoning us on Phone lines are open from 8.00am, although due to call volume early in the day, we recommend you call later for non-urgent requests and queries. There is an evening surgery on Wednesday. You can also book in person at the surgery and now have an online option via our website. We have appointments you can book up to a month ahead and others for urgent issues available each day by telephone from 8.00am. Appointments can be made with your preferred GP if available. The duration of a GP appointment is 10 minutes, please book a separate appointment for each patient. If you are unsure about the best option to meet your requirements or would like to try online booking, please ask the reception team for advice or more information. The building is open from 8.00am until 6.00pm and to 8.00pm on Wedensdays. Emergencies If you are confronted by a serious problem such as severe chest pain or severe bleeding, call an ambulance (tel: 999) before calling the surgery. General Queries The NHS 111 service (telephone 111) is available for non-urgent advice including when the surgery is closed. Useful website: NHS Choices HOME VISITS If you are physically unable to attend the surgery please telephone before 10.00am if a visit is required that day. Urgent visits only will be arranged after that time. Please give the receptionist as much information as possible to enable the doctor to allocate priority. It is usually after 12 noon before the doctor will be free to leave the surgery. WEEKEND AND NIGHT COVER Between 6.00pm and 8.00am the next day and at weekends, if you require medical assistance which cannot wait until the surgery re-opens, please dial 111. Calls to the NHS 111 services are free from both landlines and mobiles. If you have a life threatening medical emergency please dial 999. REPEAT PRESCRIPTIONS Please give 48 hours' notice when ordering your prescription. Simply call SERVICES Antenatal Clinic Patients are seen by the midwife at East Riding Hospital, Beverley. Anti-Coagulation Monitoring We provide a monitoring service in the practice for those patients taking Warfarin. Asthma/Respiratory Care Asthma and COPD reviews are carried out by our practice nurse Debra Rewston. Cervical Screening Our practice nurses provide this service. Diabetic Clinic Our practice nurse Wendy Charles leads on regular checks required for diabetic patients. Family Planning Contraceptive care is provided by all the doctors during surgery hours. We also offer contraceptive implants and coil fitting as appropriate. Please call reception to make an appointment with a doctor to discuss. Flu Vaccination An influenza vaccine is recommended for patients aged over 65 and those with long term conditions such as diabetes, asthma, heart disease, kidney disease or who are residents of care homes. Some children outside risk groups are now eligible. We start clinics in September each year. Please contact reception to make an appointment if you are in one of the eligible. If you are housebound, please let us know and we will make suitable arrangements. If you are unsure whether you are eligible, please enquire at reception and we will check for you. General Health Checks We offer three yearly health checks for patients aged and annual checks for those over 75 on request. Long Term Conditions If you have two or more long term conditions such as diabetes or COPD, we offer a long term conditions clinic. You will be asked to have a blood test and complete a questionnaire You will then discuss a plan to help manage your condition. Minor Surgery The GP team can provide some minor surgery services such as certain joint injections, removal of toe nails and excision of lumps. Non-NHS Work Some services are not available on the NHS such as forms for insurance, employment, travel, licenses, private certificates or reports. Please telephone the surgery for further information and advice on changes. Other Vaccinations We also offer other vaccinations according to national requirements such as shingles and pneumonia as well as the standard recommendations for children.

