1 2015 Grant Recipients Pitkin County Healthy Community Fund Agency Grant Activities supported by Grant Amount A Way Out $5,000 A Way Out supports people in drug and alcohol crisis to access the treatment they need. Funding is for operational costs to meet the needs of residents in Pitkin County. Access Roaring Fork $7,000 This grant is for general operating support for the Second Shift After School Program, which is one of the three programs provided by ARF. ACES: Aspen Center for Environmental Studies $10,000 Provides operating support for ACES to continue offering its core programs, including in-school environmental science education, sustainable agriculture, forest health, youth and leadership programs, and naturalist programs, to name a few. Alpine Legal Services $20,000 Offer programs that provide legal assistance to crime victims, seniors, residents of affordable housing and others who cannot access the legal system because of financial need or other challenge who live and/or work in Pitkin County. Andy Zanca Youth Empowerment Program Aspen Camp of Deaf & Hard of Hearing Aspen Community Church CO West Regional Mental Health dba Mind Springs Health Mind Springs Health Inc. dba Aspen Detox and Drug Testing Aspen High School Project Graduation Aspen Homeless Shelter $4,000 Develop leadership, multiculturalism and identity by providing training and opportunity to youth in community broadcasting. Participants are given access to media education, broadcast journalism, technical training and production activities. $8,000 This grant will support the Advocacy Program, including individual advocacy services, local community outreach, and hearing screenings and sign language interpretation services for Pitkin and other regional residents. $2,500 It will be used to purchase food vouchers, bus vouchers and provide limited medical, prescription and utilities assistance to Pitkin County residents in need. $126,000 Partnership allows the agency to continue aligning staffing with increased service needs. $30,000 Provides access to quality mental health and substance abuse services, enhancing recovery and resilience in our community, helping families and individuals lead healthier more productive lives. $500 Ensure a safe, fun and memorable evening for Aspen High School Graduates. $22,500 Provides general operating support for the Aspen Homeless Shelter programs including the Day Center, the Evening Program, and the Winter Overnight Shelter.

2 The Aspen Hope Center, a project of the Aspen Valley Foundation $18,000 Partnership funding contributes to operating support to assist in meeting program goals: crisis intervention services, and coordinating mental health providers who can serve AHC clients. Aspen to Parachute Dental Health Alliance $20,000 The funds would primarily be designated for the salaries for the Regional Oral Health Consultant and the Oral Health Outreach Coordinator, for contract labor (dental hygienists, grant writer), and for supplies for oral health screenings and treatment. Aspen Public Radio $6,000 Continued funding will ensure that Aspen Public Radio will continue to serve Aspen and neighboring communities with the best of local, regional, national, and international public radio programming via FM broadcast, channel 8, podcast, and the Internet. Aspen Science Center $3,000 Grant is to support the goals of hands-on informal educational activities for local kids concentrated in the science, technology, engineering, and mathematics (STEM) fields. Aspen Valley Ski & Snowboard Club $12,500 Provides character-building outdoor winter sports programs and scholarship support to over 2,000 children and adolescents in the Roaring Fork Valley. Aspen Youth Center $18,000 Provides enriching after school, summer and outreach programs for youth in the Roaring Fork Valley $500 Helps provide a safe, drug and alcohol free, fun evening for the Basalt High School graduating seniors. Basalt High School Project Graduation The Buddy Program $30,000 Grant funding will support the five programs serving youth: the community program, school-based, peer-to-peer programs, experiential group mentoring, and Lemonade Day. CASA of the Ninth $5,000 Funding supports recruitment, background checks, training and retention of the Court Appointed Special Advocates (CASA s), who serve as the eyes and ears for the court in cases of abuse and neglect wen children have been placed in foster care in Colorado s 9 th Judicial District. Catholic Charities $15,000 Provides direct financial assistance, short-term case management, and referrals to prevent homelessness and hardship for low income and at-risk individuals and families. Center for Independence $4000 Grant supports for program expansion from Grand Junction into the RFV. Advocate, educate, facilitate, and inform people with disabilities on their avenues for self-sufficiency. Challenge Aspen $9,000 Financial support will allow them to adequately respond to the increased demand for our programming and administrative costs. Used for the Recreational, Educational and Cultural Program. Colorado Fourteeners Initiative Colorado Mountain College Go2Work Programs Colorado West Springs Hospital $5,000 Grant funding will support operating trail reconstruction and/or trail maintenance work on our area peaks. $5,000 Funding will provide Go2Workshops for displaced and incumbent workers, with emphasis on basic skills enhancement and career skills. $20,000 The monies will be used to specifically help offset the cost of serving uninsured patients from Pitkin County. 2

