1 Bedside Medication Delivery of Discharge Medications Overview Upon admission, all patients will be asked if they desire bedside delivery of discharge medications. The question is required and is included in the standard Nursing Admission History: Answering NO ends the process and no further action is needed. Answering Unable to Determine at this time will generate a task to Nursing to re-asses. Clicking the task launches the Bedside Delivery of Meds at Discharge form, which defaults to last charted value. Answering YES or changing the answer from Unable to Determine at this time to Yes will enroll the patient in the Bedside Medication Delivery program and notify the outpatient pharmacy of this enrollment. 2 Pharmacy notification will appear on the Multipatient Task List: Clicking the task launches the Bedside Delivery of Discharge Medications Verification Form: User Handouts\Medications\Bedside Medication Delivery Overview Scott Barger 2
3 The pharmacy will then work the task by contacting the patient and/or discharging provider to coordinate the service. IF at any time, the patient changes his/her mind, NURSING or PHYSICIANS can launch the Bedside Delivery of Meds at Discharge Form from AD HOC Charting and change the answer from YES to NO. Doing so will send ANOTHER task to pharmacy, indicating that the service has been cancelled: The pharmacy can then complete the charting of the REQUESTED task by indicating Rx Not Delivered, See Notes: And then take of the CANCEL task with a single click in the Quick Chart column: Click HERE to quick chart, then click OK in the Chart Done window. 4 BEDSIDE DELIVERY: Provider Process If the answer to the Bedside Delivery of Meds at Discharge is YES, the first attempt at creating a prescription by anyone thereafter whether from Medication Reconciliation or Medication List/PowerOrders will result in the following alert the moment the orderable is selected: Clicking OK will return the user to normal order entry without any restrictions for ordering. The prescriber must then take the appropriate action. Printed Prescriptions will contain instructions for obtaining the service. See the next page for an example. Electronically Transmitted Prescriptions should be routed to the appropriate Mizzou pharmacy for any med that is desired to be delivered to the bedside. Other medications (such as routine refills of maintenance medications) can be eprescribed to the patient s preferred pharmacy. Clicking Un-Enroll launches the Bedside Delivery of Meds at Discharge form, which defaults to last charted value: Selecting No and signing the form (teal checkmark) will unenroll the patient and notify the Pharmacy.
