Accountability that spans levels of care must include the process of case management. Case managers are accountable for how the patient moves through the continuum. This session will outline how case managers can demonstrate accountability. 1. Define accountability from a case management perspective 2. List regulations that drive case management practice with a focus on accountability across the continuum 3. Identify strategies case managers can use to increase their value in the organization s accountability Session 2: State of Health Care in Virginia 2012 Update Paul A. Speidell Vice President Virginia Hospital & Healthcare Association Regulatory and legislative changes impact the dynamics of health care, and case management plays a vital role in an organization s adaptation and response to these changes. This session will present current and recent health policy reforms, challenges, and changes, including discussion regarding the impact to hospitals and case management. 1. Identify three components of current health care and regulatory issues that will affect hospitals in the coming year 2. Describe the impact of policy changes on hospital case management and the state of health care nationally and in Virginia 3. This session will provide information on the work and accomplishments of this committee. The speaker will present cases that illustrate ethical dilemmas that are faced in this patient population. The session will further illustrate how the ethical principles are applied to these cases and present the outcomes. 1. Describe ethical issues that are unique to the pediatric population 2. Detail the ethical prinicples that elucidate the understanding of pediatric ethical issues 3. Discuss how ethical dilemmas challenge us as health care providers Session 5: Challenging Complex Cases Panel Presentation Steve Lanman, LCSW, ACM Coordinator Care Management Bon Secours St. Mary's Hospital Teresa Mitchell, MSW Clinical Social Worker Care Management Carilion Clinic Representatives from hospitals across the state will share their experiences with challenging, complex cases regarding ethical and discharge planning issues. Elements and factors impacting care planning and delivery such as cultural, financial, and complex care needs will be described. Structures, resources and processes from the various hospitals will be included as well as an opportunity for discussion with questions from conference participants. 1. Describe experiences and challenges with complex cases, including identification of care resources, and resolution alternatives 2. Identify selected structures and processes for potential application in care 3. Discuss alternatives to address cultural, financial, and complex care elements
