1 B-16 COLLEGE OF THE SEQUOIAS DIVISION OF NURSING AND ALLIED HEALTH TITLE: PURPOSE: DESCRIPTION: CREDIT BY EXAMINATION This policy describes the general procedure for challenging one or more courses within the COS Nursing Program. This policy applies to the advance placement student and military personnel who have held Military Health Care Occupations. The credit by examination procedure is described in the current college catalog. The student must file a petition for approval to challenge a course by examination with the Admissions and Records Office. This petition must be approved before the Credit by Exam begins. *Individuals who have held Military Health Care Occupations, specifically: Basic Medical Technician Corpsman (Navy HM or Air Force BMTCP), Army Health Care Specialist (68W Army Medic) or Air Force Independent Duty Medical Technician (IMDT 4N0X1C) may achieve advanced placement into 2 nd semester of the nursing program with documentation of education and experience (within the last two years) qualifying them for the specific Military Health Care Occupation and upon successful completion of the challenge option that consists of two (2) parts theory portion and clinical portion. Once approval is received, the process in the division is as follows: 1. The challenge option for each course being challenged contains two (2) parts: Part 1: Theory portion Part 2: Clinical portion 2. Students wishing to challenge courses within the COS Nursing Program must notify the Director of the program in writing a minimum of six (6) full weeks before the semester begins. 3. If a student chooses to challenge a course, ALL portions (theory & clinical) in that course must be challenged. 4. The student must first achieve a score of at least 75% on the written exam for the theory portion of the course being challenged. If the theory score is below 75%, the student must take the course as scheduled. 5. After achieving a score of 75% or more on the theory written exam, the student will take the proctored ATI exam given to all students in the course being challenged and must pass with a Level 2 or higher (see Policy A-23, ATI Testing).

2 6. Once the theory portion has been passed with 75% or more, the student must take the clinical part of the challenge option. If a student earns a satisfactory (Pass) rating on the clinical portion, the grade earned will be the grade achieved on the written portion. If clinical performance is less than satisfactory, the student will take the course as scheduled. 7. The clinical portion of the challenge option will comprise at least one eight-hour day where the student is involved in direct patient care activities. 8. The Student Evaluation Record (SER) will be used to determine the student s clinical competency. Refer to SER. 9. Where applicable, the student must complete a satisfactory nursing care plan for each clinical rotation being challenged. 3 COLLEGE OF THE SEQUOIAS DIVISION OF NURSING AND HEALTH SCIENCE Challenge Procedure for NURS 161 A student who meets the requirements of the Nursing Division Challenge Policy may challenge the theory and/or clinical components of the NURS 161 course. Procedure: 1. Pass all NURS 161 exams (quizzes, midterm, and final). 2. The student must achieve a score of 75% or greater in order to pass the theory 74.9 will not be rounded up to a 75). 1. Pass all skills lab activities. 2. During a clinical week (2 clinical days), care for 2 patients, performing a head-to-toe physical exam, personal hygiene, activity/mobility exercises, and any appropriate procedures. 3. Construct a complete care plan.

Procedure: 1.. The student will take a written examination containing multiple-choice questions which cover theory content as follows: Alterations in Fluid, Electrolyte, & Acid-Base Balance 15 Alterations in Ventilation 15 Alterations in Endocrine Functions 15 Alterations in Perfusion 15 Alterations in Genitourinary Function 15 Alterations in Gastrointestinal Functions 15 Theoretical Content Related to Clinical Skills 5 Mathematical Calculations Related to Drugs & IV Fluids 5 3. The student must achieve a score of 75% or greater in order to pass the theory 74.9 will not be rounded up to a 75). 1. The student must spend one (1) full day in the clinical areas with a NURS 151 Instructor. Additional clinical days may be required if the Instructor needs more time in the clinical setting to observe the student. 2. The student will perform complete patient care, medication administration, medication research forms, biographical data forms, lab analysis form and a comprehensive assessment on all assigned clients. The student will be assigned 1-2 clients. 3. The student must satisfactorily complete an assessment, care plan and charting on one client, plus any additional assignments.

