We pride ourselves on delivering care to the highest standards. We believe in person-centred care. This means we try to learn as much as possible about each individual person who lives with us, so that we can provide them with care and support tailored to their individual needs and preferences. Working closely with Dementia UK We are among the first care providers to employ our own specialist Admiral Nurses and have been leaders in exploring other innovative ways of caring for people living with dementia, such as themed destination areas and meaningful activities. Compassionate, professional and friendly New in 2018 Grace Care Centre is a 70 bed dementia and nursing care centre split over three floors incorporating a number of features to promote reminiscence as therapy. 03
Your New Home Grace Care Centre offers up to 70 residents individual and compassionate dementia, nursing and respite care. Our brand new home is situated in a quiet residential area of Thornbury, South Gloucestershire. Grace Care Centre specialises in: Wellbeing and comfort The home is arranged in five household units, each having their own homely living area, with sitting room, kitchen and dining area. The large single bedrooms all have en suite shower and toilet facilities, although assisted bathrooms are also provided in each household. All rooms have access to digital TV, WiFi as well as a 24-hour nurse call facility. Dementia Care Nursing Care Additional features of the home include The Pavilion café, craft room, cinema and hair and beauty salon. The external area provides safe and secure gardens with access from each of the ground floor households as well as communal gardens accessible from the cafe area and living rooms. Respite Care A friendly, professionally trained team To give you the support you need when you need it, a friendly, professionally trained team are on hand around-the-clock to support you in how you wish to spend your day enabling you to enjoy a fulfilling and independent lifestyle. Tasty, Home Cooked Meals A talented chef and kitchen team prepare tasty, home-cooked meals. The freshly prepared, delicious dishes are carefully balanced to get the best nutritional value, and cater for individual dietary requirements and preferences. Seasonal produce is chosen whenever possible and of course, relatives and friends are always welcome to join. The menus change daily and the kettle is always on, so hot drinks are available throughout the day. A personal dining experience What this includes: Nutritional meals Friends and family can join My Mum appears much younger since moving to Grace as she has gained weight. We are also happier and have more quality time with Mum since she moved to this Care Home as she is more settled and less confused. Each of the 5 households have their own small and personal dining room where residents take their meals if they wish. Residents are also encouraged to help make their own food, and to make snacks whenever they feel hungry. Snacks readily available Hot drinks when you want 08
Activities to Enjoy We understand how important it is to have daily activities to take part in and enjoy. Our dedicated Activities Co-ordinators organise a wide programme of events around each persons requests, hobbies and interests. The team will be delighted to assist you to continue with existing hobbies, as well as give you the opportunity to try something new if you would like to. They make sure everyone can enjoy themselves to the fullest, be that with arts and crafts, a coffee morning or more individual pursuits. Visits from outside entertainers happen regularly and the home also has its own minibus for trips out and about. Each resident receives 1-1 time with the Activity Co ordinators and together they will work on their own personal activity plan and goals. Home Manager As head of the home, the manager ensures the smooth running of the home and that all the team are providing the best possible environment for every resident. Head of Care Experts in all aspects of care, the Head of Care is focused on the care delivered in the home and quality of life for all residents. Activity Co-ordinators Providing a wide programme of events, a personal activity plan with goals and delivering activities that are meaningful and resident focused. Dementia Lead Responsible for ensuring everyone is up-to-date on the latest learning on dementia and for championing the best quality care for dementia residents. Carers Our professional and expertly trained carers are on hand around the clock to ensure all residents enjoy a fulfilling and independent lifestyle. Admiral Nurse Specialist mental health nurses supported by Dementia UK, who are available to support the care team and families living with dementia. 12 Registered Nurses Qualified, professional nurses are on hand 24 hours a day in parts of the home, to tend to any nursing requirements. Housekeeping Team Maintaining a spotlessly clean home with a smile, all our Housekeeping Team ensure the home is spick and span in your rooms and in shared home areas. Chef All our Chefs enjoy producing healthy, home-cooked meals for our residents, and are on hand if anything is needed for a special occasion or celebration.
