9 Optum Care Blood Pressure Challenge Goal: Submit as many normal blood pressure (BP) readings or documented retakes as possible. (Normal BP considered 139/89 and below) Our main goal with the challenge was to be better about double checking blood pressures If they were continuing to have high readings after letting them rest then we would set up a blood pressure visit. - Anne Herbert, Canyon View MA Timeline: November 1, 2015, to December 31, 2015 Points awarded for: Healthy range BP submissions OR MA retook BP after letting patient rest

15 Challenge Goal: Increase STAR rating by 0.5 stars for the following outcomes: Timely initiation of care Received flu shot for current flu season Drug education on all medications Improvement in bed transferring Improvement in pain interfering with activity

16 Change Process Focus on 3 process measures: Timely Initiation of Care Drug Education on all medications Influenza vaccine received in season M2020 Oral Medication Management must be able to take ALL oral medications reliably and safely (correct dosages at appropriate times every time)

17 Results & Maintenance Drug education on all medications From 1 to 3 stars! Our overall STAR ratings went from 3 to 3.5 stars HHCAHPS All medication-related questions are above the national average Ongoing efforts focus on OASIS accuracy, especially at SOC

18 Lessons Learned Focused staff education during the first 120 days of 2016 significant improvement in 6 out of 9 quality measures (Real-Time STAR ratings report from SHP)
