2 Who we are Hello, I am Vindi Bhandal. I m a GP in Sleaford and I m Acting Chair of South West Lincolnshire CCG. South West Lincolnshire Clinical Commissioning Group became a statutory organisation on the 1 April 2013 in line with the Health and Social Care Act 2012 and subsequent changes to the NHS nationally. GPs from all our practices are involved in the development of services and quality improvement in our locality. They see patients every day and are therefore best placed to understand the health needs of the communities they serve. Our overarching plan for the commissioning of health services for our population is based on our vision for healthcare as set out in the Shaping Health for Mid-Kesteven public consultation held during November 2012 and February This follows dialogue we have been having with local people, patient groups and our stakeholders over the past two years and is consistent with the priorities identified in the Lincolnshire Health and Wellbeing Strategy. We want to see more services provided locally giving patients greater choice and we are committed to improving health services to ensure continued improvement in the health and well-being for all residents within our locality. We care passionately about our local NHS and we believe that high quality services need to be accessible to the whole community. Our clinicians are well placed to lead the development of services and quality improvement in the locality but we can only do this by close working with councils, local people, health professionals and care providers to design the very best services. Dr Vindi Bhandal What we do Our main responsibilities include: Listening to patients, carers and local people to understand health needs, create choices and take views into account Giving information and power to people to manage their own health and to influence the bigger picture for South West Lincolnshire Analysing the health and social care needs of our local population working with the newly established Lincolnshire Health and Wellbeing Board Planning health services for the next year and for the future working with our practices, partners and local people Commissioning other organisations to provide services in line with our plans Agreeing service contracts and managing performance against those agreements on your behalf Making the best use of the resources we have to provide healthcare Some facts and figures Area we cover: Districts of Sleaford, Grantham and outlying villages of Sleaford and Grantham, covering approximately square miles Population: Approximately 128,000 Main Health Issues: Obesity and diabetes are high priority areas for the CCG Budget 2012/13: 145 million Priorities for Improvement More efficient services for urgent and unplanned care Care for older people Care for people with long term conditions such as diabetes and dementia Our vision for the future is to improve lifestyles, avoiding illness by treating people earlier and avoiding emergencies. Joined up health and social care and caring for people at home and in the community where possible. When people do go to hospital that it is close to home where it is safe to do so. Our Shaping Health for Mid-Kesteven Programme focussed on improving care in Grantham Hospital and in the local community to ensure people receive the best possible care within resources available. The key projects are: Enhanced A&E to include better integration with other local health professionals in one location. Increased range of planned care for example more day case procedures at Grantham More community based access to medical assessment, treatment and recovery Rapid assessment in hospital where there is a medical need Improvements in end of life care Integrated children s services on one site to enable a holistic approach to care Midwife led birth alongside obstetric care Improvements in diagnosis and care of long term conditions How we spend your money 2 South West LIncolnshire Clinical Commissioning Group 3

3 How we make decisions Our Governing Body will be meeting in public to agree new health plans, budgets and month-by-month quality and performance improvement. Dates of Governing Body meetings can be found on our website where you can see what s on the agenda prior to the meeting. We have an Executive Committee which works on the design of services and operational issues. They make recommendations to the Members Council and Governing Body about service changes and new developments to improve care for patients. The various projects that are developed involve local people, particularly those who have experience of using services; and they work closely with GP practices in Sleaford, Grantham and surrounding villages. Many practices have a patient participation group, so there is a strong connection with patients and professionals on the front line. Living up to our public promise - The NHS Constitution The NHS Constitution was first published on 21 January It brings together in one place what staff, patients and public can expect from the NHS. It sets out the purpose, principles and values of the NHS and explains a number of rights, pledges and responsibilities for staff and patients alike. Did you know..? If your GP refers you for treatment, you have the right for any non-emergency treatment to start within a maximum of 18 weeks, or for the NHS to take reasonable steps to offer you a range of alternatives if this is not possible. You also have the right to be seen by a specialist within a maximum of two weeks from GP referral for urgent referrals where cancer is suspected. If your GP refers you to see a consultant, you generally have a choice of a number of hospitals. You might want to choose a hospital that has better results for your treatment than others, or one near family that live in another part of the country, or one that has shorter waiting times. You can view your personal health records. You do not have to give a reason to see them; just ask at your GP surgery You should always be treated with dignity and respect, in accordance with your human rights. This means, for example, that your right to privacy should be respected. You should not have to share sleeping or bathroom facilities with members of the opposite sex, except on the rare occasions where you need very specialised or urgent care. You have the right to have any complaint you make about the NHS dealt with efficiently and have it investigated properly. 4 How you can be involved We want to our job in the best way possible by working with you. Here are some of the ways to get involved: Join a patient participation group one of the best ways to have your say about local health services is to join your GP surgery s patient participation group. These groups meet regularly, and as well as influencing what happens within your own practice, your views on South West Lincolnshire issues can also be fed through to the CCG. Join our viewpoint panel the view point panel is a database of local people who are interested in having a say in how health services are developed and delivered. When registering you will be asked for basic information about yourself and the sorts of issues you are interested in and the type of activities in which you are willing to be involved. Join our readers panel if you are interested in how information is provided, enjoy reading and would be able to comment and make suggestions on language and layout of materials, we would be delighted to welcome you to our readers panel. The CCG is currently setting up a Patient Council which will act as a diverse reference group, to enable patients in South West Lincolnshire to make an effective contribution to the prioritisation, design, planning and commissioning of health services in alignment with the CCG s strategic objectives.
