This is not a request for proposals. The project is targeting a LEED silver rating.

2 1.2 Scope of Services: Provide professional services in construction inspections and materials testing associated with but not limited to: earthwork, cast in-place reinforced concrete, masonry, structural steel, wood and asphaltic materials, including the processing of both field and laboratory data. 2.0 General Information: CWU reserves the right to continue with the consultant selected or has the option to conduct a new selection process for future services beyond those services advertised above. 2.1 RFQ Coordinator: Name: Address 01 (USPS): Address 02 (Personal Deliver, FEDEX, UPS): Lathan Wedin, Capital Planning & Projects Architect 400 East University Way 205 East 11 th Avenue City, State, Zip Code: Ellensburg, Washington Phone Number: Address: Lathan.Wedin@cwu.edu During preparation, direct all communications with regarding this project to Lathan Wedin, Capital Planning & Projects Architect, telephone or via Lathan.Wedin@cwu.edu. All other communication will be considered unofficial and non-binding on. 2.2 Selection Process: Firms will be selected in a two-phase process: Phase 1 - short listing of firms based on submitted qualifications. Phase 2 - in person interviews of short listed firms. Firms will be considered for interviews based upon the following criteria as indicated: Letter of Interest Qualifications of Key Personnel Laboratory Accreditations & Certifications Demonstrated Relevant Experience in Construction Inspections and Material Testing Diverse Business Inclusion Plan Professional References 2 of 5

3 2.3 Submittal Requirements: Qualifications to be submitted in bound 8-1/2 x 11 format; double sided preferred. Each of the following parts must be indexed and included per the following order: 1. Cover Sheet with Response Title 2. Table of Contents 3. Cover Letter of Interest and Availability 4. Higher Education Heavy Construction Testing & Inspection Experience 5. Laboratory Accreditations and Certifications 6. Demonstrated Sustainability Expertise 7. Diverse Business Inclusion Plan 8. Qualifications of Key Personnel 9. Professional References 10. Standard Set of Federal Form 330 Submittals shall include each item enumerated above and specifically address expertise with similar related higher education projects. Firms selected as the most highly qualified, based upon an evaluation of the data submitted, will be contacted to schedule interviews with the selection committee. Partial submittals will not be considered. Provide three (3) hard-copies of completed qualifications. Provide one (1) reproducible digital copy in pdf format delivered on digital media. No qualifications, hard-copy or digital, will be returned to Respondent. 2.4 Proprietary Information / Public Disclosure: Materials submitted in response to this solicitation shall become the property of Central Washington University. Responses shall be deemed public records as defined in RCW 42.56, Public Records Act. Any information in the response that the desires to claim as proprietary and exempt from disclosure under the provisions of RCW be clearly designated. CWU will consider the request for exemption from disclosure. Marking the entire response exempt from disclosure will not be honored. 2.5 Revisions to the Process: In the event it becomes necessary to revise any part of this request, addenda will be published on Central Washington University s web site, under an Addenda hyperlink, at: Costs to Submit: will not be liable for any costs incurred by the Respondent in preparation of a response to this RFQ, in conduct of a presentation, or any activities related to responding to this RFQ. 2.7 Americans with Disabilities Act: complies with the Americans with Disabilities Act (ADA). Respondents may contact the RFQ Coordinator to receive this Request for Qualifications in another suitable format. 2.8 Diverse Business Inclusion Plan: (CWU) is committed to providing the maximum practicable opportunity for participation by diverse businesses enterprises (DBE). DBE are defined as; small-business, microbusiness, mini-business, minority owned business (MBE), and women owned business (WBE), as defined in RCW and veteran-owned businesses as defined in RCW 43.60A of 5

4 The following voluntary numerical diverse business inclusion goals have been established for the project: 10% MBE, 6% WBE, 5% Washington Small Business and 5% Veterans. Achievement of these goals are encouraged. However, no minimum level of diverse business participation shall be required as a condition of consultant selection. Proposals will not be rejected or considered non-responsive if they do not include diverse business participation, but a plan for diverse business inclusion is required. Upon request by CWU, contractor/consultants/vendors will be required to provide a report of the actual inclusion efforts undertaken to utilize DBE for any CWU goods and services contracted. All firms, including diverse owned firms, must submit a copy of the firm s Diverse Business Inclusion Plan. The firm s Diverse Business Inclusion Plan should address, at a minimum, the following: A. Commitment of the owner(s) and senior leadership of the firm to reaching out to contract with DBE and in helping to meet or exceed CWU s DBE utilization goals. B. Provide the date your Diverse Business Inclusion Plan was adopted by your firm. C. Individual names and titles of positions responsible for managing and implementing specific aspects of your Diverse Business Inclusion Plan. For each individual listed, include how long they have been involved with diverse business inclusion efforts with your firm. D. Provide a description of the strategies, approaches, and specific steps your firm will take to help meet or exceed the state s diverse business percentage utilization goals, which may include but are not limited to a description of the following: 1. The education and training program used by your firm to communicate to your employees your firm s expectations relative to implementing the Diverse Business Inclusion Plan. 2. The project-specific outreach your firm utilizes in identifying and contacting DBEs through media, government agencies, industry and non-profit associations and organizations, referrals, and relationships. 3. Describe your DBEs outreach/mentoring program, if any. Describe any mentoring of DBEs that your firm has been involved. Please identify how long your firm has been mentoring DBEs, and how effective the mentoring has been for DBEs. E. Describe how your firm will monitor its progress toward helping to meet or exceed the state s diverse business goals on the contract to be awarded based on this RFQ, and how your firm will adjust its strategy as necessary in order to improve its performance in this area. F. Solicit at least one DBE firm per sub consultant category. While CWU is interested in contracting with firms who employ a diverse workforce, this should not be part of the Diverse Business Inclusion Plan. The focus is on inclusion efforts to contract with DBE to the maximum extent possible. 2.9 Rejection of Responses: reserves the right at its sole discretion to reject any and all responses to this RFQ without penalty. 4 of 5