Procedure: 1. The theory challenge will be a comprehensive test of the perinatal nursing course. 2. The student will take a written examination containing multiple-choice questions which cover theory content as follows: Pregnancy and the Birthing Process 8 Postpartum Period and Neonatal 8 Adjustment to Extrauterine Life Complications During Pregnancy, Birth, and Postpartum 8 Promoting Women's Health Throughout the Lifespan 2 Medications and/or Dosages Relevant to Each Area 2 3. The student must achieve a score of 75% or greater in order to pass the theory 74.9 will not be rounded up to a 75). 1. The student must spend one (1) full day in the perinatal clinical areas with a NURS 152 Instructor. Additional clinical days may be required if the Instructor needs more time in the clinical setting to observe the student. 2. The student will perform complete patient care, including a comprehensive assessment specific for the specialty area. The student will be assigned 1-2 clients. 3. The student must satisfactorily complete an assessment, care plan and charting on each assigned client, plus any additional assignments pertinent to the specialty area.

Procedure: 1. The theory challenge will be a comprehensive test of the pediatric nursing course. 2. The student will take a written examination containing multiple-choice questions which cover theory content as follows: Pediatric Nursing Role focusing on Pediatric Assessment 5 Development and Psychosocial Concerns of Age-related Events/Issues 4 Community Acquired Disorders/Immunizations 4 Pediatric Nutrition/Fluid Maintenance Requirements 4 Pain Assessment/Nursing Management if the Pediatric Client 3 The Special Needs Child 3 Care of the Pediatric Client with alteration in function of the: Respiratory System 4 Cardiovascular/Hematologic Systems 5 Gastrointestinal System 4 Genitourinary System 4 Neuro-musculoskeletal Systems 5 Endocrine and Integumentary Systems 5 3. The student much achieve a score of 75% or greater in order to pass the theory 74.9 will not be rounded up to a 75). 1. The student must spend two (2) full days in the pediatric clinical areas with a NURS 153 Instructor. Additional clinical days may be required if the Instructor needs more time in the clinical setting to observe the student. 2. The student will perform complete patient care, including a comprehensive assessment specific for the specialty area. The student will be assigned 1-2 clients.

7 3. The student must satisfactorily complete an assessment, care plan or mindmap, and charting on each assigned client, plus any additional assignments pertinent to the specialty area.

8 COLLEGE OF THE SEQUOIAS DIVISION OF NURSING AND ALLIED HEALTH Challenge Procedure for NURS 154 A student who meets the requirements of the Nursing Division Challenge may challenge the theory and/or clinical components of the NURS 154 course. Procedure: 1. The theory challenge will be a comprehensive test of all the psychiatric nursing content as outlined in the course syllabus. 2. The student will take a written examination which cover theory content as follows: Unit 1. Maintaining and Promoting Optimal Wellness in a Client by Making Adaptations in the Environment. Unit 2. Promoting Wellness in a Client Who is Experiencing a Crisis. Unit 3. Maintaining and Promoting Optimal Wellness in a Client by Making Adaptations Through Psycho-pharmacological Medications. Unit 4. Promoting Optimal Wellness in a Client Who is Experiencing An Anxiety Disorder. Unit 5. Promoting Optimal Wellness in a Client Who is Experiencing A Depressive Disorder. Unit 6. Promoting Optimal Wellness in a Client Who is Experiencing A Psychotic Disorder or a Schizophrenic Episode. Unit 7. Maintaining and Promoting Wellness in a Client Who Has A Personality Disorder. Unit 8. Maintaining and Promoting Optimal Wellness in a Client Who Has a Chemical Dependency and/or Addiction Problem. Unit 9. Maintaining and Promoting Optimal Wellness in a Client Who Has an Alteration in Organic Functioning. Unit 10. Promoting Optimal Wellness in a Client Who has been A Victim Of Abuse and/or Violence. Unit 11. Maintaining and Promoting Optimal Wellness in a Client Who Has an Eating Disorder. Unit 12. Promoting Optimal Wellness in a Client Who is Experiencing A Psychological and Physiological Response to Being Ill. 3. The student must achieve a score of 75% or greater in order to pass the theory 74.9 will not be rounded up to a 75).

