3 FOREWORD This brochure has been put together as a visual aid to assist clinicians and buyers in identifying those brands awarded on the All Wales Wound Management contract. Its contents are not considered commercially confidentially (unlike most other contract documents) so it can be more widely distributed. The brochure includes all items available via the contract but local policy may dictate additional approvals that are required for the use of certain dressings or may rationalise the number of dressing options available. The brochure is not intended to supersede or otherwise conflict with local formularies and Health Board protocols and procedures in relation to the use of specific dressings should always be followed. In addition, as the brochure covers only those lines available via the Wound Management contract, it does not include other categories of associated items that may be present on other All Wales contracts. Further information on other associated contracts can be sought from NWSSP Procurement Services using the contact details in this brochure if required. 55 Appendix C Medical Device Defect Reporting Procedure Complaints referring to the quality of medical products should be forwarded by the customer to the Surgical & Materials Testing Laboratory (SMTL) who are responsible for investigating defective products within NHS Wales. In general, SMTL will investigate any medical disposable device purchased for use in the Welsh NHS. Examples are given in Appendix C1. If the device you are interested in is not included below, please contact the lab to find out whether SMTL can help. The Procedure: 1. The user completes a SMTL Medical Device Defect Reporting Form (See Appendix C2) A copy of this form can be obtained from the SMTL website 2. The completed form and where possible, the defective product in question is forwarded to the Local Health Boards/Trusts Defect Reporting officer (DRO). A list of the Local Health Board/Trusts DRO can be found in Appendix C3. 3. The DRO forwards the complaint form and any samples to SMTL for investigation and testing (where appropriate). For information and advice regarding the sending of contaminated medical devices to SMTL, please refer to the following document Postage of contaminated medical devices which is available via the SMTL website via the above link. 4. Following investigation and testing (if required) SMTL will produce a Report regarding its findings which will be forward to the /Manufacture for the comments and response. 5. If the device is contaminated and it is not possible for the device to be decontaminated locally (or there are concerns that decontamination could destroy important evidence) follow one of the 4 options below: a) Package appropriately and organise transfer of the package to SMTL via the hospital transport system (contact hotel services, post room or pharmacies for more details about local hospital transport arrangements). b) Package appropriately and pass on to a member of SMTL staff during a meeting (SMTL staff attend regular meetings with other NHS staff throughout Wales and devices could be passed on during these occasions). c) Package appropriately and send to SMTL via UN approved courier. d) Label item, place in quarantine and contact SMTL for advice regarding transportation. For further information and details on the sending of defective and contaminated devices to SMTL, SMTL have produced a document Postage of contaminated medical devices which evaluates the various options open to Welsh NHS staff when they have a defective device they wish to send to SMTL. 59