3 Colorado Western Slope College Fair Common Sense Regeneration Project Aspen TREE Community Health Services, Inc. Emergency Assistance Fund for Pitkin County $2,000 Supports overhead expenses of hosting the College Fair. Includes continuing education application for teachers, invitations to colleges, and speaker costs. $2,000 Grant will help develop a 4 season community education greenhouse s members cooperative. $320,000 Provides access to preventative health care services for the vulnerable population in the community. Funding will support existing programs as well as the activities needed to the new mandates for CHS as the designated public health agency for Pitkin County. $40,000 Funding supports the Emergency Services Fund for Pitkin County residents in need of assistance with rent and/or shelter, food vouchers, dental/medical support, prescription assistance transportation and food vouchers. English in Action $15,000 Grant supports program services to help immigrants learn English, strengthen their services, meet strategic objectives and serve more of the students on its waiting list. Executive Service Corps $3,500 Funding will support their operations. ESC assistance is available to any and all nonprofits from Aspen to Rifle. Extreme Sports Camp $2,000 Serves local, regional, and national populations across the autism spectrum through athletic programs. Family Visitor Programs $70,000 Grant request to help in-home support and education services focused on the young child that can begin prenatally and last for Feed My Sheep Ministry Food Bank of the Rockies the first year, for families who live and work in Pitkin County. $4,000 FMS has operated for 9 years serving the homeless and less fortunate. Through funding, their doors are able to stay open day and night except for three hours during the day, giving staff time to restock the Center. Without the Winter Overnight Program many lives would be at risk. $7,000 Grant funding will assist with operating support to help FBR meet their mission of ending hunger by procuring and distributing food through agencies in Northern Colorado, the Western Slope and Metro Denver. Forest Conservancy $13,000 Grant funding will maintain current volunteer operations; recruiting and training. Supports operating costs to bridge the gap between what services, staff and programs the Forest Service can provide. GrassRoots TV $21,000 This grant enables GrassRoots to exist. It helps to assure they can cover overhead: rent, utilities, insurance, technical support, accounting, and supplies. High Country RSVP $4,000 Support to manage programs which utilize retired seniors to assist seniors, low-income seniors, and the disabled through direct programs. HomeCare and Hospice of the Valley $48,000 Grant funding supports operations for the agency including the development of a plan for a residential hospice facility, improved coordination and patient information amongst hospitals, nursing homes, and clinics, work incorporating 3

4 Affordable Health Care Act changes and enhancement of grief support services. Independence Pass Foundation $18,000 Grant funding supports operating costs for developed projects on the Pass. Aids in rehabilitating the Upper Lost Man (Linkin Lake) Trailhead. KDNK $7,000 General operating support for KDNK community radio. LIFT-UP $1,500 The funding from these grants would allow LIFT-UP to keep its food pantries' shelves in Aspen and Basalt stocked all year long to serve 100% of clients with food assistance, promoting selfsufficiency. The Manaus Fund / The Valley Settlement Mountain Family Health Centers Mountain Rescue Aspen, Inc. Mountain Valley Developmental Services $20,000 This grant will aid in the expansion of ability to foster growth towards economic self-sufficiency, improved school readiness aimed at decreasing the achievement gap between Anglo and Latino students, as well as to expand service to Basalt by $75,000 MFHC requested funds for general operating support at the Basalt satellite clinic for the provision of primary care access to Pitkin County residents and workers and increasing behavioral and internal medicine services at Basalt location. $10,000 Grant monies will be used to: a) support team trainings and continuing education b) Technical gear replacement and c) General Working Capital such as office supplies, utilities and general administrative expenses. $70,000 Mountain Valley serves children and adults with developmental disabilities and provides comprehensive supports to the families. Programs offer a depth of service and include residential, enrichment, employment, and medical services, as well as children's programs, an early intervention program, and family support and services program. Mpower $5,000 Funding will go towards general operating support. Mpower partners with five high schools to run Momentum, an implementation of Reconnecting Youth (RY-Model), a sciencebased student support program designed as a semester-long substance abuse, dropout, and suicide prevention class for atrisk high school students. The Neighborhood Clinic at Woody Creek $15,000 The money will support the financing of the clinic, specifically program administration, assisted referrals, assessment supplies and education materials, promotion and outreach. Pathfinders $12,500 Operating support to continue providing comprehensive support Pauline S. Schneegas Wildlife Foundation in cases of chronic illness and grief and loss. $3,000 It will help PSSS Wildlife Foundation, the only facility of its kind on the Western Slope, rehabilitate and return injured/orphaned wildlife to the wild. Professional educational programs on wildlife and environmental issues supported. Pitkin County Senior Services $421,958 Provide senior nutrition, activities, transportation, information and referral and care management programs. Planned Parenthood $7,500 Will support the overall cost of running the Glenwood Springs Health Center, which offers a sliding fee scale and the Clients in Need subsidy fund in order serve a high-need community. 4

