Provides copies of MSDS & PPE Information to student (to take to physician) Take class (at own risk) Student Services Extended study Leave of absence If either of these choices is made, the student will consult with program director/dean to initiate appropriate ES Or LOA procedures. 4. Provides the student with information about: a. purchasing PPE (nitrile gloves, forearm guards, half-mask respirator with cartridges) b. fit-testing at Multispecialty Clinic/Samaritan Occupational Health c. respiratory mask care & maintenance training 5. Documents compliance with fit-testing and mask training on compliance dashboard Send a scanned copy of the form to the Compliance Coordinator for uploading to the anatomy compliance dashboard

3 (Anatomy & Student Services) Personal Protective Equipment (PPE) Information: Pregnant Students The following is a list of required PPE for pregnant students who choose to participate in anatomy laboratory-based courses. Wearing of PPE is required for ALL laboratory exposure situations (lab sessions, exams, independent study etc.). Students are encouraged to take this PPE list to their physicians for inclusion in their discussion about laboratory participation. Pregnant students choosing to participate in anatomy laboratory-based are required to purchase PPE items and any replacement items as needed. Students will meet with a Student Services representative to discuss purchase of required PPE items, respiratory mask fit-testing (including cost) and mask training. These PPE items are typically ordered from Grainger. Grainger catalogue #s (in bold) are included so that item descriptions can be easily accessed. 1. 3M Rugged Comfort Quick Latch Half Facepiece Reusable Respirator 6501, 6502 (comes in multiple sizes) 2. 3M Formaldehyde Organic Vapor Cartridge/Filter (have a shelf life) 3. Condor chemical resistant sleeves 32tl73 4. Ansell nitrile gloves TouchNTuff (come in multiple sizes and have a shelf life)

4 (Anatomy) Laboratory Information (In addition to general laboratory & safety guidelines given to all students) 1. When in the gross anatomy laboratory, wear ALL personal protective equipment (PPE) at ALL times including examinations. 2. Record respiratory cartridge time usage after each laboratory exposure. 3. After each laboratory exposure, store all PPE including clothing, shoes etc. in locker provided. (Do NOT wear work clothes or shoes inside car or home. If possible, replace clothing rather than take clothing home to launder. If clothing must be laundered at home, wash separately from the rest of clothing.) 4. After each laboratory exposure, shower (with regular soap). After delivery, if breastfeeding or expressing milk: 1. Breastfeed or express milk prior to entering the laboratory. 2. After a laboratory exposure, shower before breastfeeding or expressing milk.

5 PPE Checklist for Pregnant Students (Anatomy) Definition of PPE: Personal Protective Equipment PPE includes: 1. Respiratory mask with cartridges (specified for organic solvents and fit-tested) 2. Nitrile disposable gloves 3. Forearm guards (shields) 3. Additional PPE as described in the course syllabus (eye wear, close-toed shoes, etc.) Student Initials 1. I am responsible for the purchase of all PPE listed above including replacement of respiratory cartridges, gloves, and forearm guards. 2. I am responsible for the cost of the initial fit-testing the respiratory mask as well as any subsequent fittests that might be done. 3. I am responsible for the maintenance and cleaning of the respiratory mask as well as cartridge replacement in accordance with the manufacturer s instructions provided with the mask and cartridges. 4. I am responsible for having and wearing ALL PPE during ALL times that I am in the gross anatomy laboratory. If I do not have or am not wearing the appropriate PPE, faculty or laboratory staff will ask me to leave the lab. In this case, no credit for laboratory attendance, quizzes, etc. will be given to me for that session. 5. I am responsible for storing all PPE in a laboratory locker. I have read, understand, and agree to comply with the PPE statements listed above.
