1 9 9 HEALTH AND SAFETY In this quarter, we regretfully report three fatalities in Turkey. These tragic incidents overshadow what has otherwise been an exceptional performance in Health and Safety. Information about the investigations into these fatalities and the findings and key learnings is detailed in Section Health and Safety performance has to be viewed in relation to the scale of activity on the Project, particularly in respect of the major risk areas e.g. driving. During the quarter, over 21,000 people worked on BTC, recording 16.4 million man-hours and driving over 31.5 million kilometres. In 2004, a total of 61.7 million man-hours have been worked and million kilometres driven. This section provides more detail on H&S issues than previous quarterly reports, and summarises achievements and challenges for the Project in PERFORMANCE AND ACHIEVEMENTS In view of the aforementioned fatalities, it appears contradictory to talk about achievements for Health and Safety, but significant and real accomplishments have been made. BTC s annual Health and Safety performance is 5.6 times better on Days Away from Work Case Frequency (DAFWCF) when compared to the industry average (see Table 9.1). The purpose of setting targets is to prevent fatalities, and reduce DAFWCF, Recordable Injury/Illness Frequency rate, and Total Vehicle Accidents Rate. Conversely, targets have been set to increase the number of Advanced Safety Audits, STOP Cards and Safety Training hours, as these are positive contributors and tools used to improve safety performance. These trends have been monitored and action has been taken as required. 9-1

The Stand-Down required all sites to view a presentation on sideboom activities, and review operating procedures and equipment limits for all personnel involved in sideboom operations. Senior Site Managers were also asked to sign a declaration to verify that operations could be undertaken safely. Achievements and milestones for each country are described below AZERBAIJAN Key activities for the quarter were as follows: A joint BTC/SCP Health and Safety assurance schedule was drafted for 2005 to include subcontractor s activities; An annual CCIC / SPJV Health Promotion Audit was undertake; A first aid and Medical Provider audit was undertaken; and CCIC celebrated 3.6 million man-hours worked without a DAFWC. SPJV also achieved 1.7 million man-hours without a DAFWC. 9-2

3 9.1.3 GEORGIA In Georgia, the following activities were performed: The 5 Star Programme (to award good HSE performance) continues at Facilities and Pipeline Spreads I and II; A Transportation and Vehicle Management Audit by SPJV and BTC was completed; The Zero Tolerance driving safety campaign directed to all personnel working on the BTC Project continued; Continued delivery of Driving Immersions Part I, II and III to all BP employees and continuing Parts I and II to contractor drivers; and A winter driving campaign commenced TURKEY Health and Safety achievements for the quarter were as follows: Driver competence evaluation was completed on Lot C; A PHSER review was completed at all locations; Continuation of joint BTC / Botaş sponsored British Safety Council International Diploma training for Project HSE personnel; H&S Coaching workshop was carried out involving BTC, Botaş and contractors H&S personnel; and Tekfen achieved 8 million man-hours without a DAFWC whilst Botaş reached 1.63 million man-hours. 9.2 HEALTH ACHIEVEMENTS IN 2004 The provision of potable water in some Project locations was highlighted as an issue in the Q3 IEC audit. These findings were promptly addressed (see Appendix 2). Although covered to some extent in previous Project reports, this section aims to give a broad overview of the Project approach to health. It is therefore appropriate to refer to initiatives and achievements undertaken in the last year as well as those that took place in the final quarter of Health monitoring and food hygiene have been highly successful in 2004 with over 18 million meals being served across the Project with only 1 incident of food related illness reported in Turkey. First Aid was administered to the 14 workers complaining of stomach upsets and all returned to work. Following an investigation, changes were made to food preparation and hygiene procedures to ensure standards were adhered to. Typical Occupational Health and Hygiene (OHH) activities undertaken in each country during 2004 are given in the following sections GENERAL A Health Risk Assessment was undertaken in March 2004 in Baku, facilitated by the Azerbaijan Business Unit Health Team and involving representatives from all 3 countries. The purpose of this assessment was to identify activities to mitigate health risks and to develop individual In-Country Health plans for Throughout 2004, BTC and Contractor personnel across the Project underwent training in food safety, delivered by an independent Food Safety Specialist. 9-3

