6 2008 Honor Roll Criteria 1. Score in the top national 25th percentile on at least 3 of 4 CMS clinical categories. The hospital is disqualified if any score is below 50th percentile. AND 2. Meet either HCAHPS target OR Leapfrog target: a. HCAHPS- Score at or above national average on at least 6 of 8 questions AND on the "highest patient rating" question. b. Leapfrog- score in top 25th percentile of 4 NE states.
