8 Purpose and goals for FY15 FOA Purpose: Expand delivery of health care services in local and regional rural communities Goals: 1. Expand the delivery of health care services to include new and enhanced services exclusively in rural communities 2. Deliver health care services through a strong consortium, in which every consortium member organization is actively involved and engaged in the planning and delivery of services 3. Utilize and/or adapt an evidence-based or promising practice model(s) in the delivery of health care services 4. Improve population health, demonstrate health outcomes and sustainability 8

12 Summary of FY15 FOA Funding Estimated number of awards: up to 80 grants Estimated award amount: up to $200,000/year Project period: May 1, 2015 April 30, 2018 (3 years) 12

13 Eligibility Lead applicant must be: Rural and, This can be verified at aspx Non-profit or public entity, and Represent a consortium/network of 3+ health care providers Definition of health care provider is found at bottom of page 5 of HRSA The lead applicant (if awarded, will be the grantee of record) Must have financial and management systems in place 13

14 Consortium Definition of consortium found on page 6 of HRSA Only one member will serve as the applicant of record (must meet the lead applicant eligibility requirements) Other members can be non-profit/for profit, rural and/or urban Consortium composition must have at least 3 separately owned health care providers (each must have their own EIN from the Internal Revenue Service) Memorandum of understanding/agreement will need to be submitted with application 14

15 Am I still eligible to apply if I m a current or former grantee? Yes, as long as: Proposed project is a new proposal or an expansion/enhancement of previous grant This is not an opportunity to maintain current operations of previous grant See page 7 of HRSA for more information 15

17 Funding restrictions As listed on page 27 of HRSA (FOA): 1. To build or acquire real property or for construction or major renovation or alteration of any space; 2. To purchase equipment above 40 percent of the Federal share of funding for each budget period; or 3. To provide inpatient care. Also see pages of HRSA SF 424 Application guide 17

33 Paint the right picture Remember your audience: The panel of reviewers Reviewers typically do not know anything about your situation, your community, or even your state Explain basic facts: rural and frontier as a concept (distance, weather, roads); culture; and your unique circumstances Central goal = convince reviewer of the legitimacy of your problem, your solution, your ability Think about how your project will support all four goals of this FOA 33

36 Ways to help applicants Applicants are required to notify SORH of their intent to apply. This is an opportunity to engage with the applicants. Ideas for involvement: - Offer Technical Assistance to the project - Offer to read/critique/review the written application - Offer rural health data to support the application - If the application is successful, offer technical assistance to the project after the award Share information about the grant opportunity with the rural health community in your state. Perhaps even hold your own technical assistance webinar. 36