4 9.2.2 AZERBAIJAN Follow up to the Epidemiology Survey of Azerbaijan undertaken in 2003 carried out by independent body (Azerbaijan Republic Anti-Plague Station); BTC Occupational Health Risk Assessment; Annual OHH Review (including OHH Noise / Catering); Annual and Monthly First Aid and Medical Provision Audit; Annual and Quarterly Water Quality Review; Annual and Quarterly Food Hygiene Review; Assessment of Secondary Health Care Facilities along the ROW in Azerbaijan; Quarterly Fitness to work review; Continuous Medical Emergency Evacuation review, An HIV/AIDS Prevention Programme (HAPP) is being implemented by International Medical Corps, with the support of local NGO partners. A key objective of the HAPP is to increase awareness of HIV/AIDS and Sexually Transmitted Infection (STI) among Project personnel/ contractors and communities in proximity to worker camps. Throughout the programme full dialogue is maintained with the Ministry of Health, the Ministry of Education and other key authorities/non-governmental organisations. The programme includes: GEORGIA 9-4 Training for local health care workers from state clinics, teachers and other community care workers; Distribution of condoms; Development and distribution of HIV/AIDS/STI booklet for community members and others; Training on HIV/AIDS/STI prevention and Alcohol and Drug abuse for Project employees/contractors and community members; and Development of self-learning CD Rom on HIV/AIDS and STI s for distribution. Bi-annual Food Safety Audits undertaken by independent BP experts; Quarterly food inspections by BTC Medical Adviser; Daily and weekly food hygiene inspections by the Contractor s doctors. The scope of the inspections generally includes a review of food safety, hygiene, water quality and kitchen safety; Weekly Health Meetings between BTC and Contractor Management. Typical topics might include potable water, hygiene and sanitation, food safety, health screening, medical care and the health awareness programme; An HIV/AIDS and STI awareness campaign in Georgia called SAFE! is being implemented by World Vision International and HERA XXI, with funding from BTC, SCP and World Vision Canada; Assessment of Secondary Health Care Facilities along the ROW in Georgia; Some of the different ways in which HIV/AIDS and STI awareness was raised amongst employees and community members in 2004 are highlighted below: Presentations on infectious diseases and methods to prevent them were held in the main construction camps. Information pamphlets and condoms were presented to attendees; Established the Akhaltsikhe HIV/AIDS and STI telephone hotline;

5 9.2.4 TURKEY Information seminars for young community members were held at World Vision Inter-Communal Centres in Marneuli and Akhaltsikhe; Distribution of information pamphlets to local communities; and Inclusion of information on HIV/AIDS and STI s in Project employee Health and Safety induction training, and toolbox talks. An Occupational Health, Hygiene and Welfare Management review was conducted along the pipeline route by a BTC / Botaş OHH consultant company and site Health and Safety personnel. The scope included Food & Hygiene, medical facilities, camp accommodations and medical case management awareness. Similar management reviews were held at specific sites when the need was highlighted by BTC site management; Botaş OHH specialists continuously undertook inspections and delivered training on food and hygiene, medical facilities and accommodation along the pipeline route. The scope of the inspections included hotels and restaurants on the access route; A three phase STI/HIV Awareness programme initiated by the BTC Project and undertaken by Stars Crescent Alliance (SCA) and UNAIDS in Turkey has so far achieved the following: Training of 10,000 workers and 5,000 members of public; A HIV-themed theatrical show performed at the facilities and communities along the route; An evaluation and follow-up tour of the facilities to monitor effectiveness of the training. Over 4,000 workers participated, and initial results indicate that awareness of STI/HIV has increased; and An HIV Awareness Campaign was launched on World AIDS Day to train the trainers (medical doctors of State medical facilities) at the 10 provinces along the BTC Pipeline. This was in line with the protocol signed by Ministry of Health to ensure sustainability of the programme. 50 doctors attended the first session. Retention of Red Star support (a third party OHH and medical provider) for medical provision, case management and specialist advice; EPC Contractor Medical Teams to monitor food and hygiene, water quality and welfare of the workforce at sites; Alcohol and Substance Abuse training conducted to Health and Safety experts and site medical teams. 9.3 HEALTH AND SAFETY INCIDENTS BTC regularly review Project Health and Safety performance and a monthly report is produced to assist BTC management and BTC partners in these reviews. Table 9.3 is an extract from this report and gives an overview of the numbers of incidents that have occurred. All incidents are fully investigated to ensure that actions are put in place to prevent a recurrence and that lessons learned are shared. 9-5