4 Travel Advice/Immunisations If you plan to travel abroad, please ask at reception for a form. This will give us details of your plans so that we can advise of any immunisations you many need. This should be done with as much notice as possible to ensure that you have effective vaccine cover before you leave. Please note, some vaccinations for travel are chargeable, please ask at reception for prices. DISABLED ACCESS All patient services are provided at ground floor level with full disabled access. A wheelchair is available on request. COMPLAINTS, COMMENTS AND SUGGESTIONS Your first contact will normally be with one of our reception staff who should be able to help with most everyday questions and problems. Sally Fulker is our practice manager. Please ask for her if you have any special problems or worries, or if you have a comment about our service or a suggestion for improving the way we do things. While we always try to provide the best services possible, there may be times when you feel this has not happened. We do try to discuss and resolve any issues as they occur. However we also have a formal practice complaints procedure which is available from reception. Please note the complaints procedure is not able to deal with issues of legal liability or compensation. Due to our duty of confidentiality, if you are complaining on behalf of someone else, their consent may be necessary. If you wish to make a complaint or have any questions about the process please write to, or speak to, our practice manager. You may find it also useful to use the help available from the local Independent Complaints Advocacy service on CONFIDENTIALITY We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the team. Information will not be divulged to a third party without prior consent. All patients have a right of access to their own health records. For further information on how we use your health records please ask for a leaflet or contact the practice manager. CHAPERONE If you feel you would like to use a chaperone during an examination or consultation by one of our clinicians, please ask at reception at the time of booking or when arriving for your appointment. You may also ask during the consultation with a GP or other clinician. Our clinicians may also request the use of a chaperone. PEOPLE LOOKING AFTER OTHERS We aim to provide information for anyone who regularly looks after someone who cannot manage without help. Please ask at reception. FREEDOM OF INFORMATION - PUBLICATION SCHEME The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the classes of information the practice intends to routinely make available. YOUR RESPONSIBILITIES Courtesy Our team aims to provide the best possible service to each patient We welcome your comments and suggestions about the service we provide. In return we ask that you treat our staff with courtesy at all times. Verbal abuse of any kind or violent, intimidating conduct will not be tolerated and may mean that you will be asked to seek GP services elsewhere. This may include attending the practice contracted to deal with these circumstances by the CCG; The Quays, Myton Street, Hull. Appointments At times we can become very busy. In aiming to give each patient a quality service, we ask for your understanding in return that sometimes delays in being seen may be for reasons beyond our control. We ask that you please let us know in plenty of time if you are not able to make your appointment. Missed appointments make it more difficult to provide appointments for everyone who needs one. Similarly, if you are running late for your appointment let us know if possible, however we cannot always guarantee to see you. 5 MINOR AILMENTS SCHEME A Minor Ailments Scheme is in operation in local pharmacies. If you do not pay for prescriptions you can still be seen by a participating pharmacy rather than your GP. Please ask at reception or at your local pharmacy for details. USEFUL TELEPHONE NUMBERS The East Riding Community Hospital, Beverley Hull Hospitals Independent Complaints Advocacy Service NHS England (PO Box 16738, Redditch B97 9PT) Health Service Ombudsman Social Services SELF TREATMENT OF COMMON ILLNESSES AND ACCIDENTS Many common aches and pains can be simply treated at home without the need to consult a doctor. Back Pain Back pain causes 13 million working days to be lost in Britain each year. The spine supports the whole weight of the upper body so it is understandable that it sometimes goes wrong. Because of the complex nature of the spine it is advisable to consult your doctor if back pain persists for more than a few days. If, as is usual, the pain has been caused by abuse ie lifting too heavy weights etc, be sensible and take things easy. Take care to sit as upright as possible with a support for the small of the back. Take aspirin or paracetamol which will not only relieve the pain but will help to relieve inflammation. Your doctor may well prescribe stronger drugs, heat treatment, gentle exercise or some kind of supportive corset. Bed Sores Bed sores are far easier to prevent than cure. They are caused by prolonged pressure to certain parts of the body when lying in bed for long periods. They can be prevented by encouraging the patient to shift position as often as possible. Take care to smooth out creases in the bottom sheet to avoid irritation. If red marks appear at the pressure points such as heels, elbows, buttocks and hips, inform the doctor before they get worse. Sore Throats Four out of every five sore throats are caused by viruses and therefore antibiotics are useless. If your throat is sore but you are otherwise okay there is no need to see the doctor. Simply give children paracetamol syrup and fluids (aspirin should NOT be given to children under 16). For adults, gargling with soluble aspirin is the most effective remedy. Dissolve two aspirins in one inch