1 BUSINESS AFFAIRS PLANNING DESIGN & CONSTRUCTION Construction Manager Qualifications Supplement (CMQS) GENERAL INSTRUCTIONS UF-619, IBC & ILHC New Facility A) Submit one (1) hard copy of the proposal plus one electronic (PDF) copy as outlined in the Project Fact Sheet by the time and date stated on the Project Fact Sheet. B) Assemble proposals in a manner that facilitates disassembly & recycling and minimizes the use of plastic covers, binders, and dividers. C) Include a Letter of Interest and (11) tabbed & divided sections as follows, attaching additional pages for each section as necessary: Company Information and Certification (CMQS 0) Personnel (CMQS 1) including resumes and organization chart if necessary Experience & References (CMQS 2) Construction and Project Management Questions (CMQS 3-5) Commissioning and LEED (CMQS 6) Safety (CMQS 7) Vendor Diversity (CMQS 8) Project-Specific Questions (CMQS 9-11) Workforce Development (CMQS 12) Joint Venture Information (CMQS 13) if applicable Attachments / Supplements including license(s), proof of corporate status, and proof of bonding capacity D) Forms may be retyped or re-sized to provide additional information or pictures, provided that the requested information is provided, and in the order requested. These Instructions need not be submitted. E) Number each page consecutively, including the letter of interest, CMQS forms, questions & answers, and all attachments, licenses, résumés, supplemental information, etc. The entire proposal shall be limited to forty (40) 8½ X 11 pages. Covers, table of contents, and divider tabs will not count as pages, provided no additional information is included on those pages. Any pages over the stated limit will not be considered as part of the application. F) The Letter of Interest should concisely outline both your understanding of the IBC & ILHC New Facility project and the characteristics of your firm(s) and proposed team that make them uniquely qualified for it. Address the letter to the attention of the UF Project Manager. G) Use the project-specific CMQS 0 form to provide general company information and to formally certify the proposal. H) For CMQS 2, use the enclosed form and follow the Submittal Instructions. For the questions in CMQS 3-12, precede each answer with a re-statement of the given question or request. If the question on joint ventures (CMQS 13) does not apply, simply state NOT APPLICABLE. I) Enclose resumes, LEED accreditation, and other pertinent credentials for all proposed staff. J) Applicants operating as a corporation, limited liability company, or partnership must be registered to operate in the State of Florida by the Department of State (Division of Corporations) at the time of application. Provide proof of such status (if applicable) for the applicant. Such proof shall take the form of a Certificate of Corporate Status from the Florida Department of State. If paid fees due this office through date has passed, ensure certificate print date is within 10 calendar days of the proposal due date. PDC REVISED: MAY 2016 PAGE 1 OF 5

2 K) Enclose a copy of the applicant firm s (not individuals ) current general contracting license from the Department of Business and Professional Regulation. L) Provide a letter of intent from a surety company indicating the applicant's bonding capacity for this project. The surety shall acknowledge that the applicant may be bonded for each phase of the project, with a potential maximum construction cost of $3,854, The letter of intent shall confirm that the Surety Company is licensed to do business in the State of Florida, has a Best Rating of at least "A, and has a financial size of "Class XV." M) Only individual firms may apply. Two firms may not apply jointly unless they have formed a joint venture, and a copy of that formal agreement shall be provided with this proposal. See CMQS 13. N) Applicants should take care in assembling the proposals, as professionalism and attention to detail are among the intangible qualities the Selection Committee will consider. O) Incomplete proposals will be disqualified. This includes, but is not limited to: Failure to submit proposal by the stated time and date Failure to sign the CMQS 0 certification form Missing, invalid, or expired firm license Missing, invalid, or expired proof of corporate status in the State of Florida Insufficient proof of bonding capacity or surety rating Illegible P) See the UF-619 page of the Planning Design & Construction website ( for more information on this project and the CM selection process. 3 Construction Manager Qualifications Supplement (CMQS) SUBMITTAL INSTRUCTIONS UF-619, IBC & ILHC New Facility NOTE: The numbers on these instructions correspond to the numbered items on the University of Florida Construction Manager Qualifications Supplement (CMQS) forms. 1. APPLICANT'S PERSONNEL Name all personnel who will be part of the pre-construction and construction management team for this project. Identify each person s title, project-specific responsibilities, and city of residence. Also identify other non-essential positions planned for this project that are not, as yet, filled or named. Enclose résumés for all key staff and discipline heads tailored to demonstrate their experience as it relates to this project. If any consultants are included as part of the proposed team, provide resumes for the specific personnel proposed for this project and describe each consultant's proposed role in the project and its related experience. Provide an organization chart or diagram if necessary to clearly explain lines of authority, duties & responsibilities, continuity through design, construction, and post-occupancy, etc. The team proposed in the CMQS must be available to provide the services for the project, throughout the project. If the Applicant discovers prior to the interview that any part of the listed team (either individual key staff or consultants) will not be available, it shall notify the selection committee immediately. The selection committee will determine whether the change in the team would have affected the Applicant's shortlist score. If the change would lower the score, the Applicant may be removed from the shortlist. Once awarded the contract, the Applicant will not be permitted to alter its team without the Owner's approval. 2. EXPERIENCE / REFERENCES Submit up to ten most related projects for which the Applicant provided (or is providing) CM, GC, D/B, or other construction management services. In determining which projects are most relevant to the UF-619, IBC & ILHC New Facility project, consider the unique aspects of this project, including tight site logistics/laydown/sub parking, GRU/DOT and other utilities coordination, the delivery method, the demands of constructing on a bustling campus, and other project-specific issues as conveyed in the selection criteria outlined in the Project Fact Sheet. Also consider the Owner and location, relative size and cost, building/space types, complexity, staffing (how many members of the proposed team worked on the listed project?), currency (how recently was the listed project completed?), and performance metrics (schedule, budget, quality). Use the attached form (one two-page form for each of the ten projects) and provide all requested information as follows: Note the title and location of the project; indicate the services provided; and complete the other general project information (status, size, value, delivery method, etc.). Provide the names of all key staff whether construction, design, or both and check YES or NO to indicate whether each person is also on the proposed team for the IBC & ILHC New Facility project. Provide brief narratives on the facility and its relevance to the IBC & ILHC New Facility project. Provide contact information for the Owner, designer or builder (whichever applies), and the (3) largest subcontractors by value of trade package. Verify that the contact information is still valid (names and numbers). Add, where applicable, contact information for the current Owner s representative, if different from the Owner's representative at the time the applicant s services were provided. List the projects in priority order, with the most relevant project listed first and the least relevant project listed last. INCLUDE NO MORE THAN 10 TOTAL PROJECTS. PDC REVISED: MAY 2016 PAGE 3 OF 5

