4 HIV/AIDS Bureau Vision and Mission Vision Optimal HIV/AIDS care and treatment for all Mission Provide leadership and resources to assure access to and retention in high quality, integrated care, and treatment services for vulnerable people living with HIV and their families 4

5 Purpose To assist applicants to strengthen organizational infrastructure to respond to the changing health care landscape and/or to increase capacity to develop, enhance, or expand access to high quality family-centered HIV primary health care services for low income, uninsured, and underserved WICY living with HIV. Applicants may propose one activity in either: 1) HIV Care Innovation, or 2) Infrastructure Development Please refer to pages 1 to 3 of the NOFO 5

6 6 Award Information Approximately $3,000,000 is available to fund up to 25 grants. Applicants may submit proposals for only one of the following two categories: 1) HIV Care Innovation or 2) Infrastructure Development. Applicants may request funding amounts of up to $150,000 for the one year project period (8/1/18-7/31/19). Requests exceeding this amount will be deemed nonresponsive and will not be considered under this NOFO. Duplication of funded activities is not allowable. Applicants may propose an expansion of an activity currently supported with RWHAP Part D Supplemental or Part C Capacity Development funding; however, HRSA will not fund the same activity in FY 2018 that HRSA funded in FY Please refer to page 5 of the NOFO

7 Eligibility Information Eligibility is limited to current RWHAP Part D WICY funded recipients. Cost sharing/matching is not required for this program. Multiple applications from an organization are not allowable. Please refer to page 6 of the NOFO 7

HIV Care Innovation: HIV Case Finding Motivational Interviewing Patient-Based Treatment Adherence Chronic Disease Self-Management Transitioning Youth into Adult HIV Care Intimate Partner Violence (IPV) Screening & Counseling 2. Infrastructure Development (Expansion and Enhancement): Electronic Health Records (EHR) Financial Management Systems Management Information System (MIS) OR Please refer to pages 1-3 of the NOFO 8

9 HIV Care Innovation HIV Case Finding Train designated staff in HIV case finding techniques through local health departments and/or through the Centers for Disease Control and Prevention (CDC)-funded training centers ( Develop policies and procedures to apply these skills in the clinical setting to link WICY living with HIV into care after HIV testing to address one or more stages of the HIV care continuum. Please refer to page 1 of the NOFO 9

10 HIV Care Innovation Motivational Interviewing Train staff in motivational interviewing to engage patients in care. Staff may receive training through the local AIDS Education and Training Centers (AETCs) or other resources. Develop policies and procedures to facilitate staff application of the training in the clinical setting to address one or more stages of the HIV care continuum. Please refer to page 1 of the NOFO 10

11 HIV Care Innovation Patient-Based Treatment Adherence Implement an innovative patient-based treatment adherence program supported by policies and procedures to provide long-term adherence support for chronically nonadherent patients, in particular patients with mental health and substance abuse issues, to address one or more stages of the HIV care continuum. For resources addressing this topic, access the Knowledge Network ( located on the Substance Abuse and Mental Health Services Administration (SAMHSA) website. Please refer to page 2 of the NOFO 11

12 HIV Care Innovation Chronic Disease Self-Management Institute a clinic-wide chronic disease management program for HIV/AIDS based on the Stanford program or other resources for patient self-management (e.g., to engage patients in longterm disease control to address one or more stages of the HIV care continuum. Develop policies and procedures to apply the program. Please refer to page 2 of the NOFO 12

13 HIV Care Innovation Transitioning Youth into Adult HIV Care Implement transition planning activities that include, but are not limited to, written policies, procedures, and staff training to assist youth in transitioning from pediatric to adult HIV medical care. Transition planning is a RWHAP Part D program requirement; therefore, this activity should focus on innovative approaches that build organizational capacity to effectively implement and manage the transition for the youth population (ages 13-24) and minimize negative impacts. The activity must address one or more of the stages of the HIV care continuum. Please refer to page 2 of the NOFO 13

14 HIV Care Innovation Intimate Partner Violence (IPV) Screening & Counseling Implement IPV screening and counseling in the clinical setting and establish referral networks to community-based social services organizations. The activity must address one or more of the stages of the HIV care continuum. Please refer to page 2 of the NOFO 14

15 Infrastructure Development Electronic Health Records (EHR) Implement enhancements to or an expansion of existing EHRs to improve the quality, safety, and efficiency of patient health care. This does not include the purchase of an EHR. Describe the plan to enhance or expand the EHR and describe how this increases the capacity of the organization to support increased quality of care for WICY living with HIV. Please refer to page 2 of the NOFO 15

