6 ATTACHMENT #1 UCI LCDM SCHOLARSHIP REQUIREMENTS 1. Each applicant must spend the minimum $125 LCDM Certificate Candidacy application fee. Mail the LCDM Certificate Candidacy fee directly to UC Irvine, Extension. 2. Each applicant will be responsible for his or her own parking permit fee, books, and materials. 3. Each applicant must be a current member of HBC or be willing to join. Applicants who are not employed by BIA member companies, and therefore cannot join HBC must pay a scholarship application fee that is good for one year. An HBC Application is provided here as Attachment #3. 4. Each applicant must complete each class with a passing grade in order to be eligible for additional scholarship funding in the next quarter. 5. Each applicant must fill out an HBC Scholarship Application each quarter in order to qualify for an HBC Scholarship. 6. Scholarship applications must be received by the following dates: Spring Quarter Instruction begins last week of March Applications due by February 28 Summer Quarter Instruction begins 3 rd week of June Applications due by May 31 Fall Quarter Instruction begins last week of September Applications due by August 31 Winter Quarter Instruction begins first week of January Applications due by December 14 Page 6 of 6

Upon receipt of your completed application materials, the HBC Board of Directors will make a decision at its next scheduled Board meeting, held on the first Monday of each month. 2. The Scholarship Chair or designee will contact you regarding the Board s decision on your application. 3. If approved, the Scholarship Chair will fax your HBC Scholarship Registration Form to the HBC Scholarship Coordinator at UCI Extension. UCI Extension will send confirmation of enrollment directly to you. 4. If you wish to buy a parking permit, contact UCI Extension at (949) to pay by credit card or mail a check payable to U.C.