Minor adjustments have been made to include the latest input and output data, and removal of any SCP related data MAJOR INCIDENTS IN 2004-Q4 Three major incidents occurred in Turkey in the fourth quarter: 1. On 2 nd October 2004 on the Andirin - Goksun highway a dumper truck belonging to a PLL-JV service company, reversed uphill to access a road-crossing cut. The vehicle left the road and rolled down a 20-metre slope, fatally injuring the driver; 2. On 11 th October 2004 on the Horosan - Yenikoy highway a truck left the road and rolled over. During the roll over, the truck driver was ejected from the vehicle sustaining injuries from which he later died; 3. On 1 st November 2004 a sideboom carrying a length of pipe on a steep slope in Lot B stalled and started to free wheel backwards. The operator left his seat and tried to jump clear of the machine, but was struck by the vehicle and fatally injured. These three incidents were investigated to identify the root causes and develop lessons learned from the incidents. The main findings centred on leadership, risk assessment and compliance with procedures and were shared across the Project. Details of the Safety Stand-Down and the additional staff training in vehicle safety and sideboom operation held in response to these fatalities are listed in Section The 2005 Health and Safety programme also lists driving and operating heavy machinery as two of its focus areas for the coming year, and details on this are given in Section 9.4. The Lenders were notified of these three incidents OTHER INCIDENTS IN 2004-Q4 Day Away From Work Case injuries for the quarter are described below: Two employees sustained arm and rib fractures when the pick-up vehicle they were travelling in tipped on its side; A driver received serious injuries when his pick-up truck collided with a truck; A worker fell into an open trench injuring his arm and back; While undertaking repairs to an excavator a mechanic was injured, when ice and mud that had accumulated on the bucket of the excavator fell and struck him on his left shoulder; A Company vehicle collided with oncoming truck and the Company driver sustained a fracture to his arm;

7 Several employees were affected by frostbite at Tsalka Camp in Georgia; During a forklift operation, the operator of the machine lost sight of his flagman while turning a corner and subsequently ran him over. The injured party sustained injuries to both legs; An employee fractured his left leg when a stone struck him during a jackhammer operation; A worker slid on ice and fell down fracturing his left elbow. In addition to the above, 1 person received injuries that resulted in restricted work and 22 others received medical treatment for injuries sustained. A total of 131 people required first aid treatment as a result of minor injuries. The majority of injuries sustained were: Hand and leg injuries; Slips, trips and falls leading to general injuries; and Eye injuries. There were a total of 78 road vehicle incidents against 31,790,285 kilometers driven. The use of seatbelts, head restraints and rollover protection combined with an extensive driver-training program is considered to have had positive impact on reducing the total number of road traffic accidents and their severity. BTC continue to strive to improve driving standards and road safety through training and raised awareness of the drivers, passengers and the community. The Driving Safety Immersion programme developed by BP continues to be rolled out to all BTC and contractor personnel. Regular driving safety audits are undertaken to ensure that standards of vehicle, vehicle maintenance, driver training and certification, and compliance with BTC driving standards are maintained. 8 H&S KEY FOCUS AREAS Analysis of 2004 incidents has identified key areas that require focus throughout (see Figure 9.1). These areas are described below DRIVING Measures to improve driving safety standards include further implementation of existing initiatives and introduction of others including: BP Driving Immersion Programme; Seat belt convincer programme in Turkey; Additional transport audits; Zero Tolerance policy for failure to comply with the driving rules for drivers and transport coordinators; Driving simulator training to enhance driver competence; Winter driving campaign already underway in Georgia and to be implemented in Azerbaijan and Turkey; Winter driving training; Implementation of Safe Driver initiatives; Defensive driving training and assessment programme; and Driving Stand-Downs OPERATION OF HEAVY PLANT & EQUIPMENT Measures to improve operation of heavy plant and equipment include further implementation of existing initiatives and introduction of others as detailed below: Focused Awareness Programme presentation to all heavy plant operators and supervisors; Steep Slope/Heavy Equipment school; Supervisor Safety Leadership Training Programme; Further recruitment of equipment specialists; Competency assessment and grading of operators with redeployment as necessary; Reassessment of operating criteria (the Project reduced acceptable steep slope criteria from 20º to 15º); Increased plant and equipment inspections; Review of risk assessments and method statements; and Full independent pipeline hazard review undertaken OVERHEAD POWER LINES Measures to improve safety when working underneath overhead power lines include further implementation of existing initiatives and introduction of others as detailed below: Detailed surveys tied to Journey Management Plans; Lessons Learned/Safety Flash; Supervisor Safety Leadership Training Programme; Review of risk assessments and method statements; Full Independent Pipeline hazard review undertaken; and Task force established in Georgia. 9-8

9 9.4.4 IMPROPER POSITION OF PEOPLE Measures to improve safety through raising awareness of the importance of position include further implementation of existing initiatives and introduction of others as detailed below: Specific action plans; Safe hands and eyes campaign; A nominated Safety Day in each country; Supervisor Safety Leadership Training Programme; Full Independent Pipeline hazard review; Review of risk assessments and method statements; and Increase Supervision to Worker Ratio. 9-9