9 1. The student must spend one (1) full day in the clinical area with a NURS 154 Instructor. Additional clinical days may be required if the Instructor needs more time in the clinical setting to observe the student. 2. The student will perform complete patient care, including a comprehensive assessment specific for the specialty area. The student will be assigned 1-2 clients. 3. The student must satisfactorily complete an assessment, care plan (either verbal or written) and charting on each assigned client, plus any additional assignments pertinent to the psychiatric clinical portion of the course, i.e., clinical exit exam.

10 COLLEGE OF THE SEQUOIAS DIVISION OF NURSING AND HEALTH SCIENCE Challenge Procedure for NURS 163 A student who meets the requirements of the Nursing Division Challenge Policy may challenge the theory and/or clinical components of the NURS 163 course. Procedure: 1. The student will take a written examination containing multiple-choice questions which cover theory content as follows: Alterations in Fluid, Electrolyte, & Acid-Base Balance 10 Alterations in Ventilation 10 Alterations in Endocrine Functions 10 Alterations in Perfusion 10 Alterations in Cell Growth (Cancer) 10 Alterations in Genitourinary Function 10 Alterations in Gastrointestinal Functions 10 Theoretical Content Related to Clinical Skills 10 Mathematical Calculations Related to Drugs & IV Fluids The student must achieve a score of 75% or greater in order to pass the theory 74.9 will not be rounded up to 75). The student will: 1. The student must spend two (2) full clinical days of the rotation with a NURS 163 Instructor. Additional clinical days may be required if the instructor needs more time in the clinical setting to observe the student. 2. The student will perform complete patient care, including charting and administration of all ordered medications and IV fluids, on three (3) patients per day. 3. The student must satisfactorily complete comprehensive assessments and full nursing care plans for each assigned patient, including medication research forms for all ordered medications.

11 College of the Sequoias Division of Nursing and Allied Health Challenge Procedure for NURS 164 A student who meets the requirements of the Nursing Division Challenge Policy may challenge the theory and/or clinical components of the NURS 164 course. 1. The student will take a written examination containing multiple-choice questions that cover theory content as follows: Promoting and Restoring Optimum Wellness in Complex Clients 10 Clients With Acute Cardiac Alterations and Arterial Insufficiency 10 Clients With Acute Alterations in Respiratory Function 10 Clients With Acute Alterations In Neurological Function 10 Clients With Acute Alterations In Endocrine/Metabolic Function 10 Clients With Multisystem Alterations The student must achieve a score of 75% or greater in order to pass the theory 74.9 will NOT be rounded up to a 75). 1. The student must attend a minimum of two (2) full clinical days of the rotation with a NURS 164 Instructor. Additional clinical days may be required if the Instructor needs more time in the clinical setting to observe the student. 2. The student will perform complete patient care, including charting and administration of all ordered medications and IV fluids, on 2-3 patients per day. 3. The student must satisfactorily complete comprehensive assessments, care plans, and charting on each assigned client, including medication research forms for all ordered medications.

12 College of the Sequoias Division of Nursing and Allied Health Challenge Procedure for NURS 166 A student who meets the requirements of the Nursing Division Challenge Policy may challenge the theory and/or clinical components of the NURS 166 course. 1. The student will take a written examination containing multiple-choice questions that cover theory content as follows: Working within Organizations to Provide Nursing Care for Clients 10 Expanding Use of the Nursing Process as an Organizational Tool 10 Assuming Responsibility for Parameters of Nursing Practice 10 Developing Your Management Skills 10 Evaluating and Improving Client Care 10 Professional Progress in the Health Care Field The student must achieve a score of 75% or greater in order to pass the theory 74.9 will NOT be rounded up to a 75). 1. The student must meet with the NURS 166 instructor and identify a mentor nurse. 2. Once the mentor nurse has agreed to work with the challenge student, the student will work with the mentor nurse for a minimum of one (1) to two (2) 12-hour shifts. Additional shifts may be required if the student is unable to complete required assignments. 3. The student will complete the following assignments and achieve a score of Pass. a. Client Admission/Care Plan competency evaluation b. End of Shift Report competency evaluation c. Interdisciplinary Care Conference competency evaluation. d. Mid-rotation evaluation with the student's mentor e. Final evaluation with the student's mentor

13 4. The student will complete the delegation project and achieve a cumulative score of 75%.