4 CONSTRUCTION and PROJECT MANAGEMENT QUESTIONS 3. Describe the team s approach to, and methods for, cost estimating during the pre-construction phase and cost control during the construction phase. 4. What means will be used to identify, notify, pre-qualify, and procure sub-contractors for the unique and/or exacting disciplines involved in this project? 5. The value added by CMs during the pre-construction phase has often been limited to estimating, schedule development, and site logistics planning. Detection of coordination & constructability issues and true value engineering to assist the A/E in finding less expensive approaches to achieving the same or better quality & performance are services that are not frequently delivered well. Using specific examples by the staff proposed for this project, outline the teams past performance with these and other value added services. Further, explain the roles and level(s) of participation of your proposed field staff (PM, Supt., etc.) during pre-construction. 6. COMMISSIONING and LEED a. Describe the experience of the proposed field staff with building commissioning. b. Enclose copies of proposed staff s LEED credentials including any experience with Version 4. Describe the experience of the proposed staff with sustainable design and construction and LEED certification efforts. See Project Fact Sheet for more information. 7. SAFETY Safety on and around the jobsite is the responsibility of the Builder. The University of Florida is interested in not only the safety of the construction workers, but also the public, including UF students, faculty, staff, and visitors. (a) Explain your approach to safety, including oversight, education, and enforcement. (b) Assuming you have a standard safety plan or manual, describe how it will be tailored to working on an active campus with particular attention to the interaction between construction activities and the public. 8. VENDOR DIVERSITY The University of Florida strives to ensure equal access for Small, HUB Zone, Minority, Small Disadvantaged, Veteran, Service-Disabled Veteran, and Woman-owned businesses. Outline successful past efforts in this area, and elaborate on project-specific plans and ideas for engaging, attracting, and employing a diverse pool of trade contractors and vendors. Questions 9-10 are specific to the IBC & ILHC New Facility project. 9. Describe your familiarity with Alachua County and specifically the local municipalities such as local utility groups, DOT, zoning requirements etc. and their processes. 10. Describe your methodologies on working within a tight site, including subcontractor parking, lay down area etc. 11. WORKFORCE DEVELOPMENT The construction workforce is shrinking nationally at an alarming rate, particularly in trades requiring a high degree of skill and craftsmanship. Discuss your company s tangible efforts to recruit, train, and retain the next generation of workers, including requirements and initiatives aimed at the trade contractor level. 12. JOINT VENTURE APPLICANTS ONLY Duplicate the Certification form (CMQS 0) signature block and have a principal or officer sign on PDC REVISED: MAY 2016 PAGE 4 OF 5

5 behalf of each party to the joint venture. Enclose a copy of the joint venture agreement, which specifically states the percentage of fee to be earned by each party and each party's role in the project. Provide answers to the following questions: (a) Describe the division of responsibilities between the participating firms, the offices (location) that will be the primary participants, and the percent interest of each firm. (b) Why does the Applicant feel that a joint venture will best serve the needs of this project? (c) How many projects has the joint venture performed together?