16 Infrastructure Development Financial Management Systems Implement enhancements to or expansion of an existing financial accounting system or software. Develop protocols and billing policies based on the use of this enhanced system and illustrate how the activity will address and increase the capacity of the organization to respond to changes in the health care environment. Please refer to pages 2 to 3 of the NOFO 16

17 Infrastructure Development Management Information System (MIS) Identify, establish and strengthen administrative, managerial, and MIS structures to offer, enhance, or expand comprehensive HIV primary healthcare. Activity should describe how the activity will increase the capacity of the organization to respond to changes in the health care environment and support increased quality of care for WICY living with HIV. 23 Project Narrative - Introduction Brief description of the purpose of the proposed activity. Clearly state the proposed activity within HIV Care Innovation or Infrastructure Development. Discuss why your local community and/or organization is in need of supplemental funds. Clearly describe how the proposed activity builds upon and furthers the objectives of the previously funded activity in maximizing impact. Please refer to page 8 in the NOFO 23

24 Project Narrative - Needs Assessment HIV Care Innovation activities: Describe and document the target population(s) and their unmet health care needs. Describe the unmet need based on your evaluation of the gaps in the HIV care continuum for your target population(s) living with HIV being served by your organization. Provide data on the five stages of the HIV care continuum for your target population(s) living with HIV using the most recent three calendar years of available data. Clearly define the numerator and the denominator for each stage. Please refer to pages 8 to 9 in the NOFO 24

25 Project Narrative - Needs Assessment For Infrastructure Development activities: Describe the gaps in organizational capacity. Provide specific information to the selected activity. Describe the impact of how these gaps or limitations are affecting the optimal provision of quality HIV primary care services. Please refer to pages 8 to 9 in the NOFO 25

26 Project Narrative - Methodology Propose methods that will be used to address the selected activity; Identify collaboration and coordination efforts and strategies; Describe proposed plans for how the activity will be continued beyond the project period; and Provide information regarding PLWH involvement in the activity, collaboration and coordination efforts, and sustainability plans. 27 Project Narrative - Work Plan Describe the activity s goal, specify the objectives, and identify the key action steps that will be used to achieve the proposed activity for the 12-month period of performance of August 1, July 31, Applicants should list all action steps that will be necessary to implement the proposed project and accomplish the proposed objectives. 29 Project Narrative - Resolution of Challenges Discuss any challenges likely to be encountered in designing and implementing the activity described in the work plan, and in measuring improvement in the HIV care continuum. Discuss approaches that will be used to resolve identified challenges. Challenges should be specific to the proposed activity and relate to overall goal(s) or objective(s) proposed in the work plan. Please refer to page 10 of the NOFO 29

30 Project Narrative Evaluation and Technical Support Capacity Data Collection and Management Describe data collection systems and the methods used to collect and monitor the outcomes of the proposed activity in the work plan. Outline how the organization will collect, manage, and analyze data. Project Evaluation Describe evaluation and CQM program activities to assess the impact. Describe the plan for the program performance evaluation that will contribute to continuous quality improvement. HHS core/hab HIV indicators are recommended. Discuss how evaluation results will be disseminated to staff, consumers, and the community. Please refer to page 10 of the NOFO 30

36 Salary Limitation Please note that effective January 8, 2017, the salary rate limitation applicable to RWHAP domestic grants and cooperative agreements increased from $185,100 to $187,000. The Consolidated Appropriations Act, 2017, Division H, 202, (P.L ), Division H, S 202 states, None of the funds appropriated in this title shall be used to pay the salary of an individual, through a grant or other extramural mechanism, at a rate in excess of Executive Level II. Please see Section 4.1.iv Budget Salary Limitation of HRSA s SF-424 Application Guide for additional information. As a reminder, RWHAP funds and program income generated by RWHAP awards may not be used to pay salaries in excess of the rate limitation. 36

37 Salary Rate Limitation Example Individual s full time salary: $255,000. Amount that may be claimed on the federal RWHAP award due to the legislative salary limitation: Individual s base full time salary adjusted to Executive Level II: $187,000 28

40 Application Review Information The HAB s Division of Community HIV/AIDS Programs will review each application for eligibility, completeness, accuracy and compliance within the requirements outlined in the NOFO. HRSA may elect not to fund applicants with management or financial instability that directly relates to the organization s ability to implement statutory, regulatory or other requirements. Following review of all applicable information, HRSA s approving and business management officials will determine whether an award can be made, if special conditions are required, and what level of funding is appropriate. Note: Award decisions are discretionary and are not subject to appeal to any HRSA or HHS official board. Please refer to pages 16 to 18 of the NOFO 40