of warm water in a glass. Take sips of the solution and gargle with each sip for as long as you can without swallowing. If you are very hot and unwell and can see white spots on your tonsils you may have a true tonsillitis and you should come and see us at the surgery. Burns Apply large quantities of cold water to the affected area as soon as possible and maintain this until the pain subsides. This may take as long as 15 minutes! If the skin is unbroken but blistered, apply a loose, dry dressing. If the burn is larger than four or five inches in diameter or if the skin is broken, consult your doctor as soon as possible. Colds Even in this day and age there is still no magic cure for the common cold. Go to bed, take plenty of drinks. If you have a headache or are feverish, take aspirin or paracetamol. Do not bother to take antibiotics as these will have no effect! Diarrhoea In adults, diarrhoea is usually caused by a viral infection and is therefore unable to be treated directly. The symptoms can usually be eased by the traditional kaolin and morphine mixture or by medicines containing codeine. Holiday diarrhoea is often due to bacteria. Again, kaolin and morphine can be taken. Consult your doctor if the symptoms persist for more than a few days. Diarrhoea in very young children and babies needs careful attention. Most babies have loose bowel action during their first six months due to their predominantly liquid diet. Sudden bouts of unusually watery diarrhoea should be treated by taking the baby off solids and feeding them a cooled solution of boiled water with a teaspoon of sugar and half a teaspoon of salt to the pint. If the symptoms persist for more than 24 hours, or are accompanied by vomiting or weakness, consult your doctor. Gastroenteritis Gastroenteritis describes a group of diseases affecting the stomach or part of the intestine. Symptoms are often diarrhoea, sickness and stomachache. Because the lining of the stomach is likely to be inflamed medicines are often immediately vomited up. Large quantities of water, orange juice, milk or thin soup should be taken to counter the effects of dehydration. Consult your doctor if symptoms persist for more than a day or, in the case of babies or young children, six hours. Stomachache Most attacks are not serious and are usually caused by indigestion or wind. A hot water bottle will often relieve the symptoms and, in the case of indigestion, a teaspoon of bicarbonate of soda in half a glass of water will help. If the pain lasts for longer than eight hours or increases in intensity you should consult your doctor.

6 Sprains Treat with a cold compress, containing ice if possible, for 15 to 30 minutes to reduce the swelling. Then apply, firmly, a crepe bandage and give the sprain plenty of rest until all discomfort has subsided. Further strain will inevitably lead to further swelling and a longer recovery period. Nosebleeds Sit in a chair, lean forward with your mouth open, and pinch your nose just below the bone for approximately 10 minutes, by which time the bleeding should have stopped. Avoid hot drinks or hot food for 24 hours. If symptoms persist, consult your doctor. Minor Cuts And Grazes Wash the wound thoroughly with water and a little soap. To stop bleeding apply a clean handkerchief or dressing firmly to the wound for about five minutes. Cover with a clean dry dressing. Sunburn Treat as for other burns with cold water to remove the heat. Calamine lotion will relieve the irritation whilst paracetamol will also help. Children are particularly susceptible to sunburn and great care should be taken to avoid overexposure to the harmful effects of the sun. German Measles (Rubella) The rash appears during the first day and usually covers the body, arms and legs in small pink patches about 2-4mm across and doesn t itch. No other symptoms are usually present apart from occasional aching joints. It is infectious from two days before the rash appears, until the rash disappears in about four or five days from that date. The only danger is to unborn babies and, therefore, it is important that all contacts are informed in order that anyone who may be pregnant can contact their doctor. Immunisation can prevent this disease. Measles The rash is blotchy and red and appears on the face and body around the fourth day of illness. It is at its most infectious from two or three days before the rash appears until eight or ten days after that date. Immunisation can prevent this disease. Mumps Symptoms are swelling of the gland in front of one ear often followed, after a couple of days, by swelling in front of the other ear. It is infectious from two or three days before the swelling appears until eight or ten days after that date. If the pain is severe you should consult your doctor. Immunisation can prevent this disease. Insect Bites And Stings Antihistamine tablets can be obtained from the chemist without prescription and will usually relieve most symptoms. Note: bee stings should be scraped away rather than plucked in order to avoid squeezing the contents of the venom sac into the wound. Head Lice These creatures, contrary to popular belief, prefer clean hair and are therefore not a sign of poor personal hygiene. Medicated head lotion can be obtained from the chemist without prescription. Chickenpox On the first day a rash appears as small red patches about 3-4mm across. Within a few hours of these developing, small blisters appear in the centre of these patches. During the next three or four days further patches will appear and the earlier ones will turn crusty and fall off. Calamine lotion may be applied to soothe the often severe itching. Cool baths may also help. The most infectious period is from two or three days before the rash appears and up to five days after this date. Children may return to school as soon as the last crusts have dropped off.
