2 TOTAL PROJECT COST Cost to Reestablish Vegetation Construction Costs (if any) Total Project Cost: $ $ $ (One way to estimate total project cost is to first multiply the total square feet of the project by $1 to provide an estimate of the cost to restore vegetation on-site. This cost would include materials to establish native plants on-site including: seedlings, seeds, mulch, erosion control material, herbicide and other basic project materials. If any additional work is needed on-site to complete this project (grading/re-sloping site, removal of retaining walls/rip-rap, erosion control measures) please add these construction costs to the cost of reestablishing the vegetation to get a final estimate of total project costs. GRANT REQUEST How much grant money are you requesting? The grant program primarily covers project costs for materials, supplies and labor to install the vegetation and any approved construction costs to prepare the site $: Every three dollars of grant money received, must be matched by one dollar from the grantee (for example, for a $6,000 grant, your match would be $2,000 of which $500 would be a cash contribution). This program does require that a portion of this match be a cash contribution to the project. Please circle the appropriate amount of cash match below based on grant amount you are requesting: $500 cash match (Grant < $10,000) $1,000 cash match (Grant > $10,000) This remainder of the match can be an in-kind donation of labor, time and materials to install, establish and maintain the project. The time spent by the grantee, volunteers, local government or other non-state employees count toward meeting your in-kind match. If multiple properties are involved, please complete this information for each site. Include a sketch of the project site that indicates the location and size of each of the planting zones (aquatic, transition, and upland), paths and existing structures. B. 6 F. Plant List and Source: All plants used in your project must be native to Minnesota and to the county the project is located in. To find a list of native plants in your county please refer to the Restore Your Shore CD Rom or Lakescaping for Wildlife book. For more information go to: No cultivars of native plants or non-native plants are allowed in the project site. You may include a complete project plant list with this grant proposal or a list of species you would like to have in your project, but all projects must have their final plant lists reviewed and approved by a DNR Shoreland Habitat Specialist before the project is started. Do you plan to use the state contract greenhouse or purchase native plants from a private vendor? State Contact or Private Vendor Name of Private vendor (if determined) G. Shoreland Habitat Grants are administered on a reimbursement basis. This means that that designated grantee is responsible for paying for all project costs, and the DNR will reimburse the designated grantee for approved expenditures. If this project is funded, who will be the designated grantee (usually a local unit of government/lake association/private citizen)? H. Maintenance Plan: Describe the proposed short-term and long-term maintenance activities for the project. Include information on who will be responsible for maintenance and when maintenance activities will occur.
