3 Automated External Defibrillator Program (AED) This document describes the policies of Strathmore Union Elementary School District (SUESD) relating to its Automated External Defibrillator (AED) program. The document is intended to provide guidance to administrators and responders of the AED program in order to ensure a rapid response to victims of sudden cardiac arrest (SCA) in SUESD. AED Program Coordinator The Program Coordinator for SUESD s AED program is the Program Manager for School Health Programs at Tulare County Office of Education, Nan Arnold. The TCOE AED Program Coordinator s responsibilities are to: Communicate with the Enerspect Medical Director, SUESD Site Coordinator, and SUESD management/administrative personnel regarding the AED program. Assure case reviews, data collection, and other quality assurance activities are taking place at all sites, and participate as necessary. Assure that a plan for maintaining the defibrillators and related response equipment is implemented at all sites. AED Site Coordinator-SUESD School Nurse The AED Site Coordinator for SUESD is the school nurse, Sherry Hall. The responsibilities of the school nurse are to: Oversee the AED program for Strathmore Union Elementary School District. Develop and maintain emergency response procedures for the AED program. Communicate with TCOE AED Program Coordinator, emergency responders, management, Emergency Medical Services (EMS), employees, and guests regarding the AED program as necessary. Ensure the defibrillators and related response equipment are maintained. Ensure compliance with state and local regulations regarding defibrillator use. Ensure that someone is available during school hours to respond to calls for help from within the facility. Assure that EMS (Emergency Medical Services call 9-1-1) is notified in the event of an SCA. Train office staff to direct someone to be positioned to meet EMS responders and lead them to the emergency location whenever EMS is called. Provide documentation of and follow-up for any device use. Notify SUESD management, emergency responders, and TCOE AED Program Coordinator if a defibrillator is removed from service. Purchase necessary supplies and accessories. Participate in case reviews, data collection, and other quality assurance activities for the site. California Good Samaritan Law The California Good Samaritan Law (Health and Safety Code Section ) states that no person who in good faith, and not for compensation, renders emergency medical or nonmedical care or assistance at the scene of an emergency shall be liable for civil damages resulting from any act or omission other than an act or omission constituting gross negligence or willful or wanton misconduct Automated External Defibrillator Program (AED) SUESD APPROVED 11/15/18 Page 3 of 6

4 Under this law, any person may voluntarily provide emergency care, including CPR, defibrillation, and first aid in SUESD sites. Equipment SUESD has selected the Philips HeartStart defibrillator for its AED program. The defibrillator should be deployed to any medical emergency in the facility along with other emergency care equipment. If infant/child pads are not present, the device may be used for younger victims using standard adult pads placed on the chest and back. The device should be used on any victim who is: Unresponsive. Not breathing normally. If in doubt, apply the pads. Defibrillators are placed in the following locations at SUESD: 1. Elementary school office. 2. Middle school office. 3. Office of the athletic director to be used during athletic activities. With the exception of the device in the office of the athletic director, AEDs at each of these locations is in a cabinet and is marked overhead with a sign. Each defibrillator kit contains: one Philips HeartStart Defibrillator, with battery installed, and case one set of adult defibrillator pads one spare battery one pocket mask or other rescue breathing barrier device one pair of disposable gloves one razor one pair of scissors one small disposable towel other equipment Maintenance The HeartStart defibrillator requires little maintenance. The defibrillator performs daily tests to help assure the device is ready for use, and is equipped with a status indicator that shows if the device has passed its last self-test and is ready for use. All defibrillators shall be maintained in accordance with the HeartStart Defibrillator Instructions for Use. The school nurse or his/her designee shall inspect each defibrillator according to the recommendations in the defibrillator user documentation, in order to assure that the device is ready for use and that all supplies are present and have not reached the use-before or install-before dates on the packaging. Any problem with the defibrillator or related emergency equipment shall be reported to the school nurse immediately. If a defibrillator must be removed from service, the school nurse shall notify the TCOE AED Program Coordinator and emergency responders (Strathmore Fire Department at ). Notification of the same group shall occur when the device is returned to service Automated External Defibrillator Program (AED) SUESD APPROVED 11/15/18 Page 4 of 6

