Patients ae able to test thei own blood at home, and send thei INR esult diectly to the pactice via an app called engage. Rosalee is a patient at Pendleside Medical Pactice. He docto, D Lucy Astle, talked to us about implementing the poject at Pendleside, which is pat of a wide poject involving 5 pactices in East Lancashie CCG, and suppoted by the Innovation Agency. We wee stuggling with ou anticoagulation clinics. They wee vey busy and time consuming. It seemed thee was potential to educe the time commitment fo both patients coming in and the nuses unning the clinics. Some of D Astle s patients wee aleady self-testing using thei own devices. Howeve they still needed to speak to the pactice nuse to convey thei INR esult, and discuss any issues. engage Patients, especially woking-aged patients could spend a lot of time tying to speak to the pactice nuse.
The engage app is a solution to this poblem. The patient sends the INR esult diectly to the pactice s wafain-monitoing system, INRsta, along with answes to safety questions and any othe infomation they wish to povide. This infomation is also filed back to the clinical system, thus poviding a continuous naative. The educational aspects of the app and digital tutoials help patients to incease knowledge of thei condition and medication. They hopefully bette undestand what they ae doing and why they ae doing it. education
The poject aleady had some unexpected benefits, paticulaly fo patients who had needed to stop wafain on a tempoay basis. Rosalee has had seveal pocedues ecently which has meant testing he INR egulaly, sometimes on a daily basis. Peviously she would have tavelled to the pactice to do this, but because she is self-testing, she has been able to test and send he own eadings fom the comfot of he own home. Rosalee Stevenson is a 66 yea old patient at Pendleside Medical Pactice. She has been taking wafain fo atial fibillation fo the last 6 yeas. She has found self-testing to be invaluable. We asked Rosalee whethe she would ecommend self-testing to othe patients on wafain.
It s so simple. It s given me peace of mind, and I know it s thee in case I don t think things ae ight. If I go to the dentist o have a shoulde injection, I ve got to stop wafain. With self-monitoing I know I can come back and test my INR, and get it back to the level. It s just peace of mind. I would ecommend it to anybody. It s wondeful.
How easy it is to enable patient access to connected self-cae? D Astle told us that thei appoach to offeing connected self-cae to patients had been completely open. I confim I am eady to use eady not sue We decided not to make ou own judgements about which patients we thought would be good at self-testing. We offeed it as an option to all of ou patients on wafain, and have sometimes been supised with the outcomes. Many of ou olde patients within the poject have been as enthusiastic and successful as ou younge patients who we may have expected to be moe tech-savvy.
What we say to ou patients is thee is an option to monito you wafain at home and online. Why not go away and have a look at the taining to see if you like it, and if you do, have a go. This hopefully helps to empowe patients and pomote self-management fom the stat. We want ou patients to feel well and stay healthy. Although they may have medical conditions needing wafain, we want to be able to educe the medicalisation of this, and fo them to be able to manage thei medication fom home o on holiday!

What has been the biggest advantage to self-monitoing so fa? Flexibility and independence fo the patients aound thei wafain theapy, followed by the potential to impove patient cae. We have aleady seen some impovements in a measue used to assess the effectiveness of wafain theapy called time-in-theapeutic ange. In Rosalee s case he wafain contol is bette now than it has eve been, and we hope this esult could be seen in othe patients self-testing.