5 Reach Out and Read Colorado Provides reproductive health care, comprehensive sex education, and outreach about sexual health and responsibility. $2,000 Funding will support the purchase of new books that go to children during each well-child visit and the role of Regional Coordinator, who works with clinics to ensure program quality by conducting an annual on-site quality assurance visit. RESPONSE $35,000 RESPONSE respectfully requests funding for overall support of its staff, volunteers, and essential community-core services that provide prevention of and assistance with domestic violence and sexual assault. River Bridge Regional Center Roaring Fork Conservancy Roaring Fork Family Resource Centers Roaring Fork Leadership Roaring Fork Outdoor Volunteers Roaring Fork School Health Centers Roaring Fork Valley Early Learning Fund (aka Raising a Reader Aspen to Parachute) Rocky Mountain Human Services Shining Stars Foundation Sopris Therapy Services $10,000 Request is to assist in providing collaborative services to child abuse victims and their families in a supportive environment that reduces trauma and promotes dignity, justice and healing. RBRC also provides community-based child abuse prevention and education. $10,000 Funding will go towards operating support. RFC programs fill the need for: 1. unbiased, pro-active scientific watershed data; 2. watershed education, both for adults and children; 3. preserving and protecting river corridor and wetlands. $40,000 Pitkin County funding will be used for general operating support for the Roaring Fork Family Resource Centers throughout the Roaring Fork School District. $2,500 Roaring Fork Leadership develops leaders for the community. Long term, this transformation generates community improvement through increased civic leadership. $14,000 General operational funding for programs that promote stewardship of public lands by creating volunteer opportunities for trail work and conservation projects. $15,000 RFSHC provides accessible and affordable preventive and primary health care, behavioral health services and education to the students of the Basalt Schools. $11,500 Program provides books and engages preschool-age children in daily book-sharing activities with parents to foster early literacy skills critical for school and life success. $5,000 Dedicated to improving health, self-sufficiency, and quality of life for individuals facing challenges of all kinds. With this grant, we will provide veterans support services through a Western Slope Veterans Coalition whereby public and private initiatives will be coordinated. $6,000 A partnership grant will support the Foundation's services to children with cancer and life threatening diseases and allow an increase in outreach education programs to school districts. $9,000 Rehabilitation services for un-insured, under-insured, and limited income children and adults living or working in Aspen/Pitkin County with special needs who cannot receive treatment due to lack of income or other resources. Spellbinders $3,000 Spellbinders work supports the literacy building efforts of Valley school teachers by building strong, long-term bonds between Valley elders and schoolchildren. 5

6 Thompson Divide Coalition $7,500 The Thompson Divide Coalition will focus on Forest Service Advocacy and Withdrawal, BLM Lease Review and an aggressive grassroots plan to demonstrate the need for protecting the Thompson Divide. Valley Life for All $10,000 VLFA provides opportunities for people with disabilities to lead fulfilling lives and become respected and contributing members of our community. Plans for this coming year include a focus on housing and transportation gaps. Local Leadership Program through PEAK. Valley Partnership for Drug Prevention $38,100 All funds received from Pitkin County will go towards the general operating expenses of VPDP for the purpose of providing prevention education and programming to the residents of the city and county. WE-Cycle $2,000 Brings bike-sharing to Aspen to help transform our transportation landscape into one that prioritizes bicycles and reduces reliance on carbon-dependent vehicles. Wilderness Workshop $16,000 This grant will support the collection of baseline data to provide an early-warning system of adverse impacts to the environment and to minimize impacts of oil and gas development to Pitkin County and the region. WindWalkers Equine Assisted Learning and Therapy Center $9,000 Funding will provide therapeutic horseback riding opportunities for riders with intellectual and physical disabilities or social, emotional, educational and behavioral challenges. Your Friends for Life $6,000 The grant will help with operating expenses in providing assistance to patients and their families. YouthEntity $5,000 This money will support real-world learning experiences for youth that are educated in Pitkin County for the academic year. By providing students with real-world learning experiences that connect them to community and future professions, they can develop a vision for themselves outside of school. YouthZone $70,000 YZ provides prevention and intervention programs to youth and their families in order to remove and prevent youth from the juvenile justice system. Grant Awards Total $1,957,058 6
