2 Year in Review Polk Expands Physical Therapy Gymnasium and Activity Space The expansion of the Physical Therapy gymnasium was completed in February The gym renovations will allow more patients to use the space at the same time using a variety of therapy equipment. The expansion also includes private areas for one-on-one auditory and speech therapy as well as space for occupational therapy patients to practice activities of daily living, such as using appliances. The gym project also includes office space for Rehabilitation Services staff. The $246,000 project, which began in September 2016, also includes an expansion of the activities room on the hospital s West Wing, allowing for more classes and activities for patients in Polk s Sub-Acute Rehabilitation program, also known as swing bed. Volunteer Services Recognized for Polk Gift Shop Volunteer Services received the Georgia Society for Volunteer and Retail Professionals Best Practices Award in the category of Fundraising. The award recognizes the success of the Polk Medical Center Auxiliary Volunteer Gift Shop, which, with the support of employees and the community, raised $10,000 in its first year of operation. All money raised by gift shop sales is used to enhance patient care. The award recognizes unique and innovative fundraising programs that have made a significant impact on the hospital or community. Polk Medical Center Launches Live Well Polk! Polk Medical Center launched a three-year patient and community engagement effort, Live Well Polk! in September, The program aims to improve the health of Polk County residents. The program includes partnering with existing businesses and industries as well as introducing challenges to the community that focus on stopping bad habits and starting new, healthy habits. Activities include sponsoring the community booth at the Rockmart Farmer s Market, providing screenings at senior centers in Cedartown and Rockmart and providing disease-specific information to the community. Polk Establishes Patient Family Advisory Committee This council of community leaders, health care consumers and Polk Medical Center staff members, formed in 2016 to promote improved relationships between patients, families and staff. The committee provides an avenue of communication between these varied stakeholders and serves as a venue for patients and families to have input into policy and program development. It also gives patients and families the opportunity to participate in the development of new facilities and programs at Polk Medical Center, while providing Polk staff members an opportunity to listen to their customers. Spiritual Care Team Helps Meet Patients Needs Patient care extends beyond meeting the medical needs of those hospitalized at Polk Medical Center. The hospital s three-member Spiritual Care Team helps to meet the spiritual and emotional needs of patients while they are in the hospital. These volunteers are available every day and visit patients to let them know how they can be reached. In 2016, the Spiritual Care Team recorded 1,147 visits with patients, who say that their support was an important part of their hospital stay. These volunteers provide spiritual and emotional support to patients, oftentimes simply through listening. Polk has 21 volunteers who serve throughout the hospital, including greeting visitors at the front desk and serving in the hospital s gift shop. 2 Report to the Community

3 Community Benefits Summary Polk Medical Center is a 25-bed critical access hospital accredited by The Joint Commission and leased by the Cedartown-Polk Hospital Authority to Polk Medical Center Inc., an affiliate of Floyd Medical Center. Polk Medical Center generates more than $29.1 million in economic activity in the state. Polk has about 100 full and part-time employees. Services Polk Medical Center is a critical access hospital providing the following services to the Polk County and northern Haralson County communities. Breast Health Cardiac Rehabilitation Cardiology Diagnostic Radiology Emergency Room Emergency Medical Services Family Medicine Hospice Outpatient Rehab and Sports Medicine Senior Enrichment Program Indigent Care Polk Medical Center is committed to providing health care services to all individuals regardless of ability to pay. In FY2016, Polk Medical Center provided $3.43 million in unreimbursed care to individuals. The Polk Medical Center pharmacy provided prescription drugs at no or minimal cost to indigent patients being discharged from inpatient care or the emergency room at a cost of $1,846 during FY2016. Sub-acute Rehabilitation Program Surgery Wound Care Urgent Care Community Service Polk Medical Center co-workers and volunteers reached 7,718 Polk County residents through community endeavors at an expense of $16,669 in FY2016: Conducting health fairs Providing school-based education programs to students about automobile safety, bicycle safety, safe play and careers Providing diabetes education Providing heart disease education Providing medical support at sports and community events 2016 Polk Annual Report 3

4 Community Benefits Summary Outreach As a community hospital, Polk is continuously looking for opportunities to reach farther into our community to meet the needs of the full spectrum of individuals who seek medical care in the service area. Members of the Polk team are committed to the community in many ways. In FY2016, Polk co-workers loaned their talents and leadership skills to school, civic and professional organizations.