5 Post-Event Activities After any response to SCA with a defibrillator: The school nurse, TCOE AED Program Coordinator, and Enerspect Medical Director shall be notified within 24 hours of the event. The defibrillator shall be checked and any used supplies replaced as soon as possible following the event so that the defibrillator may be returned to service. The after-use maintenance shall be performed on the defibrillator. The Enerspect Medical Director, TCOE AED Program Coordinator, or school nurse shall conduct employee incident debriefing, as needed. The school nurse shall complete the Incident Follow-Up Report and forward it to the Enerspect Medical Director. Defibrillator Maintenance after Each Use Each time the defibrillator is used on a victim: Inspect the exterior, pads connector port or pads cartridge well for dirt, contamination, or damage. Check supplies, accessories, and spares for expiration dates and damage. Check the operation of the HeartStart Defibrillator by removing and reinstalling the battery to run a battery insertion test. AED Program Assessment and Review Any actual SCA events shall be reviewed by the Enerspect Medical Director to ensure that the AED protocol was followed and to assess the response performance of the facility. Recommendations for change or improvement should be discussed with the TCOE AED Program Coordinator, school nurse, and responders as soon as possible after an event. The TCOE AED Program Coordinator should review each site's training and maintenance records to ensure compliance with SUESD protocol. AED Response Protocol The following protocol is for use by the emergency responders, including those acting under the California Good Samaritan Law. Initial Protocol for Unresponsive Victim Assess scene for safety; use universal precautions. Assess victim for responsiveness. If unresponsive, activate EMS (Emergency Medical Services) and in-house emergency plan by phoning and/or designated number. Call for AED to be brought to the scene. Briefly assess patient's respiratory status (5-10 seconds). If respiratory effort is absent, or patient is only gasping, begin CPR starting with compressions, at a depth of inches for adults, or 1/3 the depth of the chest for children and infants. Continue CPR until AED arrives, alternating between 30 compressions and 2 breaths, at a rate of compressions per minute. For a witnessed arrest, or if desired, use compression only CPR, without performing breaths Automated External Defibrillator Program (AED) SUESD APPROVED 11/15/18 Page 5 of 6

6 Begin AED Response As soon as AED is available, turn on AED and follow prompts. If needed: o Remove victim from pool of water. (AED may be used on snow or ice.) o Wipe chest if wet from water or sweat. o Shave chest with disposable razor. (Discard razor in a safe manner.) Apply AED pads. For victims under 55 lbs or less than 8 years of age, use pediatric pads or pediatric setting if available. If infant/child pads are not present, the device may be used for younger victims using standard adult pads placed on the chest and back. Make sure that AED pads are placed in proper location and make good contact with victim's chest. Do not place AED pads over the nipple, medication patches, or implantable medical devices. (Note - if victim has an implantable cardiac defibrillator, ICD, which is discharging, the AED will note this and prevent a shock until it clears. A one inch/2.5 cm separation between AED pad placement and implantable medical device is recommended.) If shock is recommended, shout "clear" and do not touch victim. Deliver shock as advised by AED. Resume CPR for 2 minutes, then allow AED to assess victim's rhythm. Repeat cycle as needed. Continue CPR and follow AED defibrillation prompts until otherwise directed by AED or EMS. Transfer responsibility for victim to EMS when directed or appropriate. When Emergency Medical Services (EMS) Arrives Responders should document and communicate important victim information if known to EMS such as; name, known medical problems, allergies, medical history, time found, initial and current condition, and information from the AED if available, number of shocks delivered, time of first shock. Assist as requested by EMS. Post-Use Procedure Document SCA event and contact Enerspect Medical Solutions LLC immediately, either by phone ( ) or through the AED Total Solution portal, by selecting "Report Event" from the home page. Check AED and replace any used supplies as soon as possible following the event so that AED may be returned to service. Clean AED if needed.
